The early phase of orthodontic tooth movement involves sterile acute inflammation of the periodontal ligament in response to biomechanical forces. Anti-inflammatories are pharmacologic agents used in medical and dentistry clinics. The aim of the study was to analyse the bone remodelling during orthodontic movement under non-steroidal and steroidal treatment. Male Wistar rats (n = 90) were randomly divided into three groups: C (control), non-steroidal anti-inflammatory drug (NSAID; potassium diclofenac), and steroidal anti-inflammatory drug (SAID; dexamethasone dissodium phosphate). The animals of the C group received 0.9 per cent saline solution, the NSAID group received potassium diclofenac CATAFLAM® (5 mg/kg), and the SAID group received dexamethasone dissodium phosphate DEXANIL® (2 mg/kg). Animals were sacrificed 3, 7, or 14 days after placement of the orthodontic appliance. The upper first molars were processed histologically; we quantified the blood vessels, Howship lacunae, and osteoclast-like cells present on the tension and compression sides of the periodontal ligament. Bone formation was evaluated under polarized light microscopy; 4.5 Image Pro-Plus® software calculated the percentage of immature/mature collagen present. The results showed that, in 3 and 7 days, NSAID and SAID groups presented fewer blood vessels, Howship lacunae, and osteoclast-like cells when compared to the control group. On the 7th and 14th days, there was a lower percentage of mature collagen in the SAID group (P < 0.001). These data demonstrate that potassium diclofenac and dexamethasone inhibit bone resorption during the initial period of orthodontic movement and that dexamethasone delays the collagen maturation process in established bone matrix.
Introdução: a hipertrofia de adenoide é uma das principais causas da respiração bucal. Entre os métodos utilizados para o diagnóstico dessa condição, os mais precisos são a endoscopia nasal e a ressonância magnética. No entanto, o método mais utilizado, em Odontologia, é a radiografia cefalométrica lateral. Objetivo: determinar a eficácia dessa radiografia no diagnóstico da hipertrofia de adenoide, pela sua comparação com a endoscopia nasal. Métodos: foram avaliados 30 indivíduos (7 a 12 anos). Todos fizeram exame de endoscopia nasal e radiografia cefalométrica lateral. Nas endoscopias, foi registrada a porcentagem de obstrução da nasofaringe e, nas radiografias, a menor dimensão anteroposterior livre da nasofaringe. Resultados: os exames se mostraram fortemente correlacionados (r = -0,793, p-valor < 0,01). Considerou-se portadores de hipertrofia severa de adenoide os pacientes que apresentaram, na endoscopia, obstrução da nasofaringe igual ou superior a 75% e, nas radiografias, o menor diâmetro anteroposterior da nasofaringe igual ou inferior a 5mm; o exame radiográfico teve uma sensibilidade de 75%, especificidade de 86,3%, valor preditivo positivo de 66,7%, valor preditivo negativo de 90,4% e exatidão de 83,3%. Conclusão: a radiografia cefalométrica lateral se mostrou um exame eficiente para o diagnóstico da hipertrofia de adenoide. ResumoPalavras-chave: Hipertrofia de adenoide. Endoscopia nasal. Introdução: existe um grande interesse no desenvolvimento de técnicas para a manipulação de células-tronco, no intuito de instituirem-se tratamentos restauradores de tecidos e órgãos. Para que a bioengenharia seja eficaz, faz-se necessária a presença de três fatores: as próprias células-tronco, uma matriz extracelular e fatores de crescimento. Existem inúmeros fatores de crescimento envolvidos no desenvolvimento do órgão dentário. Por isso pesquisadores ainda não foram capazes de formar um órgão completo, embora existam diversos estudos evidenciando a formação de esmalte e dentina a partir de células-tronco isoladas da polpa dentária. Recentemente, também foram isoladas células-tronco da polpa dos dentes decíduos. Sabe-se que estas células são altamente proliferativas, sendo de grande importância para o cirurgião-dentista o conhecimento do seu comportamento biológico e técnicas de obtenção. Objetivo: este estudo teve como objetivo realizar uma revisão de literatura acerca das atuais tendências das pesquisas com células-tronco na Odontologia, além de discorrer sobre os fatores implicados para o sucesso na utilização prática dessas células. INTRODUÇÃOA perda dentária e dos tecidos periodontais pode resultar em movimento dos dentes remanescentes, dificuldade na mastigação, fonação, desequilíbrio na musculatura e comprometimento da estética dentária e do sorriso, comprometendo a auto-estima. Atualmente existem diversas terapias para substituição dos órgãos dentários, todas elas baseadas em técnicas não-biológicas e sujeitas a falhas 26 . Apesar desta condição ser uma anormalidade comum e não ameaçar a vida do paciente, esforços têm sido dirigidos para o desenvolvimento de mecanismos para a utilização de células-tronco na reposição de tecidos bucais 12 . Para a bioengenharia é essencial uma tríade composta por células-tronco ou progenitoras, uma matriz que funcione como arcabouço e proteínas sinalizadoras, denominadas fatores de crescimento, como estímulo para diferenciação celular (Fig. 1).O objetivo desta revisão de literatura foi discorrer acerca das técnicas da bioengenharia e relatar os resultados obtidos nos experimentos com células-tronco, bem como suas reais tendências na aplicação em Odontologia.
Extraction is very frequent indication in orthodontic planning, especially when there are crowding, biprotrusion, and aesthetically unpleasant profiles. Next to extraction comes space closure, which represents a challenge for orthodontists because of extended treatment time, discomfort created for the patient, tissue tolerance, and stability concerns. When it comes to what mechanics to choose for space closure, loops present two major advantages in relation to sliding mechanics: absence of abrasion and possibility to reach pure dental translation. A case is presented where an adult female patient with early loss of the first lower permanent molars, minor lower crowding, and tooth biprotrusion was treated with upper first bicuspids extraction along with upper and lower space closure done with T-loops to promote best space closure control in order to correct the malocclusion and enhance facial aesthetics.
