Résumé -Introduction :La famille des herpesviridae comprend de nombreux virus dont l'herpes simplex virus (HSV), le virus varicelle-zona (VZV), le cytomégalovirus (CMV), le virus d'Epstein-Barr (EBV). Le HSV, de part sa prévalence élevée (présent chez 90 % de la population adulte), est le plus fréquemment rencontré. Après la primo-infection, le HSV entre dans une phase de latence, puis se réactive sous l'influence de différents facteurs. Trois cas de récurrence herpétique après un geste opératoire de pratique bucco-dentaire quotidienne sont rapportés dans cet article. Cas rapportés : Le premier cas est celui d'un homme de 68 ans qui présente une semaine après l'avulsion de 25 et 27, des vésicules confluentes, non douloureuses, de type herpétique, sur la muqueuse palatine, en regard du site opératoire. Le second cas est celui d'une femme de 89 ans, sans altération de son état général de santé, présentant une semaine après une séance de soins prothétiques (préparation corono-radiculaire et réalisation d'une dent provisoire sur la 16) une multitude de vésicules érosives, non douloureuses, en regard de cette dent. Dans ces deux cas, à des fins pédagogiques, le diagnostic de récurrence herpétique à HSV-1 a été confirmé par analyse biologique. Aucun traitement antiviral n'a été prescrit. Le troisième cas est celui d'une patiente de 55 ans ayant bénéficié d'un retraitement endodontique chez son chirurgien-dentiste traitant. Quelques jours plus tard, elle a présenté un placard génien érythémateux associé à des vésicules douloureuses et des croûtes situées sur le territoire d'innervation du V2. Ces lésions caractéristiques ont fait évoquer un zona qui a été confirmé par la sérologie. La patiente a été hospitalisée et mise sous traitement antiviral. Discussion : Le mécanisme de la réactivation virale est multifactoriel : l'inflammation locale, les micro-traumatismes opératoires, la peur et le stress du patient semblent constituer des facteurs favorisants. Dans certains cas, la récurrence virale est douloureuse : elle nécessite alors un traitement médi-camenteux. L'utilisation d'antiseptiques locaux et d'antiviraux (aciclovir et valaciclovir) est conseillée et utilisée en prophylaxie dans les cas de récidive. Les études sur le sujet sont peu nombreuses et toutes s'accordent pour dire que la survenue de ces complications reste rare. Cependant le phénomène existe et il est important de le connaître.Abstract -Viral recurrences after dental procedures: 3 case reports and review of literature. BackgroundPostnatal depression (PND) is one of the most frequent complications in women of childbearing age in the developed world. The onset of PND is influenced by several risk factors. In an attempt to avoid unnecessary long maternity stays, a ‘Short Stay in Maternity Programme’ was installed, shifting care from the hospital environment to the outpatient setting.AimsIn order to develop an efficient program to trace vulnerable women after childbirth and to provide support within primary care, we aimed to inventory the risk factors for PND within the population of women participating in the short stay program.Design and settingThis study is a cross sectional study without follow-up and women, included in the ‘Short Stay in Maternity Program’, were invited by e-mail to participate within three months after delivery.MethodThe questionnaire addressed background features and feelings during the maternity period supplemented with the validated Dutch version of the Edinburgh Postnatal Depression Scale (EDPS). The primary outcome measure of the questionnaire was the score on the EDPS.Results138 (27.3%) of the invited women participated. 16 participants (12.4%) presented with a positive score on the EPDS. The odds ratio for a positive score on the EDPS when experiencing negative feelings was 13.5 (95% CI 4.14-44.01). If the only provided material support, the odds ratio for a positive EPDS-score was 11.2 (95%CI 2.72-55.5).ConclusionsIn this study, we identified two risk factors for PND: negative feelings during pregnancy and the provision of only material support by the partner.
Résumé -Les complications bucco-dentaires des intubations oro-trachéales sont fréquentes mais aucune étude importante n'a été conduite sur le sujet. Le but de cet article est de dresser une liste non exhaustive des complications pouvant survenir au décours d'une intubation tels que l'oedème lingual -souvent qualifié de macroglossie -, l'ulcération, la nécrose ou encore les morsures ainsi que les facteurs de risque morphophysiologiques, pathologiques ou techniques qui y sont associés. Le diagnostic différentiel de ces complications est également abordé. À travers deux cas cliniques, un de macroglossie et un de morsure linguale, les mesures de protection des tissus mous buccaux sont exposées. Les patients intubés et sédatés ne contrôlent plus leurs mouvements musculaires et ils peuvent effectuer des mouvements masticatoires anarchiques induisant de graves lésions linguales, voire labiales. Des protections dentaires siliconées peuvent être réalisées et mises en place, sans étape de laboratoire, directement au lit du malade pour protéger les tissus mous et ainsi faciliter la cicatrisation et les soins d'hygiène bucco-dentaire. Abstract -Complications during prolonged oro-tracheal intubations: literature review and two case reports.Complications on oral tissues of the oral-tracheal intubation is common but no studies of large extent has been conducted on the subject. The purpose of this paper is to develop a non-exhaustive list of complications that may occur during an intubation such as a lingual oedema, also called macroglossia, ulceration, necrosis or bites, and morpho-physiological, pathological or technics risk factors associated. The differential diagnosis of these complications is discussed. Through two clinical cases, one of macroglossia and another lingual bite, simple solutions for the protection of oral macroglossia and one lingual bite were exposed. Uncontrolled chewing movements can cause serious lingual or labial injuries. These silicone protections are made, without laboratory stage, directly on the patient to protect the soft tissues and facilitate cicatrisation and care of oral health.
