The vitamin profile of 473 elderly persons was compared with that of 204 healthy volunteers (controls). Of the 473 elderly, 327 (mean age 87) lived in nursing homes, and 146 (mean age 77) lived at home. The 204 controls were in the 20–50 year age group. The circulating levels of biotin, pantothenate, riboflavin, vitamins A, B6, B12, C, E, folate, thiamine, nicotinate, and carotenes were determined in all groups. Hypovitaminemia was not obvious for biotin, pantothenate, riboflavin, vitamins A, E, and carotenes in either the institutional or non‐institutional elderly. Thiamine, vitamin C and vitamin B12 levels were strikingly depressed in the non‐institutional elderly as compared with the institutional population or the controls. Over 30 percent of the institutional elderly had vitamin B6 and nicotinate hypovitaminemia compared to the controls. Both the institutional and the non‐institutional elderly showed depressed levels of folate and vitamin B12. Vitamin supplementation reduced the percentage of folate and B12 deficits. The marked vitamin deficits in the institutional population, in descending order of incidence were: vitamin B6, nicotinate, vitamin B12, folate, thiamine, and ascorbate, whereas the pattern for the non‐institutional elderly was: vitamin B12, thiamine, ascorbate, vitamin B6, nicotinate, and folate. In the 473 elderly studied, vitamin B6, nicotinate, and vitamin B12, in that order, were the most common deficits; folate, thiamine and ascorbate deficits comprised a lesser percentage. The role of vitamin deficits and the effects upon the aging population are discussed.

Several studies of workers exposed to various forms of chromium compounds have suggested an increased incidence of respiratory cancers. Lead and zinc chromates were among the chromium compounds implicated. The Department of Preventive Medicine and Community Health of the New Jersey Medical School undertook a detailed mortality study of a pigment plant in Newark which utilized both of these compounds. We compared observed deaths from each cause among 1296 white and 650 non-white males who were employed at the plant between January 1, 1940 and December 31, 1969, with expected deaths, as computed from cause-, age-, and time-specific standard death rates for the United States. A statistically significant relative risk of 1.6 for lung cancer among white male employees was found, as well as among the cohorts of white males employed 10 yr or more. A relative risk of 1.9 was noted for individuals employed at least 2 yr who were at least moderately exposed to chromates. An increased incidence of lung cancer among non-white males and stomach and pancreatic cancer among the total cohort was also evident. These data support the validity of the association between chromate pigment exposure and lung cancer, suggesting that pigment workers and other exposed to chromates be observed carefully in regard to possible risk or pancreatic and stomach cancers.

A case-control study of bladder cancer in two northern counties of New Jersey was conducted to investigate a tumour that has been considered to be strongly associated with industrial and environmental exposures. The study population included 75 bladder cancer cases and 142 controls. Cases and controls were matched for race, sex, age, place of birth and place of residence. Statistically significant associations with bladder cancer and risk ratios of greater than 2.0 were found for cigarette smoking and for working in dye, petroleum (fuel) or plastics industries. No statistically significant association was found for: cigar and pipe smoking; caffeine, saccharine and alcohol consumption; and life time occupational history of working in other than dye, petroleum and plastics industries. No statistically significant differences between cases and controls were found in family history of cancer. Risk ratios of at least 2.5 (but without statistical significance possibly because of sample size) were found for workers in rodenticide and printing industries, for cable workers and for cancer in the spouses of bladder cancer cases. Simultaneous multiple primary cancer sites were found in 9.3% of the bladder cancer patients; this is higher than the 0.2-8% reported in the medical literature. The life time occupational history of the bladder cancer cases points to industrial determinants: some are known (petroleum and dye industries) but the association with the plastics industry is new. If our findings are confirmed, investigations will be needed to determine whether any specific chemical or combination of chemicals used in the plastics industry is responsible for bladder cancer induction.
