Objective. To analyze whether the expression and modulation of T cell receptor (TCR) signaling is dependent on Casitas B lineage lymphoma b (Cbl-b) in T cells from patients with systemic lupus erythematosus (SLE) upon stimulation with a tolerogenic substance.Methods. Peripheral blood mononuclear cells were obtained from 20 patients with SLE (active disease or in remission) and 20 healthy controls. Levels of Cbl-b expression were measured using reverse transcriptionpolymerase chain reaction and Western blotting in peripheral CD4؉ T cells from SLE patients and healthy controls upon anergy induction. Cell proliferation was measured using the carboxyfluorescein diacetate succinimidyl ester dilution method. Cytokine production was analyzed by luminometry, and surface expression of activation markers was assessed by flow cytometry. Transfection assays were performed to induce overexpression of Cbl-b, and phosphorylation of TCRassociated kinases was evaluated.Results. CD4؉ T cells from SLE patients displayed resistance to anergy (as evidenced by increased cell proliferation, interleukin-2 production, and expression of activation and costimulatory markers), and this was associated with altered Cbl-b expression. Upon ionomycin treatment, primary T cells showed enhanced MAPK activity and decreased Akt phosphorylation, which was representative of the anergic state. In T cells from lupus patients, Cbl-b overexpression led to increased expression of phosphorylated MAPK, thus indicating the reversibility of anergy resistance.Conclusion. These findings suggest that abnormal peripheral tolerance in SLE is caused by a deficiency in Cbl-b, and that this ubiquitin ligase plays a key role in regulating TCR signaling during the induction of peripheral tolerance.The ubiquitination system mediates highly specific and regulated posttranslational modifications through the conjugation of ubiquitin to a protein substrate in a highly ordered sequence of enzyme reactions. This system is involved in a diverse array of cellular processes, including modulation of the immune response. Both proteolysis-dependent and -independent mechanisms have been implicated in ubiquitination (1). Ubiquitin ligases confer specificity to the ubiquitination system, as they catalyze the transfer of activated ubiquitin molecules to the corresponding residue (primarily, lysine) in the substrate, and their relationship to the immune system is based on their capacity to modulate peripheral tolerance mechanisms such as anergy and the differentiation of regulatory T cells (2). In particular, the role of ubiquitin or E3 ligases, such as Casitas B lineage lymphoma b (Cbl-b), gene related to anergy in lymphocytes (GRAIL), and Itch, as negative regulators of the immune response has been well characterized. Introducción: La nefrectomía citorreductiva (NC) podría mejorar la supervivencia de pacientes con carcinoma de células renales (CCR) metastásico. No obstante, debe ponderarse el riesgo de complicaciones perioperatorias importantes que obliguen a replantear la estrategia terapéutica. Con el objetivo de identificar factores predictores de morbilidad y mortalidad perioperatoria en NC, analizamos a pacientes con CCR metastásico tratados quirúrgicamente en nuestra institución. Material y métodos: Se realizó un estudio retrospectivo de pacientes con CCR metastásico sometidos a NC entre 1980-2016. Mediante un análisis univariante y multivariante de regresión logística binomial evaluamos predictores de efectos adversos en los primeros 30 días a la cirugía. Consideramos estadísticamente significativa una p < 0.05. Resultados: Incluimos 46 pacientes con CCR estadio clínico IV. El 32.6% presentó alguna complicación, un 17.4% fueron grado ≥ 3 de Clavien-Dindo. La mortalidad perioperatoria fue del 15.2%. En el análisis multivariante, la edad ≥ 65 años demostró ser el único factor de riesgo para complicaciones perioperatorias (p = 0.001). Ningún factor se asoció de manera independiente a mortalidad. Conclusiones: En pacientes ≥ 65 años de edad con CCR metastásico, la nefrectomía implica mayor riesgo de complicaciones y mortalidad perioperatoria, por lo que en esta población debe evaluarse cuidadosamente el beneficio del tratamiento quirúrgico.
La ureteroscopia es un procedimientos comúnmente efectuado en pacientes con diversos padecimientos del aparato urinario superior. Debido al progreso en la tecnología y desarrollo de destrezas médicas se ha incrementado su utilización en todo el mundo, por lo que actualmente representa un método fundamental en el área de la urología. Sin embargo, como cualquier otro procedimiento quirúrgico, la ureteroscopia no está exenta de complicaciones, de tal modo que el conocimiento y experiencia es de suma importancia para la práctica urológica. Por tanto, el objetivo de este estudio fue revisar las complicaciones más frecuentes en la práctica de la ureteroscopia. PALABRAS CLAVE: Ureteroscopia; complicaciones intraoperatorias; urolitiasis.
OBJETIVO: Determinar la prevalencia de vejiga hiperactiva y su repercusión (grado de afectación) en la calidad de vida de pacientes captados en cinco centros hospitalarios de la Ciudad de México. MATERIALES Y MÉTODOS: Estudio descriptivo, transversal y observacional efectuado mediante la recolección prospectiva de expedientes provenientes de la consulta externa de Medicina general, Medicina familiar o Medicina interna de cinco centros hospitalarios de la Ciudad de México, entre enero y diciembre de 2017. Se solicitó a los participantes responder el cuestionario ICIQ-OAB (versión validada en español), en una sola ocasión y durante la misma visita. Los pacientes se dividieron en grupos etarios decenales a partir de 18 años. Para el análisis de los datos se utilizó estadística descriptiva. RESULTADOS: Al momento del análisis intermedio, se contó con las encuestas de 352 participantes. El 72% (n = 253) reportó urgencia urinaria, como síntoma cardinal de vejiga hiperactiva (78 vs 66% en mujeres y hombres, respectivamente, p<0.05). Los síntomas moderados-severos se reportaron en 16% de los pacientes mayores de 57 años de edad. Los síntomas severos fueron más frecuentes en mujeres (68%). El 31% de los casos refirió afectación moderada-severa en la calidad de vida. El principal síntoma concomitante fue nicturia (81%); sin embargo, la incontinencia urinaria de urgencia provocó mayor molestia en los pacientes (32%). CONCLUSIÓN: La urgencia urinaria (síntoma cardinal de la vejiga hiperactiva) es una alteración con elevada prevalencia en la población de la Ciudad de México.
