Doença articular degenerativa, artrose ou osteoartrose: relato de caso 1 INTRODUÇÃO A osteoartrite, artrose ou osteoartrose, como ainda é conhecida no nosso meio, é uma doença degenerativa das articulações sinoviais (diartroses), sendo a doença reumática mais prevalente entre indivíduos com mais de 65 anos de idade. Estudos americanos apontam que mais de 50 milhões de pessoas apresentam hoje está enfermidade. No Brasil, não existem dados precisos sobre esta prevalência.É uma das causas mais frequentes de dor do sistema músculo-esquelético e de incapacidade para o trabalho no Brasil e no mundo. É uma afecção dolorosa das articulações que ocorre por insuficiência da cartilagem, ocasionada por um desequilíbrio entre a formação e a destruição dos seus principais elementos, associada a uma variedade de condições como: sobrecarga mecânica, alterações bioquímicas da cartilagem e membrana sinovial e fatores genéticos. DESCRIÇÃOPaciente S.R, sexo feminino, 52 anos, procurou a PSF relatando dor em MMII, do tipo em aperto, de alta intensidade, incapacitante, que piora com a movimentação e atrapalha o sono. Faz uso de Diclofenaco de sódio 50mg, além de pomada para dor nas pernas, mas sem melhora significativa.Relata também cefaleia, dor nos ombros, nas mãos e na coluna. Nega etilismo, tabagismo e o uso de drogas ilícitas. Relata alergia a Benzetacil.Como comorbidades, tem hipertensão arterial sistêmica de difícil controle, apesar do uso de antihipertensivos e obesidade.Ao exame físico apresenta bom estado geral, LOTE, normocorada, hidratada, acianótica, anictérica, pressão arterial de 130x80. Aparelho cardiovascular e abdominal sem alterações. Pulsos carotídeos, pedioso e tibial posterior encontravam-se presentes, cheios e simétricos. Em membros inferiores apresenta edema +/4, região plantar planificada com presença de calosidades em região plantar lateral. Joelho edemaciado, mas sem demais sinais flogísticos, presença de leve resistência bilateral e crepitações, sinal da gaveta negativo bilateralmente.A paciente foi aconselhada quanto a importância de perder peso, uso de sapatos adequados e a pratica de atividades físicas para alivio da dor. Foi solicitado raio X de joelho esquerdo e direito com carga e prescrito diclofenaco de sódio 50mg.
Introduction: Hydrocephalus is a heterogenous medical entity characterized by dilatation of the cerebral ventricles. It is present in all age groups, however, in neonates it is one of the most common congenital anomalies of the central nervous system resulting in different clinical manifestations and frequently associated with complications that have deleterious consequences in their quality of life. Moreover, its incidence and prevalence present regional and international variations while, simultaneously, it is more common in underdeveloped countries. Methodology: This study concerns an analytical, transversal epidemiological cohort aiming to associate the outcome "hydrocephalus" with possible maternal and gestational factors. Analysis was conducted based on a database composed of a set of live birth declarations from 01/01/2015 to 12/31/2019, of mothers living in Mato Grosso state. Bivariate analysis was applied, and prevalence ratio was calculated using the chi-squared test of Mantel Haenszel whilst, for a multiple analysis, Poisson regression was used. Results: A sample of 43.218 children were selected from SINASC* database of 16 cities of Mato Grosso state. It was observed that preterm new-borns are 6.31 times more likely to develop hydrocephalus compared to full term babies and there is 2.38 times higher prevalence of hydrocephalus among those children of single, divorced or widowed mothers compared to those of married or in-a-stable-relationship mothers. Discussion: These findings are consistent with available literature which confirm that prematurity is one of the most important risk factors for hydrocephalus and that a mother's marital status also plays a role not yet wellunderstood.
