BACKGROUND AND OBJECTIVES: Knowing factors associated with dental pain is important to provide adequate intervention and attention to this major health care problem. This study aimed at understanding the prevalence of dental pain as reason for the most recent dental appointment among 15-year old adolescents and at checking its association with socioeconomic factors, behavioral variables and oral health. METHOD: The probabilistic sample was made up of 592 students of 10 state schools. Dental pain variable was evaluated by questionnaires applied at the school. Clinical oral exam was performed according to World Health Organization's criteria and remaining variables were investigated by self-applicable questionnaires. Data were classified by the Excel program and Odds Ratio was used to associate variables. RESULTS: From 592 participants, 33.44% have reported dental pain as reason for their most recent dental appointment. After statistical analysis, dental pain was associated with low income (p = 0.04), higher number of people living in the same home (p < 0.01), low frequency of daily tooth brushing (p = 0.01), long interval between dental appointments (p < 0.001), longer time elapsed since last dental appointment (p < 0.001), dental anxiety (p < 0.01), consumption of cariogenic food (p = 0.03), high dental caries experience (p < 0.01) and with the presence of untreated dental caries (p < 0.001). CONCLUSION: Dental pain is related to dental caries experience and activity and to socioeconomic and psychosocial factors, showing the need for further attention to these conditions.
ResumoIntrodução: O Tratamento Restaurador Atraumático (ART) possibilita o tratamento de cárie com o auxílio de instrumentos manuais e sem anestesia, em locais onde o acesso a consultórios é impossibilitado, e é considerada uma técnica de fácil execução. Objetivo: Avaliar a efetividade do ART realizado pelos estudantes de graduação de odontologia em escolares de 4 a 11 anos de idade de Piracicaba-SP nos anos de 2009 e 2010, na dentição decídua, e a longevidade das restaurações em 6 e 12 meses. Método: A análise dos dados foi descritiva, e para avaliar diferença entre os grupos foi utilizado teste qui-quadrado (intervalo de confiança de 95%). Resultado: O sucesso das restaurações atraumáticas realizadas foi de 51,90% aos 6 meses e 47% aos 12 meses. A efetividade do ART variou segundo o tipo de restauração aos seis meses e aos 12 meses de reavaliação, apresentando maior sucesso em restaurações Classe I do que em restaurações Classe II (p<0,001). Conclusão: O sucesso do ART realizado por alunos de graduação foi próximo de 50% aos 6 e 12 meses, sendo significativamente melhor em cavidades Classe I nas duas avaliações.Descritores: Cárie dentária; saúde pública; cimentos de ionômeros de vidro. AbstractIntroduction: Atraumatic restorative treatment (ART) is a simple technique that enables the treatment of caries with hand tools and without anesthesia where clinic access is lacking. Objective: To evaluate the effectiveness of ART conducted in the primary dentition of 4-11-year-olds in 2009 and 2010 by graduate dental students in Piracicaba, SP, and the longevity of restorations at 6 and 12 months. Method: Descriptive data analysis was performed. Differences between groups were assessed using the chi-squared test with a 95% confidence interval. Result: The success rates of ART were 51.90% at 6 months and 47% at 12 months. Class I ARTs were more successful than class II ARTs at 6 and 12 months (p < 0.001). Conclusion: Approximately 50% of ART performed by undergraduates students have succeeded and was significantly better for class I carious lesions at both evaluation time points.
LUALE LEÃO FERREIRA Associação entre comportamentos em saúde bucal e variáveis biopsicossociais em escolares de Piracicaba-SP Association between oral health behavior and biopsychosocial variables in students from Piracicaba-SP Piracicaba 2015 ii iii iv v vi vii RESUMO Considerando o crescente interesse da odontologia em conceber a saúde bucal para além do paradigma biomédico, visando atender aos preceitos do modelo biopsicossocial e de prevenção e promoção da saúde, o presente trabalho teve como objetivo geral avaliar comportamentos em saúde bucal e variáveis biopsicossociais que permeiam a saúde bucal em escolares. Para tanto, foram desenvolvidas três análises baseadas em levantamento epidemiológico observacional com delineamento transversal, que apresentou os seguintes objetivos específicos: 1. Avaliar as variáveis de desfecho frequência de consultas odontológicas e prevalência de cárie, investigando sua associação com variáveis psicossociais e nível socioeconômico; 2. Investigar a ansiedade frente ao tratamento odontológico nos escolares, e testar associações entre a ansiedade, condições clínicas odontológicas, variáveis psicossociais e socioeconômicas; 3. Avaliar a existência de associações entre odontalgia e fatores socioeconômicas, condições de saúde bucal e variáveis psicossociais. Para o estudo 1 e 2, a amostra foi composta por de 532 escolares de 15 anos de idade, provenientes de escolas públicas estaduais de Piracicaba, São Paulo. Para o estudo 3, a amostra contou com 592 escolares. Foram coletados dados clínicos relativo a presença de cárie dentária, avaliada pelo índice CPOD segundo recomendações da OMS, e sangramento gengival. Informações relativas à ansiedade frente ao tratamento odontológico foram coletadas por questionário validado para população brasileira. Os escolares também responderam a questões referentes ao acesso do adolescente a consultas odontológicas, aos comportamentos de higiene bucal, ao motivo de última consulta odontológica, à ansiedade frente ao tratamento odontológico, à coesão e à adaptabilidade familiar e à alimentação do adolescente em ambiente escolar. Os pais ou responsáveis pelos adolescentes responderam às questões sobre o nível socioeconômico. A análise dos dados foi realizada por meio de estatística descritiva, teste qui-quadrado, Odds Ratio e regressão de Poisson. Em relação ao estudo 1, controlando variáveis confundidoras, observou-se que a baixa higiene bucal diária, renda familiar menor que três salários, mais que quatro pessoas residentes na mesma habitação e alta ansiedade odontológica foram associadas à baixa frequência de consultas odontológicas.A presença de dente cariado foi associada à baixa frequência de consultas odontológicas e a presença de dentes obturados foi associado à alta frequência de consultas viii e ao gênero feminino. A experiência de cárie associou-se à baixa higiene bucal diária, à alta frequência de consultas odontológicas e pai com até 8 anos de escolaridade.
