ZusammenfassungHintergrund: Heiserkeit entsteht durch Luftverlust und irreguläre Stimmlippenschwingungen. Seitdem es möglich ist, Stimmlippenschwingungen mitsamt Irregularitäten in Echtzeit zu visualisieren, haben sie eine klinische Bedeutung erhalten. Methode: Die Stimmlippenschwingung wird mit 4000 Bildern/Sekunde aufgenommen (digitale Hochgeschwindigkeits-Videokamera). Bereits die Betrachtung der verlangsamt abgespielten Aufnahmen (¹Zeitlupeª) zeigt zwei wesentliche Kategorien von Irregularitäten: seitendifferente Stimmlippenschwingungen und longitudinale Schwingungsmoden, die sich von dorsal nach ventral ausbreiten. Mit einer eigens entwickelten BildverarbeitungsSoftware werden die Bilddaten zu Mehrlinien-Kymogrammen und Contour-Diagrammen komprimiert, die für Auswertung geeignet sind. Ergebnisse: Während Bildverarbeitung zum diagnostischen Standard von HG-Aufnahmetechniken gehört, ist die Interpretation mit einem biomechanischen Modell, also die numerisch-mathematische Lösung eines Systems von Differentialgleichungen bislang nur von experimenteller Bedeutung. Damit lassen sich irreguläre Schwingungsformen der Stimmlippen physikalisch erklären sowie ± indirekt ± Parameter berechnen, die entscheidend die Schwingung beeinflussen, aber einer direkten Messung nicht zugänglich sind: der subglottische Druck und der muskuläre Tonus rechts und links. Diskussion: Zwei Kategorien von Irregularitäten lassen sich klinischen Störungsbildern zuordnen: unterschiedliche Grundfrequenzen der beiden Stimmlippen (erklärbar durch Asymmetrie in mediolateraler Richtung) entsprechen am ehesten einer organischen Dysphonie. With real time resolution, they can be observed only using highspeed recording techniques (8804; 10 000 images/s). Method: During the last years, we developed a recording device and an image processing software, dedicated for extraction of vocal fold motion curves from the image series. Irregular vibrations can be understood by using Hirano's ¹body-cover-modelª: one vocal fold is assumed to act as a system of two coupled oscillators (the vocal muscle and the epithelium, coupled by the connective tissue of Reinke's space). Results: By applying this biomechanical twomass model and inversely solving Bernoulli's equation, in selected cases the driving parameters of the vibration were computed from the highspeed image series. These parameters, like the subglottal pressure und the tension of the vocal muscle on both sides, can in general not be measured directly. Discussion: From the model it can be supposed, that laryngeal asymmetry (either in mass or tension) is the deeper reason causing irregularity.
