The aim of the study was to assess whether general practitioners (GPs) monitor and evaluate the health behavior of their patients in the field of a diet, physical activity, and weight control, and whether they provide appropriate counselling as part of this evaluation. Predictors of those activities among physicians were also determined. The cross-sectional study was conducted in the Piotrkowski district among 200 GPs. The questionnaire covered socio-demographic data and lifestyle characteristics of the physicians, their role as healthy lifestyle providers, and whether they assess lifestyle characteristics of their patients and perform healthy lifestyle counselling. More than 60% of the GPs did not evaluate lifestyle features during their patients’ examination. In total, 56% of the GPs provided healthy lifestyle recommendations among patients who have not been diagnosed with chronic lifestyle-related diseases but who do not follow healthy recommendations, and 73% of GPs provided recommendations to patients with chronic diseases related to lifestyle. The study showed that the chance to assess lifestyle characteristics of the patients was significantly higher for the GPs who believed that they were obliged to do so (OR = 6.5; p = 0.002). The chance to recommend a healthy lifestyle among patients who have not been diagnosed with chronic lifestyle-related diseases but who do not follow healthy recommendations was 5.9 times higher among the GPs working in the public sector (p < 0.001) and 16.3 times higher for these who believed that they had sufficient knowledge to provide the advice (p = 0.02). The following predictors of providing a healthy lifestyle counselling among patients with diagnosed chronic lifestyle-related diseases were identified: conviction that a GPs is obligated to provide it (OR = 4.4; p = 0.02), sufficient knowledge (OR = 8.7; p = 0.01), and following health recommendations by themselves (OR = 3.9; p = 0.04). Conclusions: The identified predictors are crucial for the development of appropriate strategies aiming at increasing GPs’ involvement in preventive measures and consequently at improving the population’s health.
Telemedicine involves diagnostic, therapeutic and educational services being offered remotely by healthcare professionals to exchange crucial clinical information. It is a rapidly developing form of medical activity and part of medical industry, with advanced technologies already available in Poland. Cardiology is one of the fields in which telemedicine methods were pioneered and introduced into everyday practice. Some of these methods have already become standard procedures for diagnosis and treatment in some Polish centers, with other soon to follow. Clinical study results not only demonstrate reliability and usefulness of telemedicine technologies but also show that their use in clinical practice improves the patients' prognoses and quality of life. Moreover, study results in highly developed countries show a potential cost-effectiveness of telemedicine from the perspective of healthcare systems. There is an unquestionable need to establish clear rules for telemedicine use in Poland, which would ensure their high quality and adequate clinical application. This paper is a summary of the current status of telemedicine solutions used in cardiology, with a particular focus on the Polish healthcare system, and presents both the commonly available solutions and those that are expected to develop rapidly in the near future.

Wstęp: Celem pracy jest analiza regulacji prawnych, które odnoszą się do obowiązku zachowania tajemnicy lekarskiej i możliwości jej uchylenia poprzez przekazanie informacji na temat stanu zdrowia pacjenta jego pracodawcy. Realizacja celu głównego wymaga udzielenia odpowiedzi na następujące pytania: Czy pracodawca może uzyskać szczegółowe dane na temat stanu zdrowia pracownika, wykraczające poza zamieszczoną w zaświadczeniu lekarskim informację o istnieniu lub braku przeciwwskazań do wykonywania pracy? Czy lekarz ma prawo uchylić tajemnicę lekarską dotyczącą stanu zdrowia pracownika, jeśli choroba rozpoznana poza badaniami profilaktycznymi w sposób bezpośredni utrudnia/uniemożliwia wykonywanie pracy na dotychczasowym stanowisku? Materiał i metody: Materiał do niniejszej pracy stanowią obowiązujące w Polsce regulacje prawne odnoszące się do obowiązku zachowania tajemnicy lekarskiej i określające zakres informacji na temat stanu zdrowia pacjenta, które mogą zostać przekazane jego pracodawcy. Wyniki: Lekarz nie ma prawa przekazać pracodawcy informacji na temat przeciwwskazań do wykonywania określonej pracy, jeżeli do ich uzyskania doszło w trybie udzielania świadczeń zdrowotnych niekwalifikujących się do katalogu badań wstępnych, profilaktycznych lub kontrolnych. Podstawy przekazania pracodawcy przez lekarza informacji nie może stanowić wyjątek od obowiązku zachowania tajemnicy lekarskiej wskazany w art. 40 ust. 2 pkt 1-6 Ustawy z dnia 5 grudnia 1996 r. o zawodach lekarza i lekarza dentysty. Wymieniono tam enumeratywnie przypadki dopuszczalnego uchylenia tajemnicy zawodowej, co uniemożliwia stosowanie powyższego artykułu w przypadkach innych niż ściśle określone w jego treści. Wnioski: Polskie prawo definiuje nieliczne przypadki, w których dopuszczalne jest przekazanie pracodawcy informacji dotyczących stanu zdrowia pracownika. W większości sytuacji uzyskanie informacji uzależnione jest od przeprowadzenia badań w ściśle określonym trybie. Gdy do stwierdzenia przeciwwskazań do wykonywania określonej pracy dochodzi podczas udzielania świadczeń zdrowotnych niemających związku z badaniami pracowników sensu stricto, informacja ta nie może zostać przekazana pracodawcy.
