Objectives: The aim of the study was to assess the level of implementation of recommendations of the Ministry of Health regarding infectious disease screening during pregnancy. Material and methods:The study included 477 patients who were admitted to the delivery room between December 2015 and February 2016. Data on screening test results were collected based on medical records covering the period before the admission. Results:The Human Immunodeficiency Virus (HIV) screening was conducted in 410 (86%). 460 (96%) of patients were screened for Hepatitis B Virus (HBV) and 427 (89.5%) for Hepatitis C Virus (HCV). Syphilis screening covered 465 (97.5%) of patients. Immunoglobulin M (IgM) titer against Rubella Virus (RV) was assessed in 218 (45%) patients and immunoglobulin G (IgG) in 319 (66.9%). Screening for Toxoplasma gondii based on assessment of IgM titer was conducted in 440 (92%) patients while IgG titter was assessed in 413 (86.6%). 343 (71.9%) patients had obtained vaginal swabs for Group B Streptococci (GBS) while the anal swabs were taken only from 268 (56.2%) patients. Conclusions:Coverage of screening for syphilis and HBV was similar to the countries with highest prevalence of conducting such screening, on the other hand RV screening place as among countries with lowest prevalence. There is an increasing trend in conducting HIV screening. Screening for HCV and toxoplasmosis is at satisfactory level and Poland is one of a few European countries offering such screening. The screening for GBS is insufficient which result in excessive use of intrapartum antibiotic prophylaxis.
Objectives: Dental agenesis -a congenital lack of teeth -is one of the most frequently diagnosed developmental defects of dentition. Genetics is a crucial factor in the etiology of this disorder. Missing teeth can be caused by mutation in genes including MSX1, PAX9, AXIN2, and EDARADD. As is also true for ovarian cancer, over 20% of cases are associated with hereditary factors. Mutations in the BRCA1 and BRCA2 genes are said to be the most frequent of these. The aim of this study was to provide a systematic review of the literature on the coexistence of ovarian cancer and tooth agenesis. Material and methods:Publications were searched for in the online databases PubMed, SCOPUS, and Wiley Online Library. Current and archival issues of the Journal of Stomatology and Dental and Medical Problems were also searched. The key words used to find relevant publications were: ovarian cancer, hypodontia, and tooth agenesis, in various combinations.Results: Three publications were qualified to this review. Two of these compared the incidence of hypodontia in women with ovarian cancer and in healthy women, and the other was aimed at locating the gene responsible for the coexistence of ovarian cancer and tooth agenesis. As shown by these studies, women with ovarian cancer are (depending on the study) 3.3 or 8.1 times more likely to have hypodontia than healthy women. However, no specific gene was found that might be responsible for the coexistence of ovarian cancer and tooth agenesis.
Ocena roli badania pozytonowej tomografii emisyjnej z tomografią komputerową (PET/TK) w monitorowaniu chorych po zakończonym leczeniu pierwotnym raka jajnika oraz jego przydatności w planowaniu wtórnego leczenia u chorych z nawrotem choroby. Próba ustalenia progowych standardowych wartości gromadzenia radioizotopu w patologicznie zmienionej tkance (SUV), charakterystycznych dla aktywnego procesu nowotworowego, pomocnych w decyzji o wdrożeniu dalszej terapii. Materiał i metody: Do badania włączono 140 pacjentek (średni wiek 57 lat) leczonych w latach 2010-2014 z powodu wznowy raka jajnika w dwóch ośrodkach ginekologii onkologicznej w Polsce. Za kryterium przyjęto leczenie oparte na standardowym protokole leczenia raka jajnika, tj. pierwotna wielonarządowa cytoredukcja, chemioterapia uzupełniająca oraz możliwość ustalenia pełnych danych klinicznych. W trakcie obserwacji po leczeniu w grupie badanej przeprowadzono badanie ginekologiczne, ultrasonograficzne przezpochwowe i przezbrzuszne, a także oznaczono marker CA-125 oraz wykonano obrazowanie PET/TK z 18-fluorodeoksyglukozą. W grupie kontrolnej nie wykonywano PET/TK. Wyniki: W grupie 34 chorych z SUV co najmniej 2,4 mikroskopową wznowę raka jajnika w rejonie węzłów chłonnych miedniczych stwierdzono u 29 osób, natomiast wśród 51 pacjentek z SUV poniżej 2,4 -w 6 przypadkach. Wartość SUV co najmniej 3,5 dla węzłów chłonnych okołoaortalnych była związana z wystąpieniem wznowy w tym obszarze w 24 na 25 przypadków. Natomiast SUV poniżej 3,5 w grupie 60 chorych potwierdził wznowę tylko w 1 przypadku. Wnioski: Badanie PET/TK charakteryzuje się wysoką skutecznością w wykrywaniu wznowy po leczeniu pierwotnym. W badanej grupie chorych wykonywanie PET/TK w ramach monitorowania pacjentek po standardowym leczeniu raka jajnika wiązało się z wydłużeniem czasu przeżycia po zastosowaniu leczenia wtórnego w stosunku do kobiet, u których w kontroli nie wykonywano PET/TK. Wartość graniczna SUV, określająca aktywny proces nowotworowy w lokalizacji okołoaortalnej, wynosiła 3,5, a dla węzłów chłonnych miedniczych -2,4.
