La pandemia de COVID-19 ha traído una nueva afección grave e inusual denominada Síndrome Inflamatorio Multisistémico en niños, de la cual aún hay mucho por conocer. Presentamos una serie de 8 casos atendidos en el Instituto Nacional de Salud del Niño, Lima Perú. La edad media fue 5,1 años. La presentación clínica incluyó fiebre, problemas gastrointestinales agudos, afectación ocular y mucocutánea. Cuatro cumplieron criterios para Enfermedad de Kawasaki clásica. Todos tuvieron serología positiva para SARS-CoV-2, hemograma patológico, marcadores inflamatorios elevados y pruebas de coagulación alteradas. Cinco casos presentaron hipertransaminasemia y tres retención nitrogenada. Cuatro casos cumplieron criterios para Síndrome de Activación Macrófagica. Todos recibieron inmunoglobulina intravenosa, corticoides y ácido acetil salicílico. Ninguno desarrolló aneurismas coronarios. Solo uno presentó miocarditis, shock y requirió ingreso a Unidad de Cuidados Intensivos. La mayoría evolucionaron favorablemente. En todo niño con fiebre, síntomas gastrointestinales y dermatológicos; asociado a exposición al SARS-CoV-2, debe investigarse compromiso multisistémico.
COMPLICATED CHICKENPOX IN A NATIONAL PEDIATRIC PERUVIAN HOSPITAL, 2001-2011 ABSTRACTThe objective of the study was to describe the clinical and epidemiological characteristics of complicated chickenpox cases seen at the National Institute of Children's Health (INSN, Spanish acronym) of Peru from 2001 to 2011. A case series was collected, including a total of 1,073 children with complicated chickenpox. The median age was 2.5 years (IQR 1.1-4.8 years), of which 578 (54%) were male. The most frequent complications were secondary skin and soft tissue infections with 768 cases (72%). 13 deaths (1.4%) were recorded. In conclusion, the hospitalizations due to complicated chickenpox in the INSN included mostly children under five, with a short stay and a low proportion of deaths most complications being related to secondary skin and soft tissue infections.
Objetivo. Describir los casos por infección por Bartonella henselae como presentación de fiebre de origen desconocida (FOD) en el Instituto Nacional de Salud del Niño (INSN) de Perú. Material y métodos. Estudio de serie de casos de niños atendidos en el INSN en el año 2012. Se definió infección por B. henselae si el caso presentaba serología positiva. Resultados. En el año 2012 se diagnosticaron 26 casos de FOD, de los cuales 12 fueron por infección por B. henselae. De estos 12, 6 fueron masculinos, con una mediana para la edad de 5 años (RIQ 3,5-4,0); 11 tenían una historia de exposición a gatos; 6 tuvieron lesión lineal ocasionada por el gato. Dentro de los exámenes de laboratorio, 4 de 12 tuvieron leucocitosis; 5 presentaron una PCR mayor de 10. El examen ecográfico reveló que 8 de 12 presentaron lesiones hipoecoicas en bazo; existió coinfección con otras infecciones; 2 casos tuvieron manifestación articular y 1, convulsión. Conclusión. Se concluye que los casos de FOD por infección por B. henselae en niñ os, están relacionados con exposición a gatos y lesiones hipoecoicas en bazo e hígado, con una evolución de la enfermedad con pronóstico bueno.
