The mechanism of neurological disturbances in patients with mitochondrial myopathy, encephalopathy, lactic acidosis, and stroke-like episodes (MELAS) is controversial. We studied 12 patients with MELAS using conventional and diffusion weighted MRI (DWI) and MR spectroscopy (MRS), to look at the physiopathology of the stroke-like events. Although conventional MRI showed lesions in all patients, DWI was more sensitive. One patient did not show high signal on DWI 48 h after a from stroke-like episode, but MRS demonstrated a lactate peak in left occipital lobe; 2 weeks after the attack, high signal was demonstrated on the right frontal lobe where MRS had shown a lactate peak. Our findings suggest a possible predictive ability of (1)H-MRS, in showing early MELAS lesions and supports the hypothesis that mitochondrial metabolic dysfunction may precedes abnormalities on DWI.
We report two cases of angiomyolipoma of the liver, where small amounts of fat on computed tomography, magnetic resonance imaging, and angiography made distinguishing this tumor from other hypervascular tumors difficult. Recognizing the imaging features of no capsule, hypervascularity of the tumor including the fat component, and early venous drainage may be helpful for correctly diagnosing angiomyolipoma of the liver.
Abstract.A preoperative molecular-based diagnostic technique for thyroid papillary and anaplastic carcinomas was recently developed to detect oncofetal fibronectin (onfFN) messenger RNA (mRNA) in fine needle aspiration biopsy (FNAB) by means of reverse transcription-polymerase chain reaction (RT-PCR).We report two cases of thyroid tumors, in which cytological diagnosis was not successful but in which RT-PCR analysis provided information that helped to identify the biological features of the tumors.One case could not be diagnosed by aspiration biopsy cytology (ABC) because of poor fixation; however, RT-PCR did detect the expression of onfFN mRNA, which suggested the existence of papillary carcinoma cells. After surgery, this tumor was diagnosed as a papillary carcinoma by histological examination. The other case was diagnosed as a papillary carcinoma by ABC, but onfFN mRNA was not detected in the FNAB by RT-PCR. The neoplasm was diagnosed as a follicular tumor by histological examination.These cases suggest the benefits of combining both genetic and cytological approaches to the examination of FNAB.
