Objectives. The aim of the present study was to clarify clinical neurological characteristics and different ischemic stroke subtypes unfavorable course predictors in acute phase.Material and methods. 482 patients with different ischemic stroke subtypes were observed. Among them there were 125 (25.9 %) with cardioembolic infarct (CEI), 119 (24.7 %) with large artery atherosclerosis (LAAS) infarct, 122 (25.3 %) with lacunar stroke (LAC), 116 (24.1 %) with stroke of undetermined etiology (UDE). The comparative analysis of clinical picture was performed. The predictors of unfavorable course of acute phase were established.Results. We have found out that severe neurological deficit, high mortality and the worst functional outcome during the first 14 days were observed in patients with CEI and LAAS. The highest frequency of early neurological deterioration (END) was detected at LAC (in 22.7 % of patients). There was a relationship between END and presence of transient ischemic attack (TIA) in past medical history, the level of systolic blood pressure (SBP) at the beginning of the disease and the degree of carotid arteries stenosis on the side of lesion. The patients with LAC had mild neurological deficit and better prognosis compared with other ischemic stroke subtypes. Among the clinical factors that have impact on the CEI, LAAS and UDE acute phase course were: the size of lesion, the level of consciousness on the 1st day, the baseline SBP, patient's age. At LAAS, the presence of transient ischemic attack (TIA) in past medical history and low SBP in the onset of the disease (less than 140 mm Hg) has an additional prognostic value for an unfavorable functional outcome. The severity of LAC in acute period depended on its localization and size. Localization of LAC in the internal capsule, thalamus and pons were characterized by the highest severity. Conclusions.Clinical neurological features of ischemic stroke depend on its subtype and have some prognostic value for the course of different ischemic stroke subtypes in acute phase.Клініко-неврологічні особливості перебігу підтипів ішемічного інсульту в гострому періодіМета роботи -уточнити клініко-неврологічні особливості та предиктори несприятливого перебігу різних підтипів ішемічного інсульту в гострому періоді.Матеріали та методи. У 482 осіб із різними підтипами ішемічного інсульту (125 (25,9 %) з кардіоемболічним інсультом (КЕІ), 119 (24,7 %) -з атеротромботичним інсультом (АТІ), 122 (25,3 %) -з лакунарним інсультом (ЛІ) та у 116 (24,1 %) пацієнтів -з інсультом невизначеної етіології (ІНЕ) здійснили порівняльну характеристику особливостей клінічної симптоматики та визначили предиктори несприятливого перебігу гострого періоду.Результати. Встановили, що найтяжчий неврологічний дефіцит, найвища смертність і гірший функціональний резуль-тат протягом перших 14 днів захворювання спостерігались при КЕІ та АТІ. The article describes a clinical case report of a young patient with ischemic stroke on the background of antiphospholipid syndrome (APLS). The uniqueness of this case lies in the complex diagnostic search that we performed. On admission, the patient had general cerebral, general infectious and focal syndromes. We suspected encephalitis due to the peculiarities of the onset of the disease and the results of computed tomography. However, after the results of lumbar puncture, the diagnosis of encephalitis required careful differential diagnosis. We performed an extensive diagnostic search. Based on clinical-laboratory, instrumental and immunological data the patient had the following final diagnosis: “Ischemic cardioembolic stroke in the right middle cerebral artery pool (15.10.17) (ICD 11: 8B11.5). Acute period with left pyramidal reflex insufficiency and changes in magnetic resonance imaging. Systemic lupus erythematous (ICD 11: 4A40.0), subacute course, activity II, with the lesion of skin (transient erythematous rash), kidneys (proteinuria, transient impaired renal function), lungs (bilateral pleurisy with immunological disorders). Secondary APLS (ICD 11: 4A45) (Acute iliofemoral thrombosis, May 2017; chronic thrombosis of the inferior vena cava, iliac veins, positive IgG to cardiolipin, beta 2 glycoproteins). Thus, we have to link thrombotic complications in young patients with APLS and to examine the patients for antiphospholipid antibody presence.
У роботі йдеться про результати виконаного порівняльного аналізу трьох скринінгових шкал для оцінки когнітивних порушень: Mini-Mental state examination (MMSE), Mini-Cog test, Montreal Cognitive Assessment (MoCA). Нейропсихологічне обстеження провели 25 пацієнтам із цереброваскулярними захворюваннями головного мозку. У результаті проведених досліджень було встановлено, що найменше часу потребує виконання тесту Mini-Cog (у середньому 3 хв), проведення MMSE займало близько 10 хв, а MoCA — 13–15 хв. Тривалість виконання тесту впливала на рівень втомлюваності та виснаження пацієнтів. Результати тесту Mini-Cog не залежали від вихідного рівня освіти, культури та мови. Проте даний тест виявився найменш чутливим і зміг діагностувати лише виражені когнітивні порушення. Перевагами тесту MMSE були можливість визначення рівня когнітивних порушень відповідно до кількості набраних балів та значно вища чутливість порівняно з тестом Mini-Cog. Найчутливішим скринінговим тестом виявився MoCA, проте система формалізованої оцінки даного тесту на сьогодні не передбачає градацію за тяжкістю порушень залежно від набраної кількості балів. Отже, результати проведених досліджень доводять, що вибір нейропсихологічної методики повинен здійснюватися з урахуванням конкретної клінічної ситуації та умов, у яких вона застосовується. Даний порівняльний аналіз загальноприйнятих скринінгових шкал для визначення когнітивних порушень може бути застосований при плануванні діагностичного пошуку для забезпечення ранньої ефективної верифікації змін інтелектуально-мнестичних функцій.
Background. Literature sources report conflicting results regarding the severity of clinical symptoms of hydrocephalus (HC) and their association with cerebral arterial blood flow (CABF); most studies do not show a direct relationship, while some suggest a link between clinical severity and progressive decrease in CABF. The study of hemodynamic changes in the brain of patients with chronic brain ischemia (CBI), elucidation of their relationship with cognitive impairments helps to improve diagnostic approaches and optimize the prognosis of the disease. The purpose of this study is to investigate the state of cerebral hemodynamics and to establish its relationship with changes in cognitive functions in patients with CBI and HC. Materials and Methods. A comprehensive examination of 110 patients with CBI and HC was performed. The localization of the HC and results of Montreal Cognitive Test (MoCA scale) were taken into account. Computed tomography of the brain was performed with subsequent determination of morphometric parameters and indices. The state of cerebral blood flow and structural changes of blood vessels were studied using transcranial colorcoded duplex ultrasonography (TCCS) of intracranial vessels and extracranial divisions. Microsoft Excel 2011 and Statistica were used for statistical processing of the results. Results. It was found that there was a significantly larger diameter of both common carotid arteries (CCA), thickness of complex intima\media (CIM), maximum systolic velocity (Vs), velocity at the end of the diastolic cycle (Vd), as well as peripheral resistance (IR) in left CCA (p <0,05) in patients with CBI and HC compared with patients with CBI without HC. Significantly larger diameter of left internal carotid artery (ICA), Vs, Vd and IR was revealed; Vs and Vd in the right ICA; Vd in the extracranial devision of vertebral artery (VA) in patients with HC.
