Resumo ObjetivoDescrever o perfil de utilização de medicamentos por idosos residentes em áreas de diferentes níveis socioeconômicos de um centro urbano do Nordeste do Brasil. Métodos Inquérito domiciliar entre idosos (60 anos ou mais) da cidade de Fortaleza, Ceará, selecionados por amostragem sistemática em múltiplos estágios e estratificada por nível socioeconômico (melhor = área central; intermediário = área intermediária; pior = área periférica). Estatística descritiva foi apresentada pelas percentagens dos respectivos totais para variáveis categóricas e médias (± desvio-padrão) para variável contínua. Realizou-se análise multivariada para identificação de fatores associados a uso de medicamentos prescritos; não-prescritos e inadequados. Resultados A maioria dos idosos (80,3% na área central) usava pelo menos um medicamento prescrito. Mais de um terço (37,4%) na área periférica usava pelo menos um não-prescrito, e quase 20% pelo menos um inadequado. O uso de medicamentos prescritos foi associado à idade avançada (razão de chances -RR=1,7; IC 95%: 1,1-2,8); sexo masculino (RR=0,5; IC 95%: 0,3-0,7); visitas a serviços de saúde (RR=2,5; IC 95%: 1,9-3,1); doenças crônicas (RR=4,0; IC 95%: 2,5-6,2); e nível socioeconômico (RR=2,0; IC 95%: 1,5-2,6). O uso de medicamentos não prescritos foi associado a comprometimento funcional (RR=1,5; IC 95%: 1,1-2,2) e nível socioeconômico (RR=0,6; IC 95%: 0,5-0,8). O uso de medicamentos inadequados foi associado principalmente a sexo masculino (RR=0,4; IC 95%: 0,2-0,8); doenças crônicas (RR=2,0; IC 95%: 1,2-3,3), e nível socioeconômico (RR=0,7; IC 95%: 0,5-0,9). Conclusões As proporções de idosos usando medicamentos prescritos, não-prescritos e inadequados foram significativas, havendo desigualdades particularmente entre aqueles de diferentes níveis socioeconômicos. Os resultados apontam para a necessidade de programas para otimizar o acesso e racionalizar o uso de medicamentos entre idosos no Brasil. Abstract ObjectiveTo identify medication use patterns among elderly people residing in areas with different socioeconomic status in the city of Fortaleza, Brazil.
OBJETIVO: Realizar revisão integrativa de literatura sobre epidemiologia de quedas entre idosos no Brasil, buscando identificar taxa de ocorrência, recorrência e fatores potencialmente modificávies associados a esses episódios. MÉTODO: Foi efetuada revisão de literatura, consistindo na busca de artigos científicos das bases de dados Literatura Internacional em Ciências da Saúde (MEDLINE), Scientific Electronic Library Online (SciELO) e Centro Latino-Americano e do Caribe de Informação em Ciências da Saúde (LILACS) em 14 de novembro de 2017. Os artigos foram selecionados a partir das seguintes palavraschave: "Acidentes por quedas" E/OU "Idoso" E/OU "Brasil". Dados epidemiológicos foram extraídos dos estudos primários e comparados. RESULTADOS: Foram selecionados 35 artigos. A maioria dos estudos foi realizada nas regiões Sudeste (15) e Sul (11). A taxa de ocorrência de quedas variou entre 10,7 e 59,3%. O domicílio, no período diurno, é o cenário mais frequente de quedas. As circunstâncias mais comumente descritas são tropeço, escorregão, tontura e existência de desnível, ocasionando tombo da própria altura. Os fatores mais frequentemente associados às quedas foram sexo feminino, idade maior que 80 anos, déficit cognitivo e sintomas depressivos. As consequências identificadas foram fraturas e o medo de cair novamente. CONCLUSÃO: Alguns fatores associados aos tombos em idosos no Brasil são modificáveis e prevenir as quedas pode possibilitar a redução da morbimortalidade nessa população.

OBJETIVO: A prevalência da infecção pelo vírus da hepatite C (VHC) é maior em pacientes submetidos à hemodiálise que na população em geral, possivelmente refletindo maior exposição a situações de risco. O estudo realizado teve por objetivo determinar a prevalência de anticorpos anti-VHC em pacientes em hemodiálise e identificar fatores relacionados às clínicas e aos pacientes, associados à transmissão do VHC. MÉTODOS: Realizou-se estudo transversal em 752 pacientes em hemodiálise em todas as 12 clínicas de Fortaleza, Ceará. Eles foram testados com anticorpos anti-VHC por ELISA de terceira geração. Foram entrevistados 663 pacientes quanto a aspectos sociodemográficos, clínicos e epidemiológicos. Fatores nosocomiais foram investigados utilizando-se questionário específico. Para a análise estatística foram aplicados o teste t de Student, o odds ratio e a regressão logística univariada e multivariada. RESULTADOS: A prevalência de anti-VHC foi de 52% (390/746; variando de 6% a 72%). A freqüência de infecção foi maior em pacientes previamente submetidos à diálise peritoneal (OR=1,76; IC 95% 1,12-2,76) e a transfusões de sangue (OR=2,75; IC 95% 1,25-6,03). O tempo de hemodiálise associou-se com positividade do anti-VHC (OR=1,47; IC 95% 1,35-1,61). Procedimentos nas clínicas associados à positividade do anti-VHC foram preparo prévio de heparina (OR=2,92; IC 95% 1,23-6,92), falha no uso ou troca de luvas (OR=5,73; IC 95% 1,75-18,72), desinfecção de máquinas de diálise (OR=2,79; IC 95% 1,57-4,96) e segregação em sala de diálise (OR=0,18; IC 95% 0,05-0,61). CONCLUSÕES: Os resultados mostraram elevada prevalência de anti-VHC em pacientes sob hemodiálise e a associação de fatores de transmissão nosocomial com a infecção pelo VHC.

Métodos: Estudo transversal, envolvendo uma amostra de 374 idosos com idade maior ou igual a 60 anos, realizado entre os meses de outubro de 2015 e abril de 2016, nas academias ao ar livre para a terceira idade em Fortaleza, Ceará. Aplicou-se um questionário contendo questões sobre dados sociodemográficos, condições de saúde e hábitos de vida autorrelatados, e sobre a utilização dos equipamentos. Mensurou-se o peso e a altura para a obtenção do índice de massa corporal (IMC). Resultados: A maioria dos entrevistados era mulher (56,7%, n=212) e com faixa etária entre 60 e 69 anos (66,8%, n=250). As principais condições de saúde autorrelatadas foram hipertensão (40,9%, n=153), colesterol alto (32,4%, n=121) e osteoartrite (26,2%, n=98). Entre os hábitos de vida, o consumo de bebida alcoólica (31,6%, n=118) foi o mais relatado. Verificou-se que 42,8% (n=160) encontrava-se com excesso de peso. A maioria utilizava os equipamentos há mais de um ano (68,2%, n=255), até 30 minutos por dia (49,5%, n=185), cinco ou mais dias durante a semana (51%, n=191), tendo em grande parte percebido melhora na saúde (93,6%, n=350). Conclusão: Os idosos apresentaram um perfil jovem e ativo, cujas condições de saúde de maior prevalência podem ser facilmente manejadas com a prática de atividade física, e apresentam características semelhantes independentemente da situação econômica em que se encontram inseridos.
