Objetivo: identificar a evolução da cobertura vacinal infantil no Brasil entre 2010 e 2020 e os impactos sobre as metas de imunizações provocados pela pandemia de COVID-19. Método: estudo observacional ecológico, com dados das coberturas vacinais em crianças menores de um ano, no Brasil, entre 2010 e 2020. Esses dados foram coletados através do Sistema de Informações do Programa Nacional de Imunizações, enquanto os demográficos pelo Sistema de Nascidos Vivos, socioeconômicos pelo Instituto Brasileiro de Geografia e Estatística e de isolamento social através da empresa Inloco. Foram trabalhados estatisticamente por regressão linear simples e correlação linear. Resultados: a amostra estimada em 31.844.835 crianças mostrou redução das coberturas para BCG (Coeficiente=-2,718; p<0,001) e Tríplice Viral (Coeficiente=-2,200; p=0,014), incremento para Febre Amarela (Coeficiente=1,264; p=0,032) e Pentavalente (Coeficiente=7,418; p=0,031), e estabilidade para Rotavírus (Coeficiente=-0,445; p=0,401). Em 2020, não houve correlação entre os níveis de isolamento social e as quedas na cobertura, apenas na região Norte, essa relação, para Tríplice Viral (Correlação de Spearman=-0,683; p=0,020), se mostrou significativa. Conclusão: perfil de baixas coberturas vacinais entre 2010 e 2020 devido a fatores como faixa socioeconômica e hesitação vacinal. Ainda, ocorreu estabilidade em 2020, já que o isolamento social não foi associado às baixas coberturas, sendo que as mesmas já se apresentavam em queda anteriormente à pandemia.
