Objetivo: Demonstrar a necessidade de estratégias desenvolvidas pelo farmacêutico junto a equipe multidisciplinar para a correção de falhas, aperfeiçoamento do serviço e contribuir com a segurança do paciente no processo de preparação, administração e uso de medicamentos. Métodos: Revisão integrativa realizada nas bases de dados,Scientific Eletronic Library Online, National Library of Medicine, Literatura Latino-Americana e do Caribe em Ciências da Saúde, US National Library of Medicine, Biblioteca Virtual em Saúde, Microsoft Academic e Science direct, utilizando-se os domínios, “medicamentos”, "segurança do paciente” e “hospital” combinando-os com os operadores booleanos 'AND' e 'OR'. As publicações foram selecionadas de acordo com os critérios de inclusão e exclusão; a análise dos artigos se deu por pares; não houve restrição de idiomas. Resultados: A busca resultou em 66 artigos, dos quais: 42 foram excluídos por temática diversa à desta revisão, 2 dissertações e 4 duplicatas. Restaram 18 artigos elegíveis divididos em 4 eixos temáticos: Fatores de risco associados à prática de preparo e administração de medicamentos, Percepção da equipe assistencial e do paciente quanto a importância de práticas seguras, Ações de assistência farmacêutica para prevenção do erro de medicação e Contribuição das práticas seguras para as instituições hospitalares e segurança do paciente. Conclusão: O trabalho do farmacêutico junto da equipe multidisciplinar no hospital é fundamental para a promoção da segurança do paciente quanto a utilização de medicamentos. Existem várias estratégias que refletem o papel do farmacêutico na segurança do uso de medicamentos, contribuindo diretamente na melhoria dos processos assistenciais e gerenciamento adequado de custos.
Essa pesquisa visa abordar a orientação farmacêutica na alta hospitalar visto que no processo de alta, na transição da internação para o autocuidado, onde o paciente fica vulnerável é de extrema importância a reconciliação medicamentosa e uma comunicação eficaz entre a equipe e o paciente. Métodos: Revisão integrativa da literatura realizada nas bases de dados Biblioteca Virtual de Saúde (BVS) e Pubmed, utilizando se os domínios “segurança do paciente”, “alta hospitalar” e “reconciliação medicamentosa”. As publicações foram selecionadas de acordo com os critérios de inclusão e exclusão; a análise dos artigos se deu por pares; idiomas: inglês e português. A busca nas bases de dados resultou em 13 estudos, para o processamento e análise dos dados, utilizou-se o software IRAMUTEC e a aplicação do formulário URSI. Resultados: A presente RIL tem um bom nível de evidência científica, colaborando para ampliar os conhecimentos acerca da temática. A Classificação Hierárquica Descendente (CHD) identificou três classes semânticas conforme a análise dos domínios textuais e interpretação dos significados: 1- Alterações e trocas terapêuticas como principal causa de danos e eventos adversos após a alta hospitalar; 2- Educação em saúde e orientação medicamentosa como estímulo ao autocuidado após a alta hospitalar; 3- Informação e acompanhamento domiciliar para a melhora do problema em relação a uso de medicamentos após a alta hospitalar. Conclusão: A presente revisão permitiu esclarecer a importância do farmacêutico na orientação de alta e como o mesmo pode intervir no processo do autocuidado, ainda que o seu papel não esteja bem definido.
Objetivo: Buscar evidências sobre como prevenir potenciais erros de medicação na prescrição, preparo e administração de medicamentos para pacientes de Unidade de Tratamento Intensivo Pediátrica e Neonatal. Método: Revisão sistemática e síntese narrativa. Resultado: 18 artigos foram selecionados com evidências sobre como prevenir potenciais erros de medicação na prescrição, preparo e administração de medicamentos para pacientes de Unidade de Tratamento Intensivo Pediátrica e Neonatal. Os estudos são bem variados quanto a metodologia adotada. Tem-se, artigos que fizeram revisões sistemáticas, integrativas e narrativas, estudo de coorte prospectivos e retrospectivos, estudos observacionais e de análise econômica, sendo a maioria de países europeus, não trazendo informações sobre equidade ou análise de subgrupos. Conclusão: Há evidências de que intervenções estruturais e tecnológicas; organizacionais e profissionais; regulatórias e educacionais e interdisciplinares e multifacetadas, reduzem erros de prescrição e de preparo, interações medicamentosas, reações adversas a medicamentos, melhorando a qualidade dos serviços e a segurança do paciente.
Introdução: Dentre as diversas áreas de sua competência, a Farmácia Hospitalar é responsável pela promoção e monitoramento do uso seguro de medicamentos. Objetivo: Investigar erros de prescrição, de dose, de preparo e desperdício de antimicrobianos distribuídos de forma individualizada para pacientes de UTI-Neonatal na Fundação Santa Casa de Misericórdia do Pará. Metodologia: Trata-se de uma pesquisa transversal, retrospectiva, fundamentada na pesquisa-ação, tendo como recorte temporal o período de novembro de 2021 a janeiro 2022. As variáveis relacionadas aos medicamentos foram comparadas ao protocolo de prescrição do NEOFAX®. Resultado: O estudo envolveu 76 pacientes e analisou 213 prescrições que envolveram 341 antimicrobianos, dos quais, 280 (82%) dos antimicrobianos prescritos não apresentavam clareza quanto às instruções de preparo e 98 (29%) apresentaram as doses em desacordo com o NEOFAX®. Os itens de maior dúvida foram quanto à estabilidade após o preparo: 341 (100%); quanto ao volume final após preparo 341 (100%) e quanto a técnica de preparo 266 (78%). Do total de pacientes, 32 (42%) eram de muito baixo peso, seguidos de 26 (34%) de extremo baixo peso, e 18 (24%) de baixo peso. Conclusão: Os dados obtidos no estudo subsidiaram a implantação de uma central de misturas intravenosas na instituição estudada.
