New tools for activating endodontic irrigants have evolved, yet their impact on root canal disinfection, in comparison to the passive placing of an inter-visit medication, have not yet been fully elucidated. The use of DNA- and rRNA-based methods may cast some new light on this issue, as they allow a comparison to be made between microbial presence and activity. Therefore, the aim of this single-arm intervention trial is to evaluate the antibacterial effect of endodontic procedures using both molecular methods. Root canal samples were obtained from 20 patients with asymptomatic apical periodontitis after each treatment step: access cavity, chemo-mechanical preparation, adjunctive procedures (XP-endo Finisher file and passive ultrasonic irrigation), calcium hydroxide medication, and 2nd-visit root canal preparation. DNA and cDNA from the samples were subjected to quantitative polymerase chain reaction with universal primers for the bacterial 16S rRNA gene. Chemo-mechanical preparation promoted a drastic reduction in bacterial levels and activity, whereas the adjunctive procedures did not make a significant contribution to further disinfection. At the 2nd visit, bacteria were active after the use of calcium hydroxide medication; however, they were significantly reduced after a 2nd-visit preparation. Consequently, the lowest bacterial levels were found at the end of the treatment. This clinical trial, which used an rRNA and rDNA combined approach, confirmed previous studies showing that root canal preparation represents the main strategy for root canal disinfection.
Introdução: A perfuração radicular é um acidente caracterizado pela comunicação entre o espaço pulpar ou canal radicular com os tecidos periodontais de sustentação do elemento dentário. Este tipo de complicação pode acontecer em virtude de cáries extensas, reabsorções radiculares ou ainda devido ao uso incorreto de brocas e outros instrumentos endodônticos. O presente estudo teve como objetivo relatar um caso clínico em que foi realizado o tratamento endodôntico de um elemento dental com perfuração radicular, utilizado o MTA para o selamento da perfuração. Relato de caso: Paciente do sexo masculino, 29 anos, foi encaminhado para a clínica de especialização para continuação e tratamento endodôntico. Ao exame radiográfico observou-se espessamento do espaço do ligamento periodontal na região apical e área radiolúcida na região coronária sugestiva de perfuração radicular, confirmada clinicamente e com o uso do localizador eletrônico foraminal. Na primeira sessão foi realizado o preparo químico-cirúrgico e uso de medicação intracanal. Na sessão seguinte foi realizado o fechamento da perfuração radicular com o uso de MTA cinza e sem seguida a obturação do canal. Considerações finais: Uma conduta clínica adequada, com a utilização de tecnologia e equipamentos que auxiliam no diagnóstico bem como o uso de materiais mais indicados para o tratamento de perfurações radiculares são fundamentais para a obtenção de resultados clínicos favoráveis.
Aim: The aim of present study was to evaluate if the cross-section and the number of use have influence in cyclic fatigue resistance of rotary files. Methods: K3 Endo (group K) and EndoSequence (group E) files, 25/.04, 25 mm, were subdivided into 3 subgroups (n=12) according to the number of uses, 1, 3 and 5 uses, totalizing 72 files. The files were submitted to dynamic assays device moved by an electric engine, using 300 rpm of speed that permitted the reproduction of pecking motion. The files run within a temperate steel ring’s groove, simulating an instrumentation of a curved root canal with 40-degrees and 5-mm of curvature radius. The fracture of file was detected by sensor of device and the time and the number of cycles was acquired. The data were analyzed statistically by two-way ANOVA (p<0.05). Results: There were no statistical significant differences in regard to the number of uses (p>0.05). K3 Endo files showed greater resistance to cyclic fatigue than EndoSequence instruments (p<0.01). Conclusion: It may be concluded that the cross-section of instruments presented significant influence in cyclic fatigue resistance and the number of uses up to 5 times had no influence in cyclic fatigue resistance in both NiTi rotation systems evaluated.
