O objetivo foi analisar a tendência da razão de mortalidade materna (RMM) segundo a classificação CID-Mortalidade Materna (CID-MM) no Estado do Rio de Janeiro, Brasil, no período de 2006 a 2018. Foi realizado estudo de tendência temporal da RMM no Rio de Janeiro segundo tipo de óbito - direto ou indireto - e grupos de causas da classificação CID-MM, da Organização Mundial da Saúde (OMS). A RMM foi calculada com dados do Sistema de Informações sobre Mortalidade (SIM) e do Sistema de Informações sobre Nascidos Vivos (SINASC). As tendências foram estimadas pelo Joinpoint Regression Program. Dos 2.192 óbitos maternos no estado, 61% foram por causas diretas, 34% indiretas e 5% não especificadas. A tendência da RMM total e por causas diretas foi de declínio: 1,2% (IC95%: -2,3; -0,1) ao ano e 3,8% (IC95%: -4,9; -2,6) entre 2006 e 2015, respectivamente. Para causas diretas, segundo a CID-MM, o grupo 2 (causas hipertensivas) foi preponderante, mas houve declínio da eclâmpsia. Seguiram-se o grupo 5 (outras complicações, das quais se destacaram a categoria O90 e a subcategoria O90.3 - cardiomiopatia no puerpério) e o grupo 1 (gravidez que termina em aborto). As causas indiretas apresentaram estabilidade e a maioria pertencia à categoria O99 e suas subcategorias, relacionadas a doenças cardiovasculares e respiratórias. A RMM no Rio de Janeiro apresentou tendência de declínio entre 2006 e 2018, porém distante do recomendado pela OMS e com diferentes comportamentos entre as causas. A classificação CID-MM foi útil para identificar grandes grupos de causas, mas é preciso desagregar por subcategorias, para o adequado conhecimento da etiologia da morte materna.
Introdução: A sífilis congênita (SC) é uma doença evitável, que pode ser eliminada por meio da detecção eficaz no pré-natal e do tratamento adequado das mulheres grávidas infectadas. Entretanto, o número de casos de sífilis em gestantes e de SC cresceu exponencialmente nos últimos cinco anos. No ano de 2016, notificaram-se 37.436 casos de sífilis em gestantes e 20.474 casos de SC (50 casos / 100 mil nascidos vivos). Apesar da crescente detecção de casos de SC e do baixo custo do tratamento, o país não alcançou a meta proposta pela Organização Mundial da Saúde (OMS) de eliminar a SC. Objetivo: Este estudo visa analisar o perfil epidemiológico dos casos de sífilis em gestantes e de SC notificados no estado do Rio de Janeiro (RJ) nos dois últimos anos para quais o Sistema de Vigilância Epidemiológica disponibiliza dados (2016 e 2017). Métodos: Estudo descritivo realizado com o uso de dados colhidos da base do Sistema de Informação de Agravo de Notificação (SINAN) para o Estado do RJ, Brasil, no biênio 2016‒2017, e do Boletim Epidemiológico, Secretaria de Vigilância em Saúde - Sífilis 2017. Entre as variáveis disponibilizadas, analisaram-se realização do pré-natal, diagnóstico de sífilis materna e tratamento materno. Discussão: Nos anos de 2016-2017, 6.361 casos de SC foram notificados pelo SINAN. De um total de 6.362 gestantes diagnosticadas com sífilis no período, 83,2% (5.292) tinham realizado pré-natal, 10,6% (676) não tinham realizado pré-natal e não havia informação sobre a realização da assistência pré-natal em 6,2% dos casos. O diagnóstico de sífilis ocorreu durante o pré-natal em 3.915 casos (61,6%), no momento do parto/curetagem em 1.783 pacientes (28%) e após o parto em 330 casos (5,2%). Em 12 (0,2%) mulheres, não houve diagnóstico de sífilis e em 321 casos o diagnóstico foi ignorado (5%). Dados de 2016 mostraram que 94,3% das 5.540 gestantes foram tratadas com pelo menos uma dose de penicilina benzatina independente da forma clínica, 0,7% foram tratadas com outro esquema, 2% não realizaram o tratamento e para 3% dos casos não consta o dado sobre o tratamento. Conclusão: Pode-se reduzir significativamente a incidência de SC com um conjunto relativamente simples de intervenções. Para isso, é preciso assegurar que todas as gestantes diagnosticadas, bem como seus parceiros e recém-natos, tenham acesso a serviços de saúde de qualidade e, consequentemente, sejam tratados de forma adequada. O preenchimento adequado das fichas de notificação compulsória e da caderneta de gestante pelos profissionais de saúde também é imprescindível. Como se pode observar, alguns dados se encontram omissos do sistema de vigilância epidemiológica. A disponibilidade dessas informações é importante para traçar novas estratégias, a fim de atingir a meta proposta pela OMS: eliminar a SC.
