Diabetes Mellitus is a severe, chronic disease that occurs when blood glucose levels rise above certain limits. Over the last years, machine and deep learning techniques have been used to predict diabetes and its complications. However, researchers and developers still face two main challenges when building type 2 diabetes predictive models. First, there is considerable heterogeneity in previous studies regarding techniques used, making it challenging to identify the optimal one. Second, there is a lack of transparency about the features used in the models, which reduces their interpretability. This systematic review aimed at providing answers to the above challenges. The review followed the PRISMA methodology primarily, enriched with the one proposed by Keele and Durham Universities. Ninety studies were included, and the type of model, complementary techniques, dataset, and performance parameters reported were extracted. Eighteen different types of models were compared, with tree-based algorithms showing top performances. Deep Neural Networks proved suboptimal, despite their ability to deal with big and dirty data. Balancing data and feature selection techniques proved helpful to increase the model’s efficiency. Models trained on tidy datasets achieved almost perfect models.
Un aspecto importante en la metodología de la investigación, es el cálculo de la cantidad de participantes que deben incluirse en un estudio. El tamaño de muestra permite a los investigadores saber cuántos individuos son necesarios estudiar, para poder estimar un parámetro determinado con el grado de confianza deseado, o el número necesario para poder detectar una determinada diferencia entre los grupos de estudio, suponiendo que existiese realmente.El cálculo del tamaño de la muestra es una función matemática que expresa la relación entre las variables, cantidad de participantes y poder estadístico.La muestra de un estudio debe ser representativa de la población de interés. El objetivo principal de seleccionarla es hacer inferencias estadísticas acerca de la población de la que proviene. La selección debe ser probabilística.Los factores estadísticos que determinan el tamaño de la muestra son: hipótesis, error alfa, error beta, poder estadístico, variabilidad, pérdidas en el estudio y el tamaño del efecto. Se revisan las fórmulas utilizadas para el cálculo del tamaño de la muestra en las situaciones más frecuentes en investigación, así como la revisión de fórmulas para un cálculo más rápido. Se incluyen ejemplos de investigación en educación médica. También se revisan aspectos importantes como: tamaño de la muestra para estudios piloto, estrategias para disminuir el número necesario de sujetos, y software para el cálculo del tamaño de muestra.
Importance and conservation challenges of Vanilla spp. (Orchidaceae) in Costa Rica. The aim of this work was to describe the importance and the main challenges for the use, management and conservation of the Vanilla genus in Costa Rica. Topics covered in this study were: taxonomy in the country and worldwide, data collection and new estimates of wild relatives, the economic, ecological and social importance, the gene pool of the Costa Rican species, the crop situation worldwide (harvested area, production, yield, major producers), causes that reduce the productive potential of the species, culture methods, the vulnerability of Vanilla planifolia to extinction, and the genetic potential in the country with respect to the concentration of vanillin. The comprehensive conservation strategy proposed in this paper aims to perpetuate vanilla plant genetic resources in gene banks through a formal system of seed production, for further use for breeding, research and commercial production. Through sustainable use, those species will be protected in Costa Rica. The natural or legal persons who use such materials must generate monetary or non-monetary benefits from the use of resources through a fair and equitable system to the suppliers. Agro-ecological conditions of Costa Rica as well as the genetic material of both wild and cultivated vanilla opens the possibility of developing production models based on organic agriculture.

Introducción y objetivos: la terapia con antioxidantes durante la quimioterapia y radioterapia en pacientes con cáncer cervicouterino es controvertida. Mientras existe evidencia que sugiere que el uso de antioxidantes disminuye los efectos secundarios propios del tratamiento contra el cáncer, hay datos que sugieren que los antioxidantes incrementan el riesgo de recurrencia de cáncer por la afectación de la terapia de los tratamientos. Métodos: se dirigió un estudio clínico controlado en pacientes con cáncer cervicouterino que fueron suplementados con una mezcla de antioxidantes o placebo, con seguimiento por 4 años posteriores al término de su tratamiento antineoplásico para evaluar el efecto de los antioxidantes en la recurrencia. Tomamos datos de niveles de hemoglobina y albúmina. Se analizaron las diferencias entre grupos con la prueba de Chi-cuadrado, la sobrevida se calculó con un análisis multivariado por medio de regresión de COX. Resultados: se dio seguimiento a 103 pacientes con cáncer cervicouterino en etapa clínica IIB y IIIB de los cuales 48% fueron tratados con suplementación de antioxidantes y el 52% con placebo, originalmente y de estos se dio seguimiento a 88 pacientes durante 4 años. El 23,9% de los pacientes tratados presentaron recurrencia por cáncer mientras que el 76,1% no la presentó. El 21,6% de los pacientes presentaron metástasis, el 8% de los pacientes perteneció al grupo de antioxidantes y el 15,9% al grupo placebo (p > 0,05). Implicaciones para los pacientes supervivientes: la suplementación con antioxidantes aparentemente no interfiere con la recurrencia por cáncer, sin embargo no hay evidencia suficiente para probarlo. Posiblemente una dosis distinta sea la clave para un mejor efecto, pero serán necesarios futuros estudios que prueben efectos sobre otro tipo de dosis. Conclusiones: la suplementación con antioxidantes durante el tratamiento de pacientes con cáncer cervicouterino no tiene efectos en la recurrencia por cáncer a 4 años de seguimiento.
