Introduction
La prévalence du dysfonctionnement thyroïdien et son association avec le pronostic de la maladie COVID-19 est toujours un sujet de controverse.
Objectif
Déterminer la prévalence et l’impact de l’hyperthyroïdie sur le pronostic de la maladie.
Matériel et méthode
Il s’agit d’une étude analytique (entre septembre 2020 et février 2021) en réanimation médicale, incluant les patients admis pour une infection sévère au Covid-19 et ayant un bilan thyroïdien à l’admission. Les patients présentant des antécédents de dysthyroidie préexistants ont été exclus. Deux groupes ont été définis : groupe 1 : patients présentant une euthyroïdie ; groupe 2 : patients présentant une hyperthyroïdie (périphérique et centrale).
Résultats
Durant la période de l’étude 68 patients d’âge moyen 61 ± 12 ans ont été inclus. La comorbidité la plus fréquente était l’hypertension artérielle (48,5 % des cas). Une dysthyroïdie était présente dans 27,9 % des cas. Parmi ceux-ci, 23,5 % avaient une hyperthyroïdie périphérique, 2,9 % une hypothyroïdie périphérique et 1,5 % une hypothyroïdie centrale. La mortalité globale était de 45,6 %. Les patients ayant une hyperthyroïdie étaient plus susceptibles de présenter une infection nosocomiale (75 % contre 40 %, p = 0,01). Il n’y avait pas de différences significatives en ce qui concerne la durée de la ventilation mécanique [11 ± 5 vs 12 ± 7 jours, p = 0,8], la durée de séjour [17 ± 10 vs 15 ± 10 jours, p = 0,54] et la mortalité (56 % contre 42 %, p = 0,32).
Conclusion
Dans cette étude, nous avons constaté que les patients souffrant d’hyperthyroïdie étaient plus susceptibles de présenter une infection nosocomiale.
Introduction
L’effet de l’infection sévère au Covid-19 sur les surrénales reste mal élucidé. Nous reportons les cas de trois patients admis pour une pneumopathie covid-19 en réanimation avec survenue d’une IS aiguë.
Cas reportés
Il s’agit de trois patients, une femme et deux hommes âgés respectivement de 77, 64 et 68 ans, sans antécédents pathologiques, admis pour une pneumopathie Covid-19. Les trois patients ont reçu initialement une corticothérapie de 6 mg/j de dexaméthasone. Devant l’hypoxémie, la dose de corticothérapie a été augmenté chez deux patients (à 2 mg/kg/j de méthylprednisolone pendant 15 jours). Devant des symptomatologies différentes (polyurie pour la femme, dépendance des drogues vasoactives et hyponatrémie pour les hommes), un dosage de cortisolémie a montré la présence d’insuffisance surrénalienne (cortisolémie respectivement à 5 ng/ml, 73 ng/ml et 116 ng/ml). Un homme est décédé dans un tableau d’hypoxémie réfractaire et les deux autres patients étaient mis sortants sous hydrocortisone.
Discussion
La relation entre le SARS-COV (proche du SARS-cov2) et les glandes surrénales est bien prouvée (virus retrouvé dans les glandes surrénales lors des études autopsiques). Par conséquent, le SARS-Cov2 pourrait avoir un effet similaire sur l’axe hypothalamo-hypophysaire. De plus, les récepteurs ACE2 pour lesquels le SARS-cov2 a une grande affinité sont exprimés dans les surrénales. Actuellement, une étude chinoise est en cours (ChiCTR20000301150) pour évaluer la cortisolémie et l’ACTH au cours des infections sévères au Covid-19.
Conclusion
Bien que leur relation soit encore peu élucidée, l’IS doit être recherchée lors des infections sévères au Covid-19.
