Die Beurteilung bestimmter therapeutischer Mafinahmen und ihrer Erfolge ist im Bereich der Gesamtmedizin an die katamnestische Erhebung gebunden. Fur die Andrologie bedeutet das, dafi in regelmafiigen Abstanden Untersuchungen daruber angestellt werden, inwieweit bei der Therapie von Fertilitatsstorungen des Mannes im Zusammenhang mit der Hormonbehandlung bzw. aufgrund einer Beratung der Eheleute bzw. allein aufgrund der andrologischen Untersuchungen (vgl. SCHIRREN und BUNGE) ohne weitere therapeutische Mafinahmen eine Graviditat bei der Ehefrau mit nachfolgender Geburt eines gesunden Kindes eingetreten ist.In Fortfuhrung einer fruheren Studie (SCHIRREN und BUNGE) legen wir jetzt eine Mitteilung vor, die eine katamnestische Befragung bei weiteren 1719 andrologischen Patienten darstellt; unsere gegenwartige Studie ist um eine ganze Reihe von zusatzlichen Fragen erweitert worden, so dafi wir in der Lage sind, uber die Anzahl von Geburten hinaus einige wichtige Fragen zu beantworten (Tabelle 1). Aufierdem fugen wir in einem besonderen Teil dieser Arbeit eine katamnestische Erhebung bei 404 Patienten unserer Andrologie an, bei denen von anderer Seite eine Hormonbehandlung v o r der Oberweisung an uns vorausgegangen war; wir meinen, dai3 diese Angaben im Rahmen dieser Mitteilung aufierordentlich wichtig sind, weil sie einen unmittelbaren Eindruck davon verschafien, was in vielen Fallen durch den Hausarzt an Behandlung durchgefuhrt wird, ohne dai3 eine Diagnose gestellt wurde bzw. ein Spermiogramm angefertigt worden ist.Es ist nach unseren Erfahrungen nicht unwichtig, in welcher Weise sich der untersuchende Androloge des Patienten und seiner besonderen Note annimnit. Wir meinen damit, dai3 fur den ,,Erfolg" nicht so sehr die mikroskopische und die makroskopische sowie biochemische Untersuchung des Ejakulates neben der sachgerechten, grundlichen Untersuchung des Patienten wesentlich sindsie sind eine unbedingte Voraussetzung jeder sachgerechten Beratung und jeder Therapie! -, als vielmehr die ,,Moglichkeit zu einem Gesprach", in dem die Grundlage fiir eine vertrauensvolle Aussprache zwischen Arzt und Patient gelegt werden kann. Die starke Betonung dieser psychologischen Faktoren erscheint uns deswegen so besonders wichtig, weii sich immer wieder in der andrologischen Sprechstunde zeigt, dafi der Patient vor allem wegen eines derartigen Gespraches kommt, ja ein solches gewissermafien ,,erwartet". Man mufi sich also sehr vie1 mehr auf diese Gegebenheiten einstellen, wenn man wirklich im Interesse der betreuten Patienten arbeiten und behandeln will. OR ist es so, dafi eine ,,Behandlung" auch schon ein Gesprach zwischen Arzt und Patient sein kann (vgl. SCHIRREN und BUNGE). Unter diesen Voraussetzungen wurde von uns ein Fragebogen entwickelt, der an 1719 Patienten verschickt '>) Auszugsweise vorgetragen auf dem Ungarischen KongreB fur Urologie und Nephrologie-Symposium ANDROLOGIE am 26.-28. 9. 1968 in Budapest.
