Várias observações publicadas por Pierre Marie e Barré, em 1910, consignavam a frequência com que eram registadas as anomalias da convergência nos parkinsonianos post-encefalíticos. Posteriormente a casuística tornou-se maior, verificando-se que a encefalite epidêmica tem especial predileção pelos movimentos conjugados oculares, cujos distúrbios constituem importantes sequelas dessa moléstia.Dentre os diversos movimentos conjugados, a prática demonstra que os mais atingidos pela encefalite são a elevação dos olhos e a convergência. O primeiro aparece em parkinsonianos post-encefalíticos sob a forma de crises espasmódicas violentas que podem durar até duas horas. O segundo oferece maior interesse clínico, pois, costuma figurar em qualquer forma de encefalite epidêmica, mental ou neurológica. Constitue, às vezes, o único sinal neurológico de certos quadros mentais de dificil diagnóstico, como aqueles em que se verificam desvios da conduta e do comportamento atuando como núcleo central das perturbações psí-quicas.Pode a convergência apresentar-se em paralisia ou em paresia, sem afetar a ação dos dois retos internos nos movimentos de lateralidade, fato esse que demonstra comprometimento da função convergência por lesão nuclear. As paralisias acompanham-se de abolição da reação pupilar sincinética à convergência e, em geral, de paralisia da acomodação. Muito mais frequentes são as paresias da convergência complicadas ou não de distúrbios da acomodação e do reflexo foto-motor. A visinhança imediata dos núcleos de Perlia e de Edinger-Westphall, explica as alterações da convergência comumente associadas a modificações do reflexo foto-motor.A paresia da convergência apresenta-se com maior ou menor intensidade, podendo ser facilmente despistada pela mensuracão do ângulo máximo de convergência.
