Methodologies have been developed to evaluate the heart rate (HR) chaotic dynamics, differentiating normality of chronic and acute disease and the evolution between these states; other dynamic variables of system on ICU have shown a chaotic behavior. An induction from 8 patients was realized, five discharged alive from ICU and 3 cases of mortality, defining sets and subsets, as well as operations between them that differentiate and predict the dynamics of patients who survive of those with fatal outcome, through the maximum and minimum values of chaotic attractors of 4 dynamic variables: Heart rate (HR), arterial carbon dioxide partial pressure (PACO2), venous carbon dioxide partial pressure (PvCO2), and oxygen venous saturation (SVO2). Finally a blind study was done with 12 patients, calculating sensitivity, specificity, PPV, NPV and accuracy. Also a temporal decrease in the cardiac dynamics evaluation was done, from 21 to 15 hours, through an induction with two cases, one with normal dynamic and one with acute dynamic. The occupation spaces of cardiac chaotic attractors were measured, and then a blind study was realized with 18 patients, calculating sensitivity, specificity and Kappa coefficient. The equation T = [(B2∩ C2) U A1 U D3] U [(A2∩ B1) U C3 U D1] differentiates and predicts the dynamics of patients who survive of those with fatal outcome in the ICU. The sensitivity, specificity, PPV and NPV were 100%, and accuracy was 1. Acute cardiac dynamic was differentiated of normal dynamic on 15 hours, allowing to reduce the time of the evaluation of this dynamic of 21 to 15 hours, the sensitivity and specificity values were 100% and Kappa coefficient was 1. Dynamical systems in the context of set algebra allow differentiate patients who are discharged alive from those with an adverse outcome. The evolution to death can be predicted in a physical and mathematical way in the ICU.
Esta investigación presenta una metodología diagnóstica de aplicación clínica fundamentada en la teoría de sistemas dinámicos, la probabilidad y la entropía. Se pretende que el lector obtenga todo el contexto físico y matemático, así como los conceptos básicos del área de cardiología. Se inicia con los fundamentos del pensamiento matemático y físico para llegar a nuevas creaciones, así como con conceptos básicos de cardiología, para luego presentar cómo la teoría del caos y los fractales se emplean con el propósito de estudiar el comportamiento altamente irregular de la dinámica cardiaca. Se presentan diversos métodos diagnósticos desarrollados desde la perspectiva físico-matemática, así como la ruta de pensamiento que permitió su creación, incluyendo un nuevo método de evaluación de reestenosis arterial y nuevos métodos de evaluación de la dinámica cardiaca del adulto desde diversas teorías, como, por ejemplo, la de los sistemas dinámicos o la ley de Zipf/Mandelbrot. Se presenta, además, una nueva ley exponencial de aplicabilidad clínica desarrollada para los atrac- tores cardiacos en el contexto de los sistemas dinámicos y la geometría fractal. También se describen los avances logrados en el diagnóstico de la dinámica cardiaca neonatal. Este recorrido traza la ruta que permite llegar a la descripción de la metodología diagnóstica fundamentada en la teoría de los sistemas dinámicos, la probabilidad y la entropía, así como la ruta de pensamiento que permitió llegar a ese descubrimiento y sus aplicaciones clínicas. Finalmente, se realiza un breve recorrido por otras metodologías diagnósticas y predictivas desarrolladas en otras áreas de la medicina desde el contexto físico-matemático.
Basados en geometría fractal se hizo un diagnóstico objetivo y reproducible de células de cuello uterino, que diferencia las normales de aquellas con lesión de bajo grado (LEIBG) o cancerígenas, identificando en forma cuantitativa las células ASCUS. Objetivo: establecer una metodología diagnóstica de las células cervicales normales y preneoplásicas con aplicación simultánea de geometría fractal y euclidiana para definir parámetros matemáticos distintivos de cada uno de dichos estados. Métodos: fotografías digitales de doce células de citologías de mujeres entre 20 y 55 años (tres normales superficiales, tres normales intermedias, tres LEIBG y tres ASCUS). Mediante un programa se calculó la dimensión fractal de tres objetos matemáticos: núcleo, citoplasma y totalidad, a partir del método box-counting; de manera simultánea se determinó el número de pixeles ocupados por la superficie de cada uno y los espacios ocupados por el borde de estos objetos en cada una de las cinco rejillas, para comparar los valores obtenidos. Resultados: al superponer las rejillas de dos y cuatro pixeles los valores de los espacios de ocupación del núcleo permiten establecer diferencias matemáticas entre los grupos de células, presentando como valores en la rejilla dos: normales superficiales (53-56), normales intermedias (75), LEIBG (120-159) y ASCUS (104-121). Conclusiones: se estableció una metodología matemática diagnóstica que diferencia estados preneoplásicos con base en medidas fractales y euclidianas simultáneas del borde del núcleo celular. Desde la teoría de los sistemas dinámicos se ha diagnosticado la dinámica cardiaca, con aplicabilidad clínica. En este trabajo se desarrolló desde la teoría de los sistemas dinámicos, una metodología de evaluación de la presión arterial de dióxido de carbono para pacientes en Unidad de Cuidados Intensivos. Para el estudio se seleccionaron 10 pacientes a los cuales se les midió mediante la toma de gases, la presión arterial de dióxido de carbono durante sus tiempos de estancia en la Unidad de Cuidados Intens Postquirúrgicos del Hospital Militar Central. A partir estos valores se construyeron atractores caóticos representativos de esta variable en un mapa de retardo. Finalmente se cuantificaron los valores mínimos y máximos del atractor. Se confirmó un comportamiento caótico de la dinámica; los valores máximos y mínimos de los atractores de la presión arterial de dióxido de carbono oscilaron entre 14,40 mmHg y 76,50 mmHg. El comportamiento caótico de la presión arterial de dióxido de carbono es cuantificabl evaluación de los valores máximos y mínimos de atractor en el mapa de retardo, constituyéndose en un nuevo método de evaluación matemática, objetiva y reproducible, útil para la evaluación de pacientes en Unidad de Cuidados Intensivos.
