ResumoO íleo biliar é uma complicação atípica e rara da colelitíase, responsável por 1-4% dos casos de obstrução abdominal mecânica. É decorrente da impactação de um cálculo litisiáco no trato gastrointestinal, resultando em um quadro de obstrução intestinal, que, se não tratada a tempo, pode apresentar elevados índices de morbimortalidade. A taxa de óbito por íleo biliar é de 20%. O presente trabalho tem como objetivo relatar um caso de uma paciente de 64 anos que apresentou quadro de abdome agudo obstrutivo, que após tratamento cirúrgico com realização de enterolitotomia, foi diagnosticado como íleo biliar. O trabalho visa, ainda, evidenciar a importância do conhecimento prévio dessa rara patologia, visto que a taxa de diagnóstico pré-operatório é moderadamente baixa e o tratamento é eminentemente cirúrgico. Desta forma, podese concluir que o ílio biliar é um dos diagnósticos diferenciais em quadros de abdome agudo obstrutivo, que se não tratado adequadamente em tempo hábil, pode levar a alta morbimortalidade, em função das comorbidades associadas. AbstractBiliary ileus is an atypical and rare complication of biliary lithiasis, responsible for 1-4% of mechanical instestinal obstruction cases. It is caused by the impaction of a gallstone in the gastrointestinal tract, resulting in obstruction which, if not treated in time, may cause high morbidity and mortality. The mortality rate of bileary ileus is 20%. The aim of the author is reports the case of a 64-year-old female with bowel obstruction which, after surgical treatment with enterolithotomy, was diagnosed as biliary ileus. The purpose of this paper is to assert the importance of prior knowledge of this rare pathology, considering that the rate of preoperative diagnosis is moderately low and the treatment is eminently surgical. In conclusion, biliary ileus should be considered as one of the main differential diagnosis in acute obstructive abdomen in view of the high morbidity and mortality rate associated with its comorbities when not properly treated.
O objetivo deste artigo é um relatar um caso de paciente psiquiátrico atendido no Hospital Universitário Sul Fluminense (HUSF), no município de Vassouras/RJ, com quadro recidivante de bezoar metálico no trato gastrointestinal. Bezoares são corpos estranhos que podem obstruir o lúmen do aparelho digestivo a partir da ingestão continuada, prolongada e intencional de materiais não comestíveis ou de medicamentos de uso contínuo. Os bezoares podem apresentar extensão em continuidade para o intestino delgado ou fragmentação com múltiplas massas detectáveis em qualquer segmento intestinal. Neste caso foram realizadas laparotomias exploradoras para retirada dos corpos metálicos em região gástrica. Houve evasão do paciente do HUSF, o que impediu o acompanhamento do caso por equipe multidisciplinar
Rev Med (São Paulo RESUMO:Introdução: o carcinoma espinocelular consiste em uma neoplasia maligna dos queratinócitos epidérmicos supra basais, que frequentemente crescem em áreas fotoexpostas, sendo a exposição cumulativa à radiação ultravioleta o principal fator de risco. O carcinoma espinocelular contribui anualmente para os óbitos atribuídos ao câncer de pele não melanoma. É um tumor maligno das células espinhosas da epiderme, de evolução rápida, altamente invasivo e metastático. Objetivo: relatar um caso de carcinoma espinocelular, moderadamente diferenciado, na região de couro cabeludo com acometimento da calota craniana em indivíduo do gênero masculino, 68 anos, diabético, leucoderma, tabagista e etilista, que durante 10 anos exerceu atividades de ambulante na praia, sem utilização de qualquer proteção química ou mecânica contra o sol. Conclusão: ações de educação em saúde sobre proteção solar são essenciais para prevenção ao câncer de pele e devem ser incorporadas ao cotidiano da prática médica. O diagnóstico precoce está diretamente relacionado ao prognóstico da doença e à qualidade de vida do portador. ABSTRACT:Introduction: Squamous cell carcinoma consists in a malignant neoplasm of the basals epidermal keratinocytes, which often grow in sun-exposed areas and this cumulative exposure to ultraviolet radiation is its main risk factor. Each year, the squamous cell carcinoma contributes to thousands of deaths attributed to non-melanoma skin cancer. It is a malignant tumor of the prickle cells of the skin characterized by faster development, highly invasive and metastatic. Objective: To report a squamous cell carcinoma case, moderately differentiated, in the scalp area, with skullcap involvement in a male patient, 68 years old, diabetic, with light-skinned, smoker and alcohol consumer that during 10 years was a salesman on the beach without using any chemical or mechanical protection against the sun resulting inactinic keratosis lesions in sun-exposed areas. Conclusion: health education activities on sun protection are essential for prevention of skin cancer and should be incorporated into medical practice. Early diagnosis is directly related to the prognosis of the disease and the patient quality of life.
