Greater superficial petrosal nerve (GSPN) schwannomas are very rare tumors. There are only 15 reports, described so far. In this study the authors discuss the clinical presentation, radiological features, surgical approach, postoperative outcome and follow up of 3 cases of this rare schwannoma of the middle fossa, possibly originating from GSPN together with the review of existing literature. The presenting features were facial paresis in all cases, xerophthalmia in case 1, and tinnitus in case 2. Imaging studies showed a sub-temporal extradural mass on the anterosuperior aspect of the petrous bone in all cases. All tumors were removed through a subtemporal extradural microsurgical approach. Improvement of facial weakness and xerophthalmia as an end result is characteristic post-operative finding.
SUMMARYIn the present investigations 4,6-diacetyl resorcinol 1 on condensation with formaldehyde and some selected amines following the Mannich reaction conditions yielded eight new Mannich bases 2a-h. It was observed that the reaction did not take place at the acetyl function but occurred at the 2-position into the aromatic ring. The compounds were characterized on the basis of elemental analysis as well as 1 H NMR and Mass spectral data. The antibacterial and antifungal activities of the title compounds were tested by the disc diffusion method using nutrient agar medium against various microorganisms such as gram positive Staphylococcus aureus and Bacillus subtilis, gram negative Escherichia coli and the fungi Aspergillus flavus and Candida albicans. Ofloxacin and voriconazole at 20 μg/mL were used as standard drugs for antibacterial and antifungal activities, respectively.
Acute onset Altered mental status is a common but clinically challenging emergency due to numerous possibilities with little clinical window and time to treat. The numerous causes include stroke, encephalitis, organ dysfunction, metabolic, endocrine and intoxication. Amongst the vascular causes, one rare but peculiar cause being artery of percheron infarct. Blood supply to the thalamus and brainstem have various anatomic variations. Artery of Percheron is one such variation in which a single arterial trunk arises from proximal segment of one of the posterior cerebral artery (PCA) and bifurcates to supply bilateral paramedian thalami. Occlusion of this artery, although uncommon results in characteristics pattern of symmetric infarction in bilateral medial thalami with or without mesencephalic infarction. We here describe a case of a gentleman who presented with sudden onset altered mental status and skew deviation. After improvement in sensorium characteristic vertical gaze palsy was noted with subtle confusion which recovered. MRI brain revealed characteristic Para median bithalamic infarcts with normal intracranial angiogram. Patient showed a slow and incomplete clinical recovery to conservative management.