Introdução: A Organização Mundial da Saúde (OMS) define como morte materna aquela que ocorre durante a gestação ou em um período de 42 dias após o seu término, independentemente da duração ou da localização, decorrente de qualquer causa relacionada com a gravidez ou de medidas em relação a ela, porém não de causas acidentais ou incidentais. O óbito materno direto é aquele relacionado à gestação, parto ou puerpério, e o indireto relaciona-se a doenças maternas preexistentes ou contraídas durante esse mesmo período. Em 2000, a Organização das Nações Unidas estabeleceu os Objetivos de Desenvolvimento do Milênio (ODM), de acordo com os quais o Brasil atingiria 35 óbitos/100 mil nascidos vivos (NV) até 2015. O Ministério da Saúde estima que a Razão de Mortalidade Materna (RMM) foi aproximadamente o dobro do objetivo (60/100 mil NV) em 2015. Redobrados esforços serão necessários para atingir o Objetivo de Desenvolvimento Sustentável (ODS) de 30 óbitos/100 mil NV em 2030. Objetivo: Analisar a tendência da RMM no estado do Rio de Janeiro (RJ) no período de 2005 a 2016 e descrever os óbitos de gestantes e puérperas em 2016, de acordo com as variáveis sociodemográficas disponíveis. Métodos: Estudo descritivo realizado com base em dados obtidos do Departamento de Informática do Sistema Único de Saúde (DATASUS) do RJ, Brasil, 2005‒2016. A população alvo deste estudo consistiu nas mulheres residentes no estado do RJ que apresentaram mortalidade materna registrada no DATASUS. Entre as variáveis disponibilizadas pelo sistema de informação acessado, analisaram-se, além dos óbitos ocorridos no estado em 2005‒2016, as causas do óbito, escolaridade, idade e etnia. Resultados: A RMM no Estado do RJ se manteve estável ao longo do período estudado. No entanto, a mortalidade materna por causas diretas mostrou queda de 27%, passando de 52 óbitos/100 mil NV em 2005 para 38 óbitos/100.000 NV em 2016. Em contrapartida, as causas indiretas apresentaram aumento de 180% no mesmo período, variando de 10 a 28 óbitos/100.000 NV. No que tange às variáveis sociodemográficas, observa-se que em 2016 as maiores RMM no estado do RJ foram entre mulheres negras, de 40-49 anos, com escolaridade menor que quatro anos. Quanto à etiologia, a síndrome hipertensiva da gravidez foi a principal causa de morte materna, seguida de hemorragia obstétrica, aborto e infecção. Conclusão: A diminuição dos óbitos por causas diretas e o aumento daqueles por causas indiretas sugerem melhora na atenção durante o momento do parto. No entanto, ainda se perdem oportunidades na identificação de riscos durante a atenção pré-natal. Em 2009, por exemplo, houve aumento da RMM por causas indiretas, principalmente em consequência do surto de H1N1 no RJ, risco que poderia ter sido minimizado com atenção pré-natal de qualidade. Ações conjuntas são necessárias para contemplar cuidados tanto no período pré-natal quanto no momento do parto, a fim de atingir a meta estabelecida pelos ODS.
