BackgroundBifid mandibular canals (BMC) and trifid mandibular canals (TMC) are variations on the normal anatomy with incidences ranging from 0.08% to 65.0%. Such aberrations have an important clinical impact. For example, an extra mandibular canal may explain inadequate anesthesia, especially when two mandibular foramina are involved. Furthermore, during mandibular surgery, a second, or even third, neurovascular bundle may be damaged causing paresthesia, neuroma development, or bleeding.Case reportTwo cases are presented in this article. One patient had a BMC on both sites, and the other patient had a TMC on one site and a BMC on the other site.DiscussionInitial screening for the presence of a BMC or TMC can be executed by conventional panoramic radiography. BMCs or TMCs are diagnosed, before executing mandibular surgery; additional CBCT scanning is indicated.
Een 32-jarige vrouw werd naar een mond-, kaak-en aangezichtschirurg verwezen in verband met een asymptomatische zwelling ter plaatse van gebitselementen 34 en 35. Na extractie van deze gebitselementen en enucleatie van de afwijking bleek bij histologisch onderzoek dat er sprake was van een centrale reuscellaesie. Dit is een goedaardige afwijking waarbij enucleatie over het algemeen tot genezing leidt en slechts een enkele keer een recidief voorkomt. Ongeveer 1 jaar na de behandeling had zich geen recidief voorgedaan en werd samen met een tandarts-prothetist gezocht naar de optimale methode om het ontstane defect te herstellen.
GegevenEen 32-jarige vrouw werd door haar tandarts verwezen naar een mond-, kaak-en aangezichtschirurg vanwege een pijnloze zwelling rondom gebitselementen 34 en 35. Zij had sinds enkele weken de langzaam in omvang toenemende zwelling bemerkt. De tandarts had 8 maanden voorafgaande aan de verwijzing nog routinematig röntgendiagnostiek uitgevoerd waarbij in de desbetreffende regio geen afwijkende bevindingen waren opgemerkt. De patiënt ondervond geen pijn of sensibiliteitsverlies. Zij had in het bijzonder hinder van de steeds verder in omvang toenemende zwelling en wilde graag verdere diagnostiek naar en behandeling van de afwijking.
AnamneseDe patiënt had een blanco voorgeschiedenis. Bij intraoraal onderzoek werd een goed onderhouden dentitie gezien. Zowel linguaal als buccaal ter plaatse van gebitselementen 34 en 35 bevond zich een pijnloze, vast-elastisch aanvoelende, bolronde, blauw-doorschemerende afwijking (afb. 1). Er was geen sprake van verhoogde mobiliteit van de desbetreffende gebitselementen, percussiepijn of pockets en beide gebitselementen reageerden positief op de sensibiliteitstest. De sensibiliteit in het verzorgingsgebied van de nervus alveolaris inferior aan de linkerzijde was ongestoord.Op een vervaardigde panoramische röntgenopname werd tussen de wortels van beide premolaren een circumscripte radiolucentie gezien die reikte tot het foramen mentale. De radices divergeerden als gevolg van het aanwezige proces. Wortelresorptie was op de röntgenopname niet zichtbaar (afb. 2).Om de mogelijke relatie van de afwijking met het foramen mentale en de canalis mandibulae als ook de uitbreiding in anterieure en posterieure richting te kunnen beoordelen, werd een cone beam-computertomogram (cone beam-CT) vervaardigd. De verkregen data werden afgebeeld in dwarsdoorsneden en in een driedimensionale weergave (afb. 3). Daarop was duidelijk te zien dat het foramen mentale aan de afwijking grensde, maar er geen onderdeel vanuit maakte. Het dak van de canalis mandibulae, iets meer posterieur gelegen, liet op de dwarsdoorsneden daarentegen wel een relatie met de afwijking zien.
