Introdução: A incidência de casos de trissomia do cromossomo 18 (Síndrome de Edwards), a segunda trissomia autossômica mais freqüente, oscila entre 1/3000 a 1/8000 nascidos vivos. Essa aneuploidia resulta em malformações severas, profundo retardo mental e alta taxa de mortalidade. Objetivos: Relatar um caso neonatal de Síndrome de Edwards, enfatizando as vantagens de confirmação diagnóstica rápida por meio da técnica Quantitativa Fluorescente da Reação em Cadeia da Polimerase (QF-PCR). Método: Revisão de prontuário Relato de caso: Prematura do sexo feminino, com idade gestacional de 31 semanas, nascida de parto cesariano e pesando 930g. Idade materna 33 anos. A recém-nascida foi transferida para Unidade de Terapia Intensiva (UTI) Neonatal por apresentar asfixia perinatal, insuficiência respiratória, doença de membrana hialina e cardiopatia congênita. As alterações fenotípicas levaram à hipótese diagnóstica de Trissomia 18. A suspeita foi confirmada por QF-PCR em apenas 24 horas e por citogenética convencional em 15 dias. Conclusões: Em gestações de risco de aneuploidias e suspeita clínica no pré-natal e/ou quando há alterações fenotípicas sugestivas após o nascimento a solicitação do teste molecular rápido é de grande valia.
