Introduction: This study was conducted to evaluate the accompanying psychosocial problems in enuresis as well as the social anxiety-depression levels in children diagnosed with enuresis.Methods: This descriptive study investigating depression and social anxiety levels of children diagnosed with enuresis was carried out with 167 children who were followed up for at least six months following their enuresis diagnosis. All participating children had no other physical or mental illnesses and were aged five years or older. Data were collected using three measurement tools and evaluated using descriptive, correlational analyzes.Results: In this study, 69.4% of the children diagnosed with enuresis were aged 7-12 years. 38.3% of their parents used "reward", while 37.1% preferred "punishment" as a method for toilet training their children. The children's depression and social anxiety mean scores were 21.48±7.29 and 54.58±8.84, respectively. A strong positive correlation was found between the children's mean depression and social anxiety scores (r=0.677, p<0.001). The median depression scores of children were found to be statistically significantly different according to the children's type of family, night sleep characteristics, type of enuresis, school success, and family life (p<0.05). Those with fragmented families (p=0.049), who did not sleep deeply at night (p=0.031), who slept for about 5-7 hours a day (p<0.01), and those whose school success was negatively affected by enuresis (p=0.021) and those who were exposed to negative family life (p=0.034) all obtained statistically significantly higher median depression scores. Conclusion:Children with enuresis had low depression and moderate social anxiety and their depression increased as their social anxiety increased. The children experienced psychosocial difficulties due to enuresis. When treating children with enuresis, it is necessary to consider both the affected child and their families through the adoption of a holistic approach, and also to evaluate the children both physically and psychosocially.
In the pathology results of the prostatic adenocarcinoma patients, the age, tumor stage and Gleason score (GS) were evaluated. Physical examination data and the serum total Prostate Specific Antigen (PSA) levels, as well as the radiological findings, were analyzed according to the hospital records. Results: After excluding nine patients with known prostate carcinoma there were 15 adenocarcinomas out of 641 patients (2.34%). The mean age was 72. Eleven patients were diagnosed as GS 6, and four patients were diagnosed with GS 7 prostate adenocarcinoma. Serum total PSA levels ranged from 1.56 to 9.22 ng/mL. T1a tumor was detected in 11 patients and T1b tumor in 4 patients. Conclusion: Considering the studies that reported incidental prostate cancer (IPC) rates in the PSA era, our rate is close to the lower limit. The application of primary therapies should not be avoided in patients with IPC after TUR-P.
Unutulmuş üreteral stentlerin infeksiyon, enkrustasyon, taşlaşma ve böbrek yetmezliği gibi majör komplikasyonları vardır. Bu komplikasyonlar stentin kalış süresi ile artar. Bu çalışmada 10 yıldan uzun süre üreteral stentle yaşayan hastaların tedavi yönetimi ve stentlerin hastalara verdiği zararlar tartışılmıştır. Gereç ve Yöntemler: 10 yıldan uzun süredir enkruste üreteral stentle yaşayan üç hastanın klinik verileri retrospektif olarak değerlendirildi. Bulgular: Her üç hastada tipik stent semptomları (hematüri, yan ağrısı, acil idrar hissi, vs) olmadığı için doktora başvurmadıklarını belirtti. İki hasta sepsis ile bir hasta komplike üriner sistem enfeksiyonu ile tarafımıza başvurdu. Uzun süreli stente bağlı olarak 2 hasta ileri derecede böbrek fonksiyon kaybı gelişirken, 1 hasta böbreğini kaybetti. Nonfonksiyonel böbrekli hastaya laparoskopik nefroüreterektomi yapılırken, diğer iki hastaya tek seans multi-modal nefron koruyucu cerrahi uygulandı. Sonuç: Teknolojideki gelişmelere rağmen unutulan stentler sepsise ve böbrek fonksiyon kaybına neden olmaya devam etmektedir. Enkruste stente bağlı komplikasyonlardan korunmanın en güzel yolunun unutmamak olduğunu düşünüyoruz.
Karbon kaplı boncuklar (Durasphere ®) ve dekstranomer/hiyalüronik asit kopolimeri (Deflux ®) ile primer grade I-III primer vezikoüreteral reflü (VUR) nedeni ile endoskopik subüreterik enjeksiyon yapılan erişkin kadın hastaları retrospektif olarak değerlendirdik. Gereç ve Yöntemler: İzmir Kâtip Çelebi Üniversitesi Tıp Fakültesi ve Niğde Ömer Halisdemir Üniversitesi Tıp Fakültesinin üroloji bölümlerinde, primer grade I-III VUR nedeni ile endoskopik subüreteral enjeksiyon yapılan kadınların tıbbi kayıtları retrospektif olarak gözden geçirildi. Nörojenik mesaneye sekonder VUR, infravezikal obstrüksiyon, mesane tümörünün rezeksiyonu, üreterosel insizyonu veya üreteral reimplantasyonu olan ve 18 yaşın altındaki hastalar çalışma dışı bırakıldı. Bulgular: Durasphere® grubu; preoperatif grade II ve III VUR sırasıyla 7 ve 8 üreterde tespit edildi. Üç hastada bilateral VUR vardı. Cinsel olarak aktif 10 ve tekrarlayan üriner enfeksiyonu olan 7 hasta mevcuttu. Doğum yapan 7 hastanın 6'sında, gebelik sırasında hastaneye yatırılması gereken piyelonefrit öyküsü vardı. Ortalama 38,00±14,70 aylık takip periyodunda vezikoüreteral obstrüksiyon gözlenmedi, ancak 2 olguda VUR nüksüne bağlı ikinci endoskopik girişim, 1 hastaya Cohen üreteroneosistostomi yapıldı. Deflux ® grubu; On dört hastanın 3'ünde bilateral VUR vardı. Grade I, II ve III reflü, sırasıyla 3, 8 ve 6 böbrek ünitesinde izlendi. Hastaların 13'ü cinsel olarak aktif olup, 11'inde doğum öyküsü, 6'sında tekrarlayan idrar yolu enfeksiyonu, 4'ünde gebelik sırasında piyelonefrit nedeni ile hastanede yatış öyküsü vardı. Dört hastaya, başarısızlık nedeni ile ikinci endoskopik subüreterik enjeksiyon uygulandı. Gruplarda postoperatif piyelonefrit izlenmedi. Sonuç: Karbon kaplı boncuklar güvenli, uygulanabilir, etkili subüreterik hacim artırıcı maddedir ve erişkin kadın popülasyonunda ciddi dilatasyon olmayan reflüksif üreterlerinin endoskopik tedavisinde Deflux ® ile karşılaştırılabilir sonuçlara sahiptir. Material and Methods: Medical records of women who underwent endoscopic suburethral injection due to primary grade I-III VUR in the urology departments of İzmir Kâtip Çelebi University Faculty of Medicine and Niğde Ömer Halisdemir University Faculty of Medicine were retrospectively reviewed. Patients with secondary VUR to neurogenic bladder, infravesical obstruction, bladder tumor resection, ureterocele incision or ureteral reimplantation, and being under 18 years of age were excluded from the study.
