RESUMOIntrodução: O tumor de pele não melanoma é o de maior prevalência e incidência no Brasil, englobando o carcinoma espinocelular (CEC) e o basocelular (CBC). O principal fator de risco para o desenvolvimento desses tumores é a exposição crônica aos raios solares, justificando a faixa etária e as localizações anatômicas mais acometidas. O tratamento objetiva extirpar a lesão com deformidade mínima. A excisão cirúrgica com margens seguras é a principal opção terapêutica; outras dependem do número de lesões, localização, tipo histológico, estado geral e comorbidades clínicas. Objetivos: Descrever o perfil epidemiológico dos pacientes com tumores cutâneos malignos; identificar a prevalência dos não melanoma; e avaliar a margem cirúrgica. Métodos: Estudo observacional, longitudinal e retrospectivo de pacientes submetidos à ressecção de lesões suspeitas em serviço secundário. Resultados: Foram identificadas 140 lesões em 67 pacientes, sendo 59% do sexo masculino e 71,6% maiores de 60 anos. O segmento mais acometido foi cabeça/pescoço (72,1%); 69,1% eram CBC, 29,2% CEC e 1,6% melanoma; 80,4% informavam margens livres, 7,3% indicavam comprometimento; esses pacientes apresentavam média de 75,2 anos e 8,7 lesões concomitantes. A associação foi significante entre gênero feminino e presença de lesões <5 mm. Conclusões: O predomínio de lesões em indivíduos do sexo masculino acima de 60 anos é compatível com o padrão já descrito na literatura. As margens livres, obtidas em grande parte do presente estudo, indicam eficácia terapêutica da excisão cirúrgica simples. As margens macroscópicas adotadas na marcação pré-operatória favorecem a eficácia, reservando a cirurgia de Möhs para recidivas ou comprometimento de margem. ABSTRACT Introduction: Non-melanoma skin tumor is the one with highest prevalence and incidence in Brazil, comprising squamous cell carcinoma (SCC) and basal cell carcinoma (CBC). The main risk factor for the development of these tumors is chronic exposure to sunlight, justifying the most affected age group and anatomical locations. Treatment aims to excise the lesion with minimal deformity. Surgical excision with safe margins is the main therapeutic option; others depend on the number of lesions, location, histological type, general health and medical comorbidity. RESUMOIntrodução: A ferida da queimadura, a princípio, é estéril. Esse período varia de 24 a 72 horas. Pela análise microbiológica, o Staphylococcus aureus é o germe mais frequentemente encontrado, seguido de Pseudomonas aeruginosa e Escherichia coli. Objetivo: Analisar qualitativamente a microbiota colonizadora das lesões por queimaduras, nas primeiras 24 horas do ocorrido, em 25 pacientes atendidos no Centro de Tratamento de Queimados (CTQ) do Conjunto Hospitalar de Sorocaba (CHS). Método: Foram estudadas as amostras da margem e da área central da ferida pela técnica de coleta com uso de swab estéril. Resultados: Das 50 amostras analisadas, 15 (60%) pacientes possuíam positividade para colonização de microrganismos tanto na região central quanto na margem destas. Nas amostras obtidas da região central da lesão, pudemos encontrar seis colonizadas pelo microrganismo Staphylococcus sp e oito por Staphylococcus aureus, com valores semelhantes às amostras retiradas da região marginal, com oito colonizadas pelo Staphylococcus sp e sete pelo Staphylococcus aureus. Conclusão: As lesões por queimaduras nas primeiras 24 horas são colonizadas por microrganismos. A microbiota possui correlação com o descrito na literatura consultada. A análise microbiológica precoce de queimaduras demonstra-se pertinente para a constatação de colonização e subsequente manejo para prevenção de invasão e de infecção dessas lesões, dessa forma contribuindo indiretamente para a queda na morbidade e mortalidade associada às queimaduras. ABSTRACT Introduction:The burns wound initially is sterile, if no contaminated materials are put on the burn area or if the patient have not fell on dirty surfaces. This time gap, from 24 to 72 hours, is considered clean or sterile. Staphylococcus aureus is the most frequently found germ on burn wounds, followed by Pseudomonas aeruginosa and Escherichia coli. Objective: Quantitative analysis of the microbiota of the burn wounds, in the initial 24 hours of the injurie, in 25 patients from the Burns Unit of "Conjunto Hospitalar de Sorocaba". Methods: Samples taken with swab technic from the center and periphery of the wounds were analyzed. Results: In an amount of 50, 15 samples (60%) of the patients were tagged positive with peripheric and central colonization of microorganisms. In the sample's central portion, 6 were colonized by Staphylococcus sp and 8 by Staphylococcus aureus. Conclusion: The burn wounds in the initial 24 hours are colonized with microorganisms. The main microbiota is similar to the previously consulted literature. With this conclusion, the early microbiological analysis seems pertinent to the confirmation of wound colonization and the following care to prevent its infection, contributing indirectly to the decrease of morbidity and mortality related to burning.
