A través del tiempo, se han generado múltiples definiciones para estrés, pero aún no se logra un consenso satisfactorio; en general, es percibido como una amenaza que pone en riesgo la armonía interna. Esta condición genera como respuesta una serie de cambios fisiológicos (respuesta adaptativa) que buscan mantener el equilibrio; con la activación subyacente de ejes como el hipotálamo-hipófisis-suprarrenal, cuyas sustancias liberadas tienen impacto en varios sistemas, lo que explicaría el desarrollo de ciertas enfermedades y justifica el uso de biomarcadores para la medición objetiva del estrés. Los médicos residentes en formación se han identificado como una población en riesgo para presentar estrés, específicamente estrés laboral o burnout (en inglés), el cual es posible medir de manera subjetiva mediante la aplicación de cuestionarios como el Maslach Burnout Inventory Human Services Survey (MBI), y a través del cual se ha identificado alta prevalencia de la condición; sin embargo, aún no existe consenso respecto a las estrategias de medición objetiva. La presente revisión pretende explorar la literatura existente sobre los biomarcadores como herramientas de medición de estrés laboral en residentes.
Introducción: Los residentes de especialidades médico-quirúrgicas son una población en riesgo de desarrollar síndrome Burnout, lo que afecta negativamente su salud y en consecuencia, desencadena alteraciones en los niveles de citoquinas séricas que conllevan a un estado inflamatorio. Objetivo: Evaluar el estrés laboral a través del diagnóstico del síndrome de Burnout y los niveles de citoquinas pro y anti inflamatorias en residentes de primer año de la Universidad del Cauca. Materiales y Métodos: Estudio descriptivo longitudinal prospectivo que empleó encuestas autodiligenciadas para evaluar variables sociodemográficas, clínicas y de la residencia, el test de Zung y el Maslach Burnout Inventory. Se tomaron muestras de sangre venosa para medición de citoquinas mediante ELISA. Esto se realizó al inicio, a los seis meses y al año (tres tomas). Resultados: Participaron 14 médicos residentes. El 100% de la población presentó alteraciones en alguna de las dimensiones asociadas al síndrome de Burnout, predominando el agotamiento emocional. Al final, 14% de los residentes presentaron Burnout y se observó una disminución significativa en la producción TNF-α. Conclusión: Dicho síndrome es multifactorial y se requieren estímulos estresores crónicos y constantes que repercutan en la salud de los residentes, evidenciando una alteración de su estado inflamatorio.
Los tumores adenomatoides del tracto genital son neoplasias poco frecuentes, de comportamiento benigno; debido a sus características histológicas se confunden fácilmente con tumores de estirpe vascular. Estudios recientes demuestran que los tumores adenomatoides del tracto genital femenino están genéticamente definidos por la mutación TRAF7 que activa la expresión aberrante de la vía NFkB. Presentamos el caso de una mujer de 27 años, asintomática, a quien se le realiza salpingectomía bilateral como método de anticoncepción definitivo; en el examen macroscópico de la tuba derecha se encuentra formación quística que por sus características microscópicas es diagnosticada como tumor adenomatoide de la tuba uterina. Finalmente se realiza una revisión de la literatura.
