Die häufigste Ursache der Müttersterb-lichkeit (30-40%) in entwickelten Län-dern ("developed countries") ist -160 Jahre nach Semmelweis Entdeckung des Übertragungswegs des Kindbettfieberseine andere Erkrankung, über die in der geburtsmedizinischen Weiterbildung wenig (deutlich weniger als über typische geburtsmedizinische Komplikationen) geredet wird: die Lungenarterienembolie, eine "internistische" Erkrankung. > Die Lungenembolie ist die häufigste Ursache der Müttersterblichkeit in entwickelten Ländern Wir wissen, dass sich das Thromboserisiko in der Schwangerschaft vervierfacht [1, 2] und im Wochenbett mehr als vervierzehnfacht. Bei einer Schnittentbindung erhöht sich das Risiko (im Vergleich zur vaginalen Entbindung) noch einmal bis auf das Zehnfache, und noch einmal mehr im Fall einer notfallmäßigen Sectio caesarea. Niedrig dosierte Ovulationshemmer erhöhen das Risiko von Venenthrombosen um den Faktor 2-5 [3], eine Hormonersatztherapie scheint das Thrombembolierisiko um den Faktor 2-4 zu erhöhen [4]. Diese dubiose Rolle der Lungenarterienembolie ist allerdings keineswegs auf die Geburtsmedizin beschränkt; die Lungenarterienembolie ist die häufigste schwerwiegende übersehene Erkrankung im Krankenhaus. Die Lungenarterienembolie präsentiert sich in changierendem Mimikry. So einfach ihre Diagnose ist, sobald sie sich typisch manifestiert, so rät-selhaft ist sie, falls Mosaiksteine fehlen oder zu fehlen scheinen. > Die Lungenarterienembolie präsentiert sich in changierendem Mimikry Dabei ist die Lungenarterienembolie ein gleichermaßen oft simples wie undurchschaubares Geschehen: die typische Lungenarterienembolie F bei dokumentierter massiver Lungenarterienembolie werden die verschiedenen Leitsymptome und -befunde in folgenden Häufigkeiten beschrieben: 1 96% Tachypnoe (Atemfrequenz >16/min), 1 58% Rasselgeräusche, 1 53% betonter zweiter Herzton (S 2 P), 1 44% Tachykardie (Herzfrequenz >100/min), 1 43% Fieber (Temperatur >37,8° C), 1 36% ausgeprägtes Schwitzen (Diaphorese); F eine massive Lungenarterienembolie führt über das akute Cor pulmonale zu einer hypotensiven Reaktion, aber die ersten Untersuchungsbefunde bei einem nicht massiven Verlauf können vollständig normal sein; F neu aufgetretenes Giemen und ein Pleurareiben, falls die pleuralen Lungenoberflächen betroffen sind, eine ansonsten unerklärliche neue Schmerzhaftigkeit der Brustwand ohne vorausgegangene Verletzung (in manchen Fällen das einzige Symptom); F nach 2-3 Tagen triggert der Verlust von Surfactant häufig Atelektasen und Infiltrate, welche klinisch und röntgenologisch nicht von pneumonischen Infiltraten zu unterscheiden sind; ist schnell zu erkennen. Die, die sich nicht an diese typischen Merkmale hält, ja, die kein einziges davon aufweist, ist aber keinesfalls immer harmlos, sondern häu-fig die Vorbotin eines kurzfristig eintretenden lebensbedrohlichen Rezidivs. Jeder Intensivmediziner kennt die Fälle, die all die oben aufgeführten Kriterien vermissen ließen, und trotzdem dramatisch verliefen.