A new animal model for training in rhinoplasty A new animal model for training rhinoplasty Novo modelo animal para treinamento de rinoplastia ABSTRACT Background: Rhinoplasty poses a unique set of challenges to the plastic surgeon. Surgical dexterity is associated with a long learning curve; this sometimes causes undesirable results, especially in the case of surgeons who are beginners at performing such a complex procedure. Understanding the anatomic basis and mechanical properties of its components is fundamental for achieving satisfactory results in rhinoplasty. To enhance surgical training for those learning the principles of rhinoplasty, a wide variety of models has been described. Among these are the nose of the pig and rabbit, chicken sternal cartilage, nasal silicone models, and even a virtual computer-generated 3-dimensional model of the human nose. Objective: The authors propose the use of a sheep head as a model to replicate the technical steps involved in rhinoseptoplasty for surgical education and research. Methods: Several fresh domestic sheep heads were obtained from a local butcher shop, dissected according to a predetermined set of steps, and modeled in different shapes as described in the literature for human models. Sutures were placed in the cartilage to simulate the steps of a structured rhinoplasty. Results: The caprine model presents anatomical structures similar to those found in humans, including the medial and lateral crura, with appropriate cephalic orientation and domal angles. Septal cartilage was sufficient to simulate several grafts, and bones were available for fracture and rasping. Conclusions: With an estimated more than 1 billion domestic sheep worldwide, this model has the potential to improve outcomes in rhinoplasty by providing wider opportunities for training in a procedure that requires knowledge, precision, and artistry.
RESUMOO papel da vitamina D nativa na fisiologia do tecido ósseo é evidente não só como um nutriente essencial capaz de prevenir e tratar o raquitismo mas também como regulador da homeostase do cálcio, fosfato e do metabolismo ósseo. Um amplo espectro de ações biológicas tem sido descrito, incluindo efeitos sobre os sistemas cardiovascular, nervoso central e imune, na diferenciação e crescimento celular, bem como no controle de outros sistemas hormonais. Evidências epidemiológicas associaram estados de déficit de vitamina D com doenças como câncer, diabetes mellitus, hipertensão arterial, doenças autoimunes, infecções e aumento da mortalidade. O conjunto dessas informações levou à medicalização da vitamina D, acarretando seu uso em diversas doenças, e até mesmo em situações clínicas questionáveis. No entanto, os resultados de estudos clínicos apontam para resultados divergentes, colocando em questão o uso indiscriminado da vitamina D. Este artigo apresenta uma revisão sobre a vitamina D e relaciona algumas das principais evidências científicas sobre os reais benefícios de sua suplementação em seres humanos em diversas condições. Nas dé-cadas seguintes, tornou-se evidente o papel da vitamina D na fisiologia do tecido ósseo não só como um nutriente essencial capaz de prevenir e tratar o raquitismo mas também como regulador da homeostase do cálcio (Ca), fosfato (P) e o do metabolismo ósseo.
