Introdução: Os atuais avanços tecnológicos são responsáveis por constantes aprimoramentos na área da saúde, na odontologia uma das especialidades que mais se beneficia com as constantes atualizações do mercado é a endodontia que constantemente preocupa-se na resolução de desafios clínicos como o tratamento de canais calcificados, remoção de retentores radiculares e cirurgias de apicectomia. Objetivo: elucidar as vantagens e limitações da cirurgia endodôntica assistida por computador. Metodologia: Uma revisão integrativa da literatura avaliou qualitativamente os achados resgatados na Pubmed, Web of Science, Embase, Scopus e Cochrane Library utilizando a estratégia de busca baseada nos descritores em ciências da saúde “("endodontics" AND "Surgery, Computer-Assisted")”. Não houve exclusão quanto ao idioma ou data de publicação, todavia manuais de uso, bem como notas técnicas foram excluídas. Resultados e discussão: Dos 117 artigos originalmente resgatados, apenas 11 cumpriram com nossos critérios de inclusão e exclusão. Todos os artigos ressaltam a técnica de endodontia guiada como segura e enfatizam seu uso em desafios da clínica de endodontia, nenhum artigo mencionou desvantagens no uso. Conclusão: A endodontia guiada é uma técnica segura e eficaz para situações críticas como canais calcificados, remoção de retentores radiculares e cirurgias de apicectomia, além de ser efetiva na preservação da estrutura dentinária.
