INTRODUÇÃOSão cada vez mais frequentes discussões sobre a relação mé-dico-paciente e as consequências de um bom ou mau atendimento por parte de um profissional da saúde. O profissionalismo é um traço essencial para os médicos, sendo reconhecido como uma competência importante para os alunos de Medicina 1 . Além disso, o profissionalismo é a base do contrato do médico com a sociedade. Isso requer colocar os interesses dos pacientes acima daqueles dos médicos, estabelecer e manter padrões de competência e integridade, e oferecer à sociedade informações especializadas sobre questões de saúde 2 . Dentre todos os fatores que envolvem o profissionalismo, vamos dar destaque à empatia, uma de suas características centrais.A palavra "empatia" é derivada do grego empatheia (em = dentro e pathos = sofrimento ou sentimento) e se refere à relação emocional entre o autor-cantor e seu público. Em sua conceituação moderna, é uma tradução do termo alemão einfühlung, introduzido no vocabulário da experiência estética para designar a relação entre a obra de arte e seu observador, que imaginativamente se projeta no objeto contemplado. O termo ganhou um significado dissociado da arte e mais próximo do âmbito da relação pessoal ao ser traduzido para o inglês por Titchener, com o nome de empathy, cujo significado era de que, por meio da imitação interior ou esforço mental, seria possível conhecer a consciência de outra pessoa 3,4,5 . Dessa forma, a empatia pode ser definida como uma disposição genuína de ser capaz de ouvir, compreender, compadecer-se através de deduções, de informações retidas na memória ou colocando-se no lugar do outro, prestando assim apoio a outro indivíduo, fazendo com que essa pessoa se sinta compreendida e validada 5,6 . Recentemente, os pesquisadores têm considerado a empatia como uma capacidade multidimensional que abrange componentes cognitivos, afetivos e comportamentais 7,8 . Falcone 7 , cujo modelo vamos utilizar, entende que a empatia "corresponde à capacidade de compreender, de forma acurada, bem como de compartilhar ou considerar sentimentos, necessidades e perspectivas de alguém, expressando este entendimento de tal maneira que a outra pessoa se sinta compreendida e validada". Tal definição considera a empatia como uma habilidade social que distingue as espécies humanas e não humanas, na medida em que somente nas primeiras são identificados aspectos tais como tomada de perspectiva, autoconsciência, consciência do outro, flexibilidade, reavaliação da emoção, além de expressão verbal e não verbal de entendimento.Na esfera das relações no campo da saúde, empatia é definida como um atributo com dimensões emocionais e cognitivas que possibilita uma compreensão das experiências interiores e da perspectiva do paciente como um indivíduo singular, somada à capacidade de comunicar esse entendimento ao paciente 9 . In April 2020, the UK pediatric society reported to the World Health Organization the appearance of a rare syndrome, with a wide spectrum of signs and symptoms, temporarily associated with COVID-19, which mainly affected normally healthy schoolchildren and adolescents, with high lethality when not diagnosed and treated early. We present the case of a 12-year-old child who had high and persistent fever, rash and vomiting, who, after 6 days, developed acute respiratory failure, myocardial and renal dysfunction, requiring ventilatory and hemodynamic support, treated with antibiotics, intravenous immunoglobulin, and anticoagulant. The purpose of this report is to present the reader with the peculiar characteristics of the diagnosis and treatment of the syndrome and its main differential diagnoses at the hospital admission.
INTRODUCTION: Mechanical changes are the ventilatory support used to maintain lung function while the cause of intubation is reversed, and it has greatly contributed to increased survival in intensive care units, however, the need for orotracheal intubation, especially for prolonged periods, brought concerns about the failure to withdraw this support. Extubation failure is a worldwide problem, and a search for predictors, risk factors and therapies capable of preventing it has mobilized research groups. Objectives: We present the results of an observational study carried out in a pediatric intensive care unit for one year, which aimed to identify risk factors related to extubation failure in children and adolescents mechanically ventilated for at least 48 hours. METHODS: Eighty-five children aged between 29 days and 15 years of age were included, of which 11 (12.9%) required reintubation. RESULTS: In our sample, the risk factors found were age less than 3 months [OR: 2.71], mechanics for more than 15 days [OR: 7.30], fatal from shock [OR: 2.45], fatal of cardiorespiratory arrest [OR: 8.0] and those who were drilled to intubation cannula changes [1.97]. CONCLUSION: These conditions increased the risk of extubation failure in our sample.
