A reabsorção radicular de dentes decíduos se caracteriza pela perda de parte da estrutura dentária e é determinada fisiologicamente pela erupção do dente sucessor ou patologicamente por lesão externa ou interna. Afeta o cemento, a dentina e, às vezes, até o osso alveolar. As atipias observadas nas reabsorções radiculares podem derivar de um processo inflamatório subsequente a trauma ou provocado por pressão de algum agente lesivo, como um dente em erupção ectópica, e quase invariavelmente complicam a cronologia e a sequência de erupção normal. Perdas precoces de dentes decíduos costumam gerar problemas na prática clínica, possibilitando redução do comprimento do arco, inclinações e rotações do dente sucedâneo e dos vizinhos. O objetivo deste trabalho é relatar um caso de perda precoce do dente 75 por reabsorção radicular externa seguida de infecção e necrose pulpar em consequência da erupção do dente 34, bem como tecer breve discussão sobre erupções ectópicas e o impacto das perdas precoces de dentes decíduos. Com base no relato e na discussão do caso, conclui-se que é necessário que os cirurgiões-dentistas tenham conhecimento sobre as reabsorções radiculares atípicas e estejam habilitados para o diagnóstico precoce e a intervenção adequada. Entende-se, ainda, que mais estudos sobre a condição se fazem necessários para uma melhor compreensão dos mecanismos fisiopatológicos envolvidos nessas reabsorções radiculares que fogem ao padrão fisiológico.