Analiza wskazań do zabiegu wytrzewienia oraz powikłań śród-i pooperacyjnych, na podstawie przypadków z jednego ośrodka ginekologii onkologicznej w ostatnich 5 latach. Materiał i metody: Szczegółowy przegląd dokumentacji medycznej pacjentek, u których wykonano zabieg wytrzewienia miednicy mniejszej w ciągu ostatnich 5 lat (od roku 2014 do 2018). W przeglądzie nie uwzględniono wytrzewień z powodu raka jajnika. Analizie poddano wskazania do zabiegu, wiek operowanych, lokalizację i typ histologiczny guza, przebyte wcześniej leczenie, stan sprawności i współchorobowość, cel i rodzaj zabiegu, czas trwania zabiegu, powikłania wczesne i późne wg klasyfikacji Claviena-Dindo, sposób odprowadzenia moczu, osiągnięte marginesy operacyjne. Wyniki: W okresie od początku 2014 do pierwszej połowy 2018 roku wykonano 8 zabiegów wytrzewienia miednicy mniejszej -5 z intencją wyleczenia, 3 paliatywnie. Połowę chorych stanowiły przypadki wznowy raka sromu. Średni czas trwania zabiegu wyniósł 315 minut, średni czas pobytu w oddziale -24,38 dnia. Wczesne powikłania pooperacyjne o różnym stopniu ciężkości wystąpiły w każdym operowanym przypadku, w tym powikłania ciężkie (IIIb-V wg Claviena-Dindo) u 5 kobiet (62,5%). Nie odnotowano zgonu we wczesnym okresie pooperacyjnym. Powikłania późne zaobserwowano łącznie u 6 pacjentek (75,0%), w tym jeden zgon 11 miesięcy po egzenteracji paliatywnej. Wnioski: Zabieg wytrzewienia miednicy mniejszej pomimo postępu w opiece okołooperacyjnej łączy się z wysokim ryzykiem powikłań, często zagrażających życiu. Każdy przypadek kwalifikujący się do tak radykalnego zabiegu operacyjnego należy traktować indywidualnie, a sama procedura powinna zostać przeprowadzona w ośrodku dysponującym odpowiednią kadrą i sprzętem medycznym.Słowa kluczowe: wytrzewienie miednicy mniejszej, wznowa nowotworu ginekologicznego, powikłania, odprowadzenie moczu Aim: Analysis of indications for the procedure of exenteration, and intra-and postoperative complications, based on cases from a single gynecologic oncology center over the past 5 years. Material and methods: Detailed review of medical records of female patients who underwent pelvic exenteration surgery over the last 5 years (2014)(2015)(2016)(2017)(2018). The review excluded cases of exenteration for ovarian cancer. The analysis included indications for the procedure, age of operated patients, location and histological type of tumor, prior treatment history, performance status and comorbidities, purpose and type of procedure, duration of operation, early and late complications according to the Clavien-Dindo classification, method of urinary diversion, and achieved surgical margins. Results: A total of 8 pelvic exenteration procedures were performed between early 2014 and mid-2018, including 5 procedures with the intention to cure, and 3 palliative procedures.
