The coronavirus disease 2019 (COVID-19) outbreak has created unprecedent challenges for healthcare systems worldwide. Oncology services have been reorganized to decrease the risk of nosocomial acquisition of SARS-CoV-2, but changes in treatment pathways and follow-up cancer care can result in patients receiving suboptimal or delayed care. Herein, we describe a cross-sectional nested cohort study conducted to evaluate delays in care for patients with head and neck cancer (HNC) in post-treatment follow-up or palliative care during the COVID-19 pandemic in Northeast Brazil and its impact on health outcomes. Information was extracted from medical records and supplemented by telephone interviews. We compared the following health outcomes: self-perception of anxiety or sadness, fear of COVID-19 infection, cancer-related complications during social isolation, self-medication, diagnosis of COVID-19, and death between patients with and without delayed cancer care. The Mann-Whitney U test was used to compare distributions of continuous variables and the Fisher exact test was used for categorical variables. Thirty-one HNC patients were included in the study, and no case of confirmed SARS-CoV-2 was found. Delayed cancer care due to restriction in health services was reported in 58.1% of cases, and there was no report of telemedicine use during the COVID-19 outbreak. Cancer-related complications during the COVID-19 pandemic were described for most patients (67.7%) and included pain or discomfort, swelling, and dyspnea. Eight (25.8%) patients reported use of prescribed morphine or codeine to manage pain and six (19.4%) patients reported self-medication with over-the-counter (OTC) non-steroidal anti-inflammatory drugs (NSAIDs). We found an association between delayed HNC care and the use of self-medication (p = 0.028). This study indicated that patients with delayed HNC care during the COVID-19 outbreak are more likely to use self-medication with NSAIDs for pain management. Better strategies to follow HNC patients in socioeconomically disadvantaged communities need to be discussed and implemented.
Objetivo: descrever o perfil epidemiológico, clínico e a razão de mortalidade materna no Nordeste entre 2010 e 2019. Métodos: estudo ecológico, descritivo, com dados provenientes do Sistema de Informação sobre Mortalidade (SIM) e Sistema de Informação sobre Nascidos Vivos (SINASC). As variáveis do perfil epidemiológico referem-se à idade, cor/raça, escolaridade e estado civil, enquanto as variáveis clínicas tratam do local de ocorrência, tipo de morte por causa obstétrica, período gravídico e investigação do óbito. Ainda, apresenta-se uma série temporal descritiva quanto à razão de mortalidade materna no período. Resultados: Foram registrados 4521 óbitos maternos no período, sendo 1144 (25,30%) na Bahia. O perfil epidemiológico demonstra que os registros se referem a mulheres entre 25 e 34 anos (42,68%), pardas (66,95%), com 8 a 11 anos de escolaridade (30,28%) e solteiras (46,25%). Quanto às características clínicas, 4057 (89,75%) dos óbitos ocorreram no ambiente hospitalar, 3141 (69,49%) por causas diretas e 2800 (61,93%) durante o puerpério. Em 2013 foi observada a maior razão de mortalidade materna, com 61 óbitos por 100.000 nascidos vivos, em contraste com 2019, apresentando 47 óbitos por 100.000 nascidos vivos. Conclusão: Foi possível elucidar o perfil epidemiológico e clínico dos óbitos maternos no Nordeste entre 2010 e 2019. Ainda, a variação da razão de mortalidade materna observada ao longo dos anos é um fato relevante, mas que deve ser analisado à luz de estudos analíticos que explorem a tendência, sobretudo em uma região historicamente negligenciada em termos de desenvolvimento socioeconômico e de acesso à saúde.
OBJETIVO: Analisar os dados epidemiológicos da vacinação brasileira do ano 2000 a 2020 e refletir sobre as variações na cobertura vacinal no Brasil e possíveis eventos interferentes no PNI. MATERIAIS E MÉTODOS: Estudo descritivo e transversal a partir da análise dos dados do DATASUS sobre a cobertura vacinal brasileira nos últimos 20 anos, coletados em maio de 2021, com o auxílio do Microsoft Office Word 2016 e Microsoft Office Excel 2016 foram comparados quanto aos percentuais alcançados, variação anual e distribuição regional. RESULTADOS E DISCUSSÃO: Observou-se um impacto positivo da vacinação a partir do Programa Nacional de Imunização (PNI), propiciando a erradicação de algumas doenças antes consideradas problemas de saúde pública. No entanto, apesar dos níveis crescentes de cobertura vacinal até 2015, quando atingiu o percentual máximo, houve decréscimo nos anos subsequentes, podendo estar relacionado à difusão das fake news e aos movimentos antivacina. CONCLUSÃO: Constatou-se que apesar dos avanços obtidos a partir da adesão social ao PNI, o advento das fake news e as campanhas antivacina têm se mostrado verdadeiros desafios à saúde pública, inclusive durante a pandemia da Covid-19, e podem ter repercussões para além dos índices de cobertura vacinal, visto que possibilitam a perpetuação de doenças que poderiam ser erradicadas pela vacinação.
