Valproate is the best choice drug for a variety of medical conditions. As with any other drug, it has adverse effects, and it is important to emphasize the possibility of those adverse effects to prevent complications. We present the case history of a 44-year-old male with valproate-induced hyperammonemic encephalopathy, despite having normal liver function tests. This case includes a detailed literature review of this rare adverse event. In the light of this case report, we illustrate the importance of checking ammonia levels in all psychiatric patients receiving valproate as a treatment who present with new onset neurological symptoms or altered mental status.
Compulsive sexual behavior (CSB) or sexual addiction is a term that generally indicates excessive and uncontrolled sexual behavior. This may lead to subjective distress, social and occupational impairment, or legal and financial consequences. Often, this condition is underreported and untreated. Until now there are no FDA-approved medications for sexual addiction or compulsive sexual behaviors. However, the therapeutic benefits of selective serotonin reuptake inhibitors (SSRIs) and naltrexone are known. This is a case of a 53-year-old male with a history of extensive alcohol use, alcohol withdrawal seizure, and delirium tremens. The patient was treated with naltrexone 50 mg/day for alcohol use disorder. The patient reported that his “sexual compulsion” also reduced after the medication and there was an improvement in both alcohol addiction and self-reported compulsive sexual behavior. This case report also includes a literature review of pharmacotherapy, especially naltrexone, for the treatment of sexual addiction/compulsive sexual behavior. The literature review has shown that patients’ symptoms were improved in different doses without side effects, and based on this and our experience, it can be said that naltrexone is effective in the reduction and remission of the symptoms of CSB or sexual addiction.
This paper focuses on studying various ethical and moral theories, which influences the person's decision on euthanasia. It has also studied the stance of religion on euthanasia. With the help of a lot of research, this paper aims to find out the views and reason of different theorist on euthanasia; it also aims to find out reasons of theorist for declaring euthanasia morally justified or not. This paper further aims to give both pro euthanasia theories and their viewpoints as well against euthanasia theorist making easier to analyze the worth of judgment. This paper also aims to find the relation between religion and morality while performing an action and its effect on people situation, through this paper will highlight the situations in which euthanasia is morally justified or not.
Alzheimer's disease (AD) is the leading cause of dementia in the USA and worldwide. In this article, we have discussed the incidence of dementia, especially Alzheimer's disease dementia, and its familial association, according to the latest reports. There is a menu of instruments discovered in past decades for the behavioral, cognitive and functional assessment of an AD person. Behave-AD, E-BEHAVE-AD, E-BEHAVE-AD FW, Neuropsychiatric inventory are discussed in detail, in view of the researches conducted in their context. Global deterioration scale (GDS), Brief cognitive rating scale (BCRS), Mini-mental state exam (MMSE), Functioning and self-care (FAST) and other tools are used in the cognitive and functional assessment of an AD person. We threw light on the pros and cons of utilization of these divergent tools.
This case is of a patient 24 years old male of Haitian-American origin, speaks English who is diagnosed with schizophrenia and cannabis use disorder. The patient had a premature delivery and the mother was not on any sort of medication during pregnancy. The first outbreak occurred in 2010 and was diagnosed with schizophrenia then. The patient is a drug addict and upon MRI cerebral leukomalacia was observed. There was no family history of such diseases and the symptoms of the patient were extremely self-damaging.
