Define-se gastroparesia como uma síndrome associada a retardo patológico do esvaziamento gástrico, sem obstrução mecânica do tubo digestivo. As gastroparesias decorrem de anormalidades da função motora gástrica secundárias a distúrbios neurais, musculares, da rede de células intersticiais de Cajal, eletrolíticos e hormonais 1. Este capítulo fará uma revisão sobre vários aspectos relevantes das gastroparesias, destacando-se aspectos peculiares relativos a esta condição clínica em população idosa. EFEITOS DO ENVELHECIMENTO SOBRE O ESVAZIAMENTO GÁSTRICO Considerando-se suas funções motoras, a região gastroduodenal pode ser dividida em 4 segmentos, que apresentam padrões motores distintos: região proximal do estômago, região distal do estômago, piloro e duodeno. A ação conjunta destes segmentos permite que ocorra um esvaziamento gástrico adequado e ordenado. O esvaziamento gástrico em ritmo adequado depende da acomodação à distensão da porção proximal do estômago, das contrações antrais, da resistência pilórica e da resistência oferecida pela atividade motora duodenal. Para tanto, complexos mecanismos que atuam por meio de sinais neurais e hormonais advindos da interação entre nutrientes com a mucosa intestinal controlam o esvaziamento gástrico.
