Background:Failures in the processes of cleaning and disinfecting health service surfaces may result in the spread and transfer of pathogens that are often associated with healthcare-related infections and outbreaks.Aims:To assess the effectiveness of environmental surface cleaning and disinfection in a hospital clinic.Method:The study was conducted in a nursing ward with 45 beds. A total of 80 samples from five high-touch surfaces were evaluated before and after cleaning and disinfection, using the following methods: visual inspection, adenosine triphosphate bioluminescence assay, aerobic colony count, Staphylococcus aureus colony count, and evaluation of resistance to methicillin. The data analysis used nonparametric comparative and correlative tests to observe any differences in the pre- and post- cleaning and disinfection results for the surfaces assessed.Results:Effective cleaning and disinfection had a significant effect on only two surfaces when measured for the presence of adenosine triphosphate, the inner bathroom door handle (p=0.007) and the toilet bowl (p=0.01). When evaluated for Staphylococcus aureus colony count, the toilet flush handle also demonstrated a significant effect (p=0.04).Conclusion:The effectiveness of cleaning and disinfection of the surfaces tested was not satisfactory. An educational intervention is recommended for the cleaning and disinfection staff and the nursing team at the healthcare facility.Relevance to Clinical Practice:The data in the study revealed that daily hospital cleaning and disinfection in the sampled sites are not sufficient in medical and surgical wards. Hospital cleanliness must be reevaluated from the point of view of materials, such as an adequate supply of clean cloths, in addition to establishing more precise cleanliness protocols and accurate monitoring systems.
Background Cleaning and disinfection processes must be improved so that there is a reduction in environmental contamination of frequent-contact surfaces. The objective of this study was to evaluate cleaning and disinfection of surfaces at a specialized healthcare unit after an intervention program. Methods Exploratory, longitudinal, and correlational study carried out in a medium-complexity clinic. Two hundred and forty samples from five surfaces were collected during three phases: diagnosis; implementation of an intervention program; and evaluation of immediate and long-term effects. In total, 720 evaluations were made, performed through three monitoring methods: visual inspection; adenosine triphosphate bioluminescence assay (ATP); and aerobic colony count (ACC). The Wilcoxon, Mann-Whitney, and Fisher’s Exact tests were run to analyze data statistically. Results Cleaning and disinfection of surfaces were not being performed properly in most cases. Failure rates of surfaces reached 37.5 and 100% when the ATP and ACC procedures were used, respectively. However, after an intervention program, an improvement occurred. Success rates increased by 43.96% (ATP) and 12.46% (ACC) in phase I, by 70.6% (ATP) and 82.3% (ACC) immediately after interventions, and by 76.52% (ATP) and 85.76% (ACC) two months after the changes, showing that the program was effective. Conclusion The present study reveals that implementing intervention actions with a cleaning and healthcare team brings benefits to prevent the spread of pathogenic agents through frequently touched hospital surfaces.
OBJETIVO: O estudo teve como objetivo identificar o conhecimento, prática e fontes de informações de enfermeiros, de um hospital universitário, sobre o cuidado com feridas. MÉTODOS: Estudo transversal, descritivo-exploratório, realizado em julho de 2010. Os dados foram coletados por meio de um questionário adaptado. Participaram deste estudo 35 enfermeiros distribuídos em diferentes setores de trabalho. Os dados sofreram tratamento estatístico descritivo e analítico. RESULTADOS: Considerando os resultados globais, os enfermeiros obtiveram, em média, 69,4% de acertos (dp=13,5%). Os enfermeiros que citaram utilizar às vezes algumas fontes de informações tiveram maior porcentagem de acertos. Conclui-se que os enfermeiros apresentaram conhecimento insuficiente em algumas áreas referentes ao tema. CONCLUSÃO: Destaca-se a necessidade da atualização dos profissionais quanto aos avanços das evidências atuais que embasam o cuidado de feridas, a fim de nortear, no contexto estudado, o planejamento de estratégias para implementação das melhores evidências disponíveis.
RESUmEnObjetivo: Evaluar la seguridad de la ejecución de las técnicas tradicional y protegida de la recolección de aspirado traqueal e identificar la concordancia cualitativa y cuantitativa de los resultados de los cultivos microbiológi-cos entre estas técnicas. Método: Investigación clínica, prospectiva, comparativa, ciega simple. La muestra fue constituida por 54 pacientes mayores de 18 años que fueron sometidos a ventilación mecánica invasiva durante 48 horas o más y con sospecha de Neumonía Asociada a la Ventilación Mecáni-ca. Las dos técnicas fueron implementadas en el mismo paciente, una inmediatamente posterior a la otra. El orden de ejecución de las técnicas fue aleatorio, hecho por un software especializado. Resultados: No hubo eventos significativos en la disminución de la saturación de oxígeno, inestabilidad hemodinámica y hemorragias traqueo bronquiales (p<0,05) y aunque ocurrieron diferencias en algunas cepas, hubo concordancia cualitativa y cuantitativa entre las técnicas (p<0,001). Conclusión: El uso de la técnica protegida no ofrece ninguna ventaja sobre la técnica tradicional y la aplicación de ambas técnicas es segura para el paciente. RESUmoObjetivo: Avaliar a segurança da execução das técnicas tradicional e protegida de colheita de aspirado traqueal e identificar a concordância qualitativa e quantitativa dos resultados de culturas microbiológicas entre as técnicas. Método: Pesquisa clínica, prospectiva, comparativa, simples-cega. A amostra foi constituída de 54 pacientes com idade ≥18 anos, submetidos à ventilação mecânica invasiva por período ≥48 horas e com suspeita de Pneumonia Associada à Ventilação Mecânica. As duas técnicas foram implementadas no mesmo paciente, uma imediatamente seguida da outra, sendo a ordem de execução aleatória, segundo randomização por software especializado. Resultados: Não ocorreram eventos significativos de queda da saturação de oxigênio, instabilidade hemodinâmica e hemorragias traqueobrônquicas (p<0,05) e, embora tenham ocorrido divergências em algumas cepas, houve concordância qualitativa e quantitativa entre as técnicas (p<0,001). Conclusão: A utilização da técnica protegida não proporciona vantagem em detrimento da tradicional e a execução de ambas as técnicas foi segura para o paciente.
Objetivo: Analisar a qualidade de vida de pessoas com feridas complexas crônicas. Métodos: Os dados foram coletados por meio de questionário contendo o instrumento WHOQOL-Bref da Organização Mundial de Saúde e questões sobre variáveis sociodemográficos e clínicas. Para análise estatística foram utilizados os métodos: Correlação Linear de Pearson; Análise de Variância e Tukey com confiabilidade de 95% e p<0,05. Resultados: Foram avaliados 53 pacientes com feridas complexas. Desses, 28 (52,83%) são do sexo feminino com média de idade de 62,17 anos e com baixa escolaridade. Na avaliação da qualidade de vida a dor se correlacionou com o domínio físico (p=0,030) e psicológico (p=0,051); enquanto que a área se correlacionou com a idade (p=0,051) e a dor se correlacionou com o tempo de lesão (p=0,001). Conclusão: Conhecer as características clínicas e a qualidade de vida possibilita melhor compreensão das queixas e melhor planejamento assistencial contribuindo para melhoria da qualidade de vida.
