Descrever a implementação da Conciliação de medicamentos em um Hospital de Ensino de Sergipe. Tratou-se de um estudo de ciência da implementação, que descreve a implementação da conciliação de medicamentos e os resultados preliminares deste serviço, no período de maio de 2016 a novembro de 2017. Inicialmente, foi realizado o diagnóstico situacional dos setores, seguido de fundamentação teórica, posteriormente foram elencados os indicadores a serem analisados: estrutura (estrutura física e recursos humanos), processos (fluxos de trabalho) e perfil dos pacientes atendidos (idade, sexo, perfil farmacoterapêutico). Assim, o próximo passo, foi descrever as etapas para implementação da conciliação de medicamentos. Além disso, caso outros setores do hospital almejem a implementação da conciliação de medicamentos, este estudo poderá auxiliar na projeção dos processos nas etapas de exploração, preparação e implementação inicial. Ademais, a promoção de atitudes colaborativas interprofissionaiscontribui para maior aceitação das intervenções farmacêuticas, bem como, uma ampliação no número de farmacêuticos destinados a serviços clínicos auxiliará no atendimento da elevada demanda por serviços de conciliação.
BACKGROUND The clinical activities developed by pharmacists in a hospital environment can improve health outcomes and generate savings for hospitals. However, to determine whether pharmaceutical interventions are cost effective, it is essential to define a method according to which cost-effectiveness is intended to be measured. In addition, the quality of economic assessments and the amount of information present in systematic reviews in the literature make it difficult to analyze the effects of this intervention. OBJECTIVE This paper aims to provide an overview of systematic reviews on the pharmacoeconomic impact of the performance of pharmaceutical care in hospitals. METHODS A systematic search of the Cochrane Library databases, PubMed or MEDLINE, LILACS, Scopus, Web of Science, Google Scholar, and Open Thesis will be performed using the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) statement. The search will involve the use of keywords determined using the Medical Subject Headings database to define the search terms and include the following terms: “pharmacoeconomics,” “pharmaceutical care,” and “hospital.” The study designs to be included will be systematic reviews of good quality. Studies will be included that address pharmacoeconomics; studies that evaluated pharmaceutical care in hospitals; and studies published in Portuguese, English, or Spanish. The primary outcome sought in the systematic reviews will be the cost ratio in monetary units and the outcomes in monetary or natural units. The secondary economic outcomes considered will be determined based on factors associated with the drugs and translated into benefit, efficacy, or utility. RESULTS It is intended to start this overview in January 2023. Thus far, only previous searches have been carried out to contextualize the theme and build the protocol. CONCLUSIONS This overview will determine the pharmacoeconomic impact of pharmaceutical care interventions in the hospital environment. In addition, this study will point out which clinical outcomes in natural units are impacted by the performance of pharmaceutical care and the strengths and limitations of each approach. O presente estudo tem como objetivo descrever a elaboração de um guia prático sobre compatibilidade entre medicamentos administrados em Y. Trata-se de um estudo de caráter descritivo, do tipo relato de experiência, que foi desenvolvido no período de março a agosto de 2022 no Hospital Universitário de Lagarto (HUL), a partir do levantamento dos medicamentos endovenosos padronizados no hospital. Para a construção do instrumento foi realizada revisão bibliográfica narrativa e consultas a guias e manuais publicados por outras unidades hospitalares especializadas e bulas de medicamentos. Os medicamentos que possuíam incompatibilidades foram listados em um quadro, bem como foram sinalizados aqueles que não apresentaram informações de testes de compatibilidade. Após revisão criteriosa dos itens padronizados no HUL, foram selecionados 123 medicamentos para compor o guia de incompatibilidade em Y, por representarem aqueles com administração por via endovenosa. O guia foi estruturado tendo como base o modelo padrão dos materiais produzidos pelo Setor de Farmácia Hospitalar do HUL. Para os medicamentos com dados insuficientes na literatura, foram colocados no quadro do guia os avisos de poucos testes, não há testes, não são indicadas misturas com outros medicamentos ou não foram encontradas informações nas referências, conforme pertinente. Os medicamentos que tiveram mais dados na literatura sobre incompatibilidades foram a fenitoína e o diazepam. Com este trabalho, foi possível demonstrar a importância de pesquisas com foco em estratégias que minimizem os potenciais danos causados por incompatibilidade de medicamentos.
Objective: To characterize compliance with good practices in medical gas management in federal teaching hospitals in Brazil. Methods: A cross-sectional survey-type study, designed to perform a situational diagnosis of the pharmacy services in 40 federal teaching hospitals in Brazil linked to the Brazilian Hospital Services Company, with respect to compliance with good practices in gas management, through the application of the ABPGasMed 1.0 instrument. This instrument consists of 54 compliance standards divided into two sections (structure and process). The characterization of research participants and hospitals, and the classification of hospitals in terms of performance categories were expressed as absolute and relative values. Chi-square tests of independence were performed to investigate the association between the hospital’s performance category and the hospital’s geographic region and size. Results: In total, 87.5% of the invited hospitals participated in the study, and only 27.59% of the hospitals had a pharmacist responsible for medicinal gases. Pharmacovigilance was performed by pharmacists in 20.59% of the hospitals. Analyzing the hospitals by region of the country and size, statistically significant associations were found between the general classification of hospitals and the geographic region (x2(8)=18.936, p= 0.015), as well as the classification of the hospital and structure and size (x2(9)= 20.373, p= 0.016). Analyses of the adjusted standardized residues returned an association between the southeastern region and the satisfactory performance category when analyzing the entire instrument, and between the excellent performance category in the structure section and size of a small hospital. Conclusion: In most of the hospitals studied, management of medicinal gases did not show the desired performance, which indicates the need to comply with current healthcare legislations and improve the provided services. It is believed that compliance rates may evolve training of healthcare team members, with an emphasis on the pharmacist.
Os incidentes que ocasionam dano ao paciente ocorrem diariamente em instituições de saúde. Estudos revelam que aproximadamente 10% de todos os eventos adversos a medicamentos estão relacionados a uma interação medicamentosa (IM). O objetivo do estudo foi avaliar as potenciais IMs em pacientes da clínica médica de um hospital universitário. O estudo foi do tipo transversal e prospectivo, com abordagem quanti-qualitativa, de agosto a dezembro de 2020. A pesquisa foi realizada com todos os pacientes idosos internados. A coleta de dado foi realizada por meio da análise dos prontuários, do quinto ao décimo dia de internação. Os dados dos pacientes foram analisados através da prescrição com ênfase nos medicamentos que foram administrados. As IMs foram classificadas quanto à gravidade e ao mecanismo de ação de acordo com a base de dados. As intervenções farmacêuticas foram realizadas, quando necessário, e avaliado o grau de aceitação pela equipe de assistência à saúde. Após a coleta, os dados foram registrados e analisados mediante estatística descritiva simples, bem como estatística comparativa. Verificou-se que 33,33% dos pacientes apresentaram possibilidade de desenvolver IM grave, sendo a mais frequente entre espironolactona e losartana, podendo provocar hipercalemia. As intervenções mais realizadas foram a monitorização de sinais, sintomas e achados laboratoriais. Pacientes polimedicados e hipertensos são mais suscetíveis à IM, isso pois os anti-hipertensivos são umas das classes mais envolvidas na ocorrência de IM. Assim, a atuação do farmacêutico pode constituir um dos principais instrumentos de prevenção das IMs, o que pode contribuir para a segurança do paciente.
