ZusammenfassungAllergische Typ-IV-Reaktionen in Form einer allergischen Kontaktdermatitis auf Lokalanästhetika vom Amid-Typ wie Lidocain, Bupivacain, Mepivacain und Prilocain sind selten. Wir berichten über eine Patientin mit einer Typ-IV-allergischen Reaktion auf Prilocain nach subkutaner Infiltration von Tumeszenz-Lokalanästhesie auf Prilocain-Basis im Rahmen einer Varizen-Operation, die zu einem allergischen Kontaktekzem mit Blasenbildung führte. Wir behandelten mit systemischen und lokalen Steroiden. Im Verlauf erfolgte eine allergologische Diagnostik mittels Epikutantestung. Die Ergebnisse zeigten sich positiv für Prilocain ebenso wie für Lidocain und Articain und negativ für die restlichen getesteten Lokalanästhetika. Eine später durchgeführte subkutane Provokation präsentierte eine gute Verträglichkeit von Procain und Mepivacain als Ausweichpräparate.
After acute treatment of deep vein thrombosis, not only the risk but also associated side effects of postthrombotic syndrome (PTS) are often underestimated.There are essentially two main types of PTS.1. Obstructive type—no sufficient recanalization of the deep vein.2. Refluctive type—sufficient recanalization of the deep vein, but insufficient venous valves in conjunction with venous reflux.A statement regarding deep vein recanalization and venous valve function can be made at the earliest after 6 months.PTS is often diagnosed without appropriate medical history. However, the assessment of the degree of recanalization and venous reflux is paramount to the medical prognosis. In our opinion, beside proximal thrombosis, sufficient recanalization combined with a strong venous reflux, especially in the popliteal vein, works as a powerful predictor for an unfavorable and fast progression of PTS and chronic venous insufficiency. Thus, the obstructive type is prognostically more favorable. For PTS in general, consistent compression therapy represents the first-line treatment option.With concomitant varicosis, one should assess whether the varicose veins represent primary varicosis with reflux or secondary varicosis without reflux. Especially in the presence of venous ulcers, the elimination of concomitant primary varicosis leads to an improved prognosis. Moist wound treatment is considered to be the standard treatment for all wounds undergoing secondary healing. A standardized set of topical therapeutic agents also facilitates the treatment. In individual cases “ulcershaving” and mesh graft transplantation proved to be successful.
Der Morbus Mondor bezeichnet ursprünglich eine oberflächliche Venenthrombose einer gesunden Vene entlang der anterolateralen thorakalen Brustwand. Mittlerweile werden auch oberflächliche Venenthrombosen anderer spezifischer Lokalisationen, beispielsweise am Penis, darunter subsumiert. Im folgenden Fallbericht stellen wir einen jungen Patienten mit einem nach einer beidseitigen inguinalen Crossektomie aufgetretenen penilen Morbus Mondor vor. Der Klinische FallWir berichten über einen sonst gesunden 48-jährigen Patienten, der sich 3 Wochen nach beidseitiger inguinaler Crossektomie in unserer Klinik vorstellte. Er berichtete von Schmerzen und einem Druckgefühl im Penis sowohl im unerigierten als auch im erigierten Zustand. Die Eigen-und die Familienanamnese in Bezug auf thromboembolische Erkrankungen wurden verneint. Das präoperative Blutbild sowie die Gerinnung zeigten keine Auffälligkeiten. Körperliche UntersuchungKlinisch zeigte sich eine tastbare Verhärtung ohne umgebende Rötung im Verlauf der Vena dorsalis penis superficialis und einer Vene am lateralen rechten Hoden (▶ Abb. 1 und ▶ Abb. 2).▶ Abb. 1 Klinischer Befund des penilen Morbus Mondor. ▶ Abb. 2 Klinischer Befund der V. dorsalis penis superficialis.
ZusammenfassungPostoperative Serome können im chirurgischen Alltag immer wieder eine Herausforderung darstellen. Die Therapiemöglichkeiten sind vielfältig. Oftmals erfolgt zunächst die sterile Punktion mit anschließender Aspiration der Seromflüssigkeit und Anlage eines Druckverbandes. Eine alternative Therapiemöglichkeit bietet der zusätzliche Einsatz von Polidocanol oder Antibiotika, wie z. B. Erythromycin oder Doxycyclin. Bei besonders hartnäckigen, punktionsresistenten Seromen sollte eine perkutane Durchstichdrainage oder eine offene Revision als Therapiealternative in Betracht gezogen werden.
