Introdução: A gravidez é um evento resultante da fecundação do ovócito pelo espermatozóide. Este é um momento de diversas transformações no organismo materno, como o aumento de demandas nutricionais, dessa forma sendo fundamental para o harmônico desenvolvimento fetal uma nutrição materna adequada. Relacionado aos aspectos da alimentação, constata-se que em todas as regiões brasileiras, uma porcentagem considerável dos brasileiros já deixou de comer carne, por vontade própria, pelo menos uma vez na semana. Associado a esse dado, no que diz respeito às gestantes e a sua alimentação, houve também mais adesão às dietas vegetarianas promovendo a necessidade de analisar quanto à segurança materno e fetal. Objetivo: Elencar os possíveis impactos no organismo materno e fetal devido a adoção de uma dieta vegetariana durante o período gravídico. Metodologia: Trata-se de uma revisão de literatura qualitativa realizada por meio de buscas nas bases de dados Google Scholar, BVS e PubMed, com seleção de 10 artigos. Para isso, foi utilizado os seguintes descritores: "dieta vegetariana", "feto", "gravidez" e "relações materno-fetais", sendo que a correlação destes foi feita a partir do operador booleano AND. Resultados: As dietas vegetarianas são o consumo alimentar, de forma majoritária de origem vegetal, excluindo carne e pescado. Em estudos recentes, concluiu-se que mesmo diante das disparidades na literatura sobre a temática, a adoção da alimentação vegetariana aparentemente é segura. Durante o período gravídico, uma dieta rica em plantas pode ser fator protetor contra patologias maternas como pré-eclâmpsia, obesidade e diminui a exposição a agentes genotóxicos, além de reduzir a probabilidade do desenvolvimento posterior de doenças pediátricas, como diabetes, defeitos de tubo neural, fendas orofaciais e alguns tumores pediátricos. Porém também existem possíveis desvantagens dessa dieta para gestantes, algumas deficiências nutricionais, de ferro, vitamina B12 e zinco. Conclusão: Em razão disso, é evidente que a adoção da dieta vegetariana no período gravídico tem repercussões materno-fetal positivas e negativas. Dessa forma, a literatura afirma que gestantes podem seguir uma dieta vegetariana sem oferecer malefícios à própria saúde ou à saúde do feto, porém é importante salientar que é necessário a suplementação correta dos principais nutrientes.
Introdução: Complicações obstétricas, como parto prematuro, pré-eclâmpsia e aborto recorrente de primeiro trimestre, essas são algumas das manifestações da Síndrome Antifosfolipídeo (SAF). A Síndrome Antifosfolipídeo é uma condição pró-trombótica e inflamatória combinada pela presença de pelo menos um dos seguintes antifosfolípides (AFL): anticoagulante lúpico (LAC), anticardiolipina (ACA) ou anti-β2glicoproteína I (aβ2GP1). Essa condição utiliza anticorpos antifosfolípides como marcadores diagnósticos, o LAC positivo, IgG ou IgM ACA em título médio ou alto; IgG ou IgM aβ2GP1 em título médio ou alto como critérios laboratoriais. Importante salientar que pelo menos um dos critérios laboratoriais deve estar presente no mínimo em duas ocasiões, com pelo menos 12 semanas de intervalo. Objetivo: Correlacionar o mecanismo de ação da Síndrome de anticorpos antifosfolipídios com as repercussões no período gravídico. Material e métodos: Trata-se de uma revisão de literatura qualitativa realizada por meio de buscas nas bases de dados Google Scholar e BVS com seleção de 6 publicações. Resultados: Aproximadamente 20% das gestações em pacientes portadoras de Síndrome Antifosfolipídica apresentam complicações que estão relacionadas, principalmente, a alterações placentárias. Dentre os anticorpos antifosfolípides, o LAC revelou associação mais significativa com perdas fetais recorrentes antes da 24ª semana de gestação; a tripla positividade para LAC, ACA e aβ2GP1 também relacionou-se à maior incidência de eventos adversos na gravidez. Frente a esse contexto, os fatores angiogênicos circulantes, como tirosina quinase fms solúvel tipo 1 (sFlt-1), fator de crescimento placentário (PIGF) e endoglina solúvel, medidos durante o início gestação, demonstraram um elevado valor preditivo negativo para descartar o desenvolvimento de complicações gestacionais. Conclusão: Portanto a síndrome antifosfolípideo tem inúmeras repercussões na gravidez, que incluem: aborto prévio, parto precoce, trombose, morte fetal, aborto recorrente e pré-eclâmpsia, nesse sentido as pacientes portadoras de SAF devem ser diagnosticadas precocemente para que o tratamento adequado com aspirina e heparina (na maioria dos casos heparina de baixo peso molecular) seja instituído e resultados favoráveis na gravidez sejam atingidos, como: viabilidade fetal e profilaxia de trombose.
Apesar dos avanços dos tratamentos das doenças cardíacas, a cirurgia cardíaca (CC) ainda se faz indispensável no tratamento de certas patologias. Nesse caso, o diabetes mellitus (DM) pode estar associado a graves complicações perioperatórias. O objetivo foi identificar e avaliar os riscos e eventos no período perioperatório de pacientes com DM submetidos à CC. A busca foi feita nos bancos de dados do PubMed, Cochrane Library e Biblioteca Virtual em Saúde. Foram incluídos estudos originais de natureza qualitativa ou quantitativa disponíveis na Internet que foram publicados entre 2015 e 2021. Foram excluídos estudos duplicados, dissertações, teses, livros e aqueles que não se adequaram à pergunta de pesquisa. Como parte dos resultados, cerca de 58% dos estudos selecionados avaliaram o pré-operatório, 8%, o intraoperatório e 83%, o pós-operatório. As comorbidades pré-operatórias mais comuns foram hipertensão arterial sistêmica e doença renal. Acerca dos eventos pós-operatórios, observaram-se prolongamento da hospitalização, infecções, menor sobrevida, hiperglicemia e lesão renal aguda. Os achados deste estudo corroboram a hipótese da associação do DM como risco independente para complicações em CC.
