The evolution of medical knowledge and technological growth have contributed to the development of different techniques and devices for airway management. These appear to play a role in optimizing the number of attempts and overall success, ultimately reducing the negative consequences of airway manipulation. In this literature review, we highlight the recent evidence regarding new technologies applied to airway management. Before intubation, every patient should have an individualized structured airway management plan. Technology can help with both airway evaluation and tracheal intubation. Point-of-care cervical ultrasound and artificial intelligence models with automated facial analysis have been used to predict difficult airways. Various devices can be used in airway management. This includes a robotic video endoscope that guides intubation based on real image recognition, a laryngeal mask with a non-inflatable cuff that tries to reduce local complications, video laryngeal masks that are able to confirm the correct position and facilitate intubation, Viescope™, a videolaryngoscope developed for combat medicine with a unique circular blade, a system that uses cervical transillumination for glottis identification in difficult airways and Vivasight SL™ tracheal tube, which has a high-resolution camera at its tip guaranteeing visual assurance of tube position as well as guiding bronchial blocker position. To conclude, we detailed the challenges in airway management outside the operating room as well as described suction-assisted laryngoscopy and airway decontamination technique for contaminated airways. Further research in the clinical setting is recommended to better support the use of these technologies
O presente estudo apresenta uma revisão sistemática da literatura sobre o business intelligence (BI) na gestão do setor público, tendo como principal objetivo identificar as aplicações e contribuições no contexto de abordagem. A pesquisa foi realizada na base de dados Scopus e analisou 14 artigos selecionados com base na aderência ao proposto. O estudo identifica que, mesmo diante do expressivo aumento na taxa de implementação de BI no setor público, as falhas na sua implementação se revelam igualmente significativas. As fases de definição, avaliação, qualificação e seleção se revelam mecanismos para superar os desafios na implementação do BI e gerar maior sucesso. Geração, coleta, análise e a reutilização sistemática dos dados podem otimizar os recursos para atender as demandas sociais em crescimento constante, melhorando os processos e a implementação de novos produtos e serviços. O BI se apresenta como uma ferramenta estratégica para as organizações públicas, que buscam visualizar e acompanhar processos, e simular cenários futuros para a tomada de decisões mais assertivas. Como recomendação de trabalhos futuros, o estudo orienta investigações empíricas sobre as possibilidades de aplicações do BI, em diferentes contextos do setor público brasileiro, a fim de ampliar as discussões acadêmicas e apresentar os seus benefícios aos gestores públicos e à sociedade.
Objective: The purpose of this study was to assess the impact of arterial hypertension (AH) and dyslipidemia on the severity, cardiovascular events and death in patients hospitalized for pneumococcal community acquired pneumonia (CAP). Design and method: Retrospective, cohort, multicenter, epidemiologic study. All adult inpatients of 4 different centres in Portugal, with microbiological and clinical documented pneumococcal CAP were included. Demographic data, comorbidities, antibiotic therapy, hospital length of stay, need for ICU admission, invasive mechanical ventilation (IMV) and renal replacement therapy (RRT) were collected. All patients were monitored until death or 1-year of follow up. Risk factors for short- and long-term all-cause mortality, including cardiovascular events, were computed. Patients were divided into 3 groups: H) with isolated AH; HD) with both AH and dyslipidemia; Control) without AH or dyslipidemia. The significance level was defined as p <0.05. Results: 797 patients were included in this study, 53.6% male, mean age 72.4±16.5 years. Comparing the 3 groups, females were prevalent in H and HD groups (52% and 54.3% vs 40.6% Control; p<0.002). Mean age was higher in H and HD (78.2±12.6 and 78.4±10.7 years; vs Control 67.5±18.2 years, ANOVA p<0.001). 18.8% patients required ICU admission, 6.7% IMV and 13.1% RRT. H required more IMV (7.5%, p = 0,5) and HD more RRT (16.6%, p = 0.2). The overall in-hospital mortality was 17.8%. During the follow up, H and HD groups were more readmitted to the emergency department due to cardiovascular events than Control group (11.7% and 13.4% vs. 6.5%, OR 1,91 p = 0.04 and OR 2.26 p = 0.016, respectively); also required more hospitalization for cardiovascular events than Control group (14.4% e 12.6% vs. 6.2%; OR 2.49, p = 0.002; and OR 1.99, p = 0.02, respectively). One-year all-cause mortality was higher in H group (39.2%; p = 0.026). Conclusions: Prevalence of arterial hypertension was 47.4%, higher than the national rate. H group resorted, with or without dyslipidemia, more often to the emergency department, requiring more hospitalization and with higher mortality rates, without difference in hospital mortality. It's crucial to control cardiovascular risk factors, providing greater cardiovascular protection to reduce the risk of long-term mortality.
O interesse pela inovação, seus processos e gerenciamento tem avançado bastante, à medida que os mercados se tornam mais dinâmicos. Isso tem demandado das organizações a necessidade de se tornarem inovadoras para o melhor aproveitamento das oportunidades oferecidas pela tecnologia, nas mudanças nos mercados, estruturas e dinâmicas. O objetivo deste artigo é identificar e difundir como se deu a introdução da inovação aberta IAb, as principais evoluções do processo de IAb, suas características na atualidade na visão de Chesbrough, considerado o pai da IAb. Vários documentos de revisão de literatura da base científica Scopus compõem este estudo, para compreensão do processo evolutivo do conceito da IAb aplicado nas organizações. Como resultado, o artigo apresenta os tipos de IAb, seus benefícios, destacando também os desafios a serem superados, bem como sua perspectiva de futuro da IAb para ser integrada em diferentes propósitos das atividades organizacionais. O método adotado foi de pesquisa qualitativa, com abordagem exploratória e foram encontrados 50 artigos do autor especificado e seus respectivos coautores, utilizando metodologia teórico‑conceitual.
A denominada Lei de Inovação, vigente desde 2004, prevê a possibilidade dos Núcleos de Inovação Tecnológica (NITs) adotarem uma personalidade jurídica própria, como forma de potencializar o desenvolvimento de suas atividades e finalidades. Entretanto, existem diversos tipos de natureza jurídica de direito privado sem fins lucrativos existentes no ordenamento jurídico brasileiro que poderiam ser adotados. Este artigo tem por objetivo identificar e descrever os tipos de personalidade jurídica que melhor atendem aos propósitos dos NITs, apontando suas vantagens e benefícios. Dentre as personalidades jurídicas avaliadas, as Associações e as Fundações de Apoio, qualificadas como Organizações Sociais (OS), proporcionariam o melhor atendimento às finalidades legais pré-estabelecidas para o funcionamento dos NITs, como por exemplo uma maior autonomia financeira e administrativa, além de uma maior agilidade no relacionamento, na cooperação e na transferência do conhecimento gerado nas ICTs com os demais entes privados e públicos.
