Context:Stress is a state of mental or emotional strain or tension, which can lead to underperformance and adverse clinical conditions. Adaptogens are herbs that help in combating stress. Ayurvedic classical texts, animal studies and clinical studies describe Ashwagandha as a safe and effective adaptogen.Aims:The aim of the study was to evaluate the safety and efficacy of a high-concentration full-spectrum extract of Ashwagandha roots in reducing stress and anxiety and in improving the general well-being of adults who were under stress.Settings and Design:Single center, prospective, double-blind, randomized, placebo-controlled trial.Materials and Methods:A total of 64 subjects with a history of chronic stress were enrolled into the study after performing relevant clinical examinations and laboratory tests. These included a measurement of serum cortisol, and assessing their scores on standard stress-assessment questionnaires. They were randomized to either the placebo control group or the study drug treatment group, and were asked to take one capsule twice a day for a period of 60 days. In the study drug treatment group, each capsule contained 300 mg of high-concentration full-spectrum extract from the root of the Ashwagandha plant. During the treatment period (on Day 15, Day 30 and Day 45), a follow-up telephone call was made to all subjects to check for treatment compliance and to note any adverse reactions. Final safety and efficacy assessments were done on Day 60.Statistical Analysis:t-test, Mann-Whitney test.Results:The treatment group that was given the high-concentration full-spectrum Ashwagandha root extract exhibited a significant reduction (P<0.0001) in scores on all the stress-assessment scales on Day 60, relative to the placebo group. The serum cortisol levels were substantially reduced (P=0.0006) in the Ashwagandha group, relative to the placebo group. The adverse effects were mild in nature and were comparable in both the groups. No serious adverse events were reported.Conclusion:The findings of this study suggest that a high-concentration full-spectrum Ashwagandha root extract safely and effectively improves an individual's resistance towards stress and thereby improves self-assessed quality of life.
Introduction:Percutaneous destruction of cancer cells using a radiofrequency energy source has become an accepted part of the modern armamentarium for managing malignancies. Radiofrequency ablation (RFA) is a relatively novel procedure for treating recurrent and metastatic tumors. It is used for debulking tumors and as adjuvant therapy for palliative care apart from its role as a pain management tool. Its use in the third world countries is limited by various factors such as cost and expertise. In the remotest parts of India, where economic development has been slow, abject poverty with poor health care facilities advanced malignancies present a challenge to health care providers. We undertook this study to assess the safety of the percutaneous RFA tumor ablation as a therapeutic or palliative measure in patients where surgery was not possible. We observed that RFA may be an effective, alternative therapeutic modality for some inoperable tumors where other therapeutic modalities cannot be considered.Context:Palliative and therapeutic image-guided RFAs of tumors may be the only treatment option in patients who are inoperable for a variety of reasons. To assess the safety and complications of RFA in such a patient population is important before embarking upon any interventions given their physically, mentally, and socially compromised status in a country such as India.Aims:To assess the safety of percutaneous image-guided radiofrequency tumor ablation and to note the various immediate and early complications of the intervention.Settings and Design:This was a prospective, observational study conducted in Tata Main Hospital, Jamshedpur, Jharkhand, India.Subjects and Methods:After approval by the Hospital Approval Committee all patients who consented for percutaneous RFA of their tumor admitted in the hospital were included after taking fully informed consent from patient/close relative keeping the following criteria in view.Inclusion Criteria:Patients who were likely to derive a direct benefit in the survival or as a palliative measure for relief in their symptoms and patients who were inoperable because of any of the following reasons: (1) Exhausted conventional treatment options, (2) technical and anatomical contraindications to conventional treatment, (3) medical comorbidities precluding surgery, (4) patient refusal, (5) recurrent tumors, and (6) advanced tumor stage. Conventional Treatment has been defined as surgical resection, radiotherapy, and/or chemotherapy, although the patient eligibility for each treatment may vary.Exclusion Criteria:Patients with the following were excluded: (1) Severe coagulopathy, (2) heart, renal, or liver failure, (3) lesions within 1 cm of gall bladder, hilum, bowel wall, and major blood vessels, (4) patient with any metal implant, (5) patients in sepsis, and (6) tumor adjacent to structures at risk (main bile ducts, pericardium, stomach, or bowel).Results:The duration of procedure as well as ablation of tumor free margin was significantly related to the size of the tumor. Cancer is second in all the leading causes of morbidity and mortality worldwide, with more or less 13% in total deaths worldwide in 2018. Total deaths due to cancer in India was 7, 84,821 in 2018. Estimated number of people living with the disease is around 2.25 million and every year new cancer patients registered are 11, 57294 lakh. The number of recent cases is predicted to rise by concerning seventieth over consequent a try of decades. Globally, nearly one in six deaths is because of cancer. The present study was used to assess the effectiveness of progressive muscle relaxation technique on physical symptoms among patients receiving chemotherapy. The data from a sample of 20 cancer patients receiving chemotherapy admitted in shri guru ramdass hospital of Amritsar, Punjab. 10 patients in experimental group and 10 patients in control group were selected using purposive sampling technique. The results of the study showed highly significant difference in pre-interventional and post-interventional physical symptoms of experimental and control group. The Z value (Wilcoxon signed rank test) between pre-intervention and post-intervention in experimental group related to nausea was 2.897 which were found statistically significant at p< 0.05. The Z value between pre-intervention and post-intervention in control group related to nausea was 1.000 which was found statistically nonsignificant in control group at p>0.05. Hence, it was concluded that in control group physical symptoms were not relieved. The Z value between pre-intervention and post-intervention in experimental group in experimental group related to vomiting was 2.879 which were found highly significant at p< 0.05. Hence, it was concluded that progressive muscle relaxation technique has significant effect in relieving physical symptoms. The Z value between pre-intervention and post-intervention in control group related to vomiting was 1.000 which was found non-significant at p< 0.05. Hence, it was concluded that in control group physical symptoms were not relieved. The Z value between pre-intervention and post-intervention in experimental group related to pain was 2.899 which was found highly significant at p<0.05. The Z value between preintervention and post-intervention in control group related to pain was 0 which was found non-significant at p<0.05. Hence, it was concluded that progressive muscle relaxation technique has significant effect in relieving physical symptoms.
The moment of birth is both joyous and beautiful. Birth is a unique dynamic process, fetal and maternal physiologies interact symbiotically. Every woman who became pregnant has to undergo the process of delivery. In normal process of delivery, the baby is delivered per vagina; an episiotomy is performed on vagina by health care provider or midwife. The present study was under taken to assess the effectiveness of medicated and non medicated sitz bath on episiotomy wound healing among postnatal mothers at Govt. SMGS Maternity Hospital, Jammu (J&K).The sample consisted of 40 postnatal mothers (20 in experimental and 20 in control group). Purposive sampling technique was used to select the sample. Socio-demographic profile, obstetrical history variables tool and REEDA scale were used to collect the data from subjects. The results revealed that both medicated and non medicated sitz bath are equally effective in episiotomy wound healing among postnatal mothers. Thus, sitz bath either medicated or non medicated should be encouraged among postnatal mothers so as fasten the episiotomy wound healing and easy recovery of postnatal mothers.
