As disfunções temporomandibulares referem-se a um conjunto de condições que afetam os músculos da mastigação e/ou a articulação temporomandibular. Quando esta condição abrange também problemas clínicos na região cervical, utiliza-se o termo disfunção craniocervicomandibular. Além de comprometer funções próprias do sistema estomatognático, as alterações nas estruturas da ATM podem trazer sintomatologia auditiva associada devido à grande proximidade anatômica e funcional entre os componentes da orelha e ATM. O Vírus da Imunodeficiência Humana, quando associado à uma terapia antirretroviral deficiente ou ao abandono do esquema terapêutico, é o responsável pelo desenvolvimento da Síndrome da Imunodeficiência Adquirida, quadro clínico de imunossupressão que predispõe o paciente ao aparecimento de infecções oportunistas. A infecção pelo Vírus do Papiloma Humano é uma das infecções oportunistas mais comuns e diversos fatores de risco podem estar associados à sua prevalência, dentre eles o tabagismo e etilismo, que também contribuem para a persistência da infecção e possível cancerização de lesões causadas pelo vírus. O presente estudo tem como objetivo relatar o caso clínico de um paciente do sexo masculino, ex-tabagista, ex-etilista, ex-usuário de drogas com diagnostico tardio do Vírus da Imunodeficiência Humana e em quadro de Síndrome da Imunodeficiência Adquirida, que apresentou leucoplasias em bordo bilateral de língua. Foi necessária exérese total das lesões e a análise histopatológica apresentou características de infecção pelo Vírus do Papiloma Humano, além de displasia atípica com alta replicação viral e possibilidade de malignidade.
A sialoadenite é uma inflamação das glândulas salivares que pode ser de origem infecciosa ou não, caracterizada por edema, dor e diminuição ou ausência de salivação da glândula afetada. A redução do fluxo salivar pode ser encontrada também em casos de hipertrofia das glândulas parótidas, como observado nos casos de diabetes, alcoolismo, desnutrição, anorexia, bulimia e, dentre elas, a estenose ductal, que pode ocorrer, em razão de sialólitos, mas também devido à contratura muscular em indivíduos com disfunção muscular mastigatória, cronificada pela perda de dimensão vertical decorrente da não reabilitação protética e potencializada por razões ambientais diversas. Este trabalho apresentará um caso em paciente de 61 anos de idade, sexo feminino, com queixa de fortes dores na região de glândula parótida direita, edema ipsilateral, mioespasmo com presença de pontos gatilhos massetéricos e ordenha negativa, levando a hipótese diagnóstica de Sialoadenite de Refluxo. Houve indicação da confecção de um aparelho protético inferior com plano deslizante de resina acrílica para aumentar a Dimensão Vertical de Oclusão (DVO) e restabelecer a dinâmica maxilomandibular, regularizando o fluxo salivar do ducto parotídeo e restaurando a homeostasia glandular até a aquisição de novas próteses reabilitadoras totais para paciente. Dor e disfunção da articulação temporomandibular (DTM) são comuns e resultam de uma ampla variedade de agentes etiológicos, tanto traumáticos quanto não-traumáticos. O conjunto de sinais e sintomas (dor, alteração da função, ruídos articulares) é denominada disfunção temporomandibular (DTM) e, quando abrange também problemas clínicos na região cervical, utiliza-se o termo disfunção craniocervicomandibular (DCCM). Esta condição pode concorrer para as alterações coclear e vestibular, onde a primeira é justificada pela origem embrionária e proximidade anatômica da ATM com a orelha e a segunda, pela disfunção dos músculos da mastigação e cervical (Masseter e Esternocleidomastóideo) que, por serem antigravitacionais, concorrem para a vestibulopatia, e também, otalgias reflexas. Apesar dessas relações existirem sua patogênese não está totalmente esclarecida. Este trabalho tem como objetivo apresentar um caso clínico de diagnóstico e tratamento de paciente com DCCM associado à alteração vestibulococlear.
