Aumento de Coroa Clínica (ACC) com finalidade funcional e estético é preconizado como uma alternativa terapêutica na ocorrência de invasão dos tecidos de inserção supracrestais/espaço biológico. O presente trabalho teve o objetivo de elucidar, por meio de uma revisão narrativa e relato de caso, a importância da cirurgia de aumento de coroa clínica funcional com finalidade protético-restauradora. A revisão foi realizada nas seguintes bases de dados: Google Acadêmico, Lilacs, MEDLINE/PubMed e Scielo. O relato de caso seguiu as diretrizes CARE para Relatos de Caso. Mulher, 55 anos, apresentou-se com o elemento 16 afetado de uma lesão de cárie cervical subgengival, Classe V, na face palatina. Foi realizado o procedimento cirúrgico de ACC funcional por técnica de retalho com osteotomia e osteoplastia. Posteriormente, foi realizado o tratamento restaurador. O caso foi acompanhado por mais de 3 meses. Houve efetividade do procedimento cirúrgico e estabilidade dos tecidos periodontais que corresponderam com êxito ao tratamento restaurador executado, sem limitações. Em conclusão, destaca-se a importância da cirurgia periodontal de ACC com finalidade funcional, pois consiste em um procedimento minimamente invasivo empregado para auxiliar tratamentos protético-restauradores, prevenindo falhas durante o tratamento e injúrias ao periodonto, mantendo os tecidos de inserção supracrestais saudáveis. Logo, o conhecimento das dimensões relativas à saúde dos tecidos de inserção supracrestais, bem como sua compreensão anatômica, e as consequências de sua invasão, contribui para adequados tratamentos protético-restauradores e o consequente sucesso, devolvendo função e conforto e longevidade.
