Em outubro último, foi lançada a segunda edição em português de A entrevista psiquiátrica na prática clínica (Artmed, 2007), livro consagrado da literatura psiquiátrica, publicado originalmente em 1971. Os autores MacKinnon e Michels, agora juntamente com Peter Buckley, voltam à cena depois de 35 anos, com uma edição amplamente renovada e adaptada às vicissitudes dos tempos atuais.Conforme muito apropriadamente assinalado por Glen Gabbard em sua introdução, esse livro chega em boa hora. Sua primeira edição foi publicada em uma época em que os programas de formação de residentes em psiquiatria contemplavam, de forma natural, uma compreensão muito mais abrangente do paciente, visando conhecê-lo mais profundamente como pessoa. Já essa edição aparece dentro de um contexto de progressivo "desaprendizado" da arte da boa entrevista. O psiquiatra contemporâneo muitas vezes encontra-se preso a um reducionismo biológico, limitando suas entrevistas a inventários de sintomas que possam satisfazer critérios de manuais diagnósticos e, assim, orientá-lo a uma prescrição medicamentosa.Estruturalmente, o livro é dividido em quatro partes: 1) Princípios gerais; 2) Síndromes clínicas importantes;3) Situações clínicas especiais; e 4) Fatores técnicos que afetam a entrevista.A primeira parte traz dois capítulos que, embora introdutórios, são altamente relevantes, pois fornecem as bases fundamentais da entrevista. Apresentam conceitos e orientações bastante úteis ao entrevistador psiquiátrico, especialmente ao iniciante. O primeiro capítulo, sobre princípios gerais da entrevista, aborda, de modo bastante abrangente, temas como as características básicas da entrevista psiquiátrica, aspectos relevantes do exame psiquiátrico, bem como técnicas de condução da entrevista. O segundo capítulo introdutório, sobre princípios gerais da psicodinâmica, revela a ênfase central dos autores na importância do conhecimento (ao menos básico) do funcionamento psicodinâmico para o sucesso da entrevista. Aborda, de forma clara e simplificada, conceitos básicos da psicodinâmica e da psicanálise, bem como aspectos psicodinâmicos das condições psicopatológicas. Além disso, apresenta, de modo sumário, os principais modelos psicanalíticos do funcionamento mental.A parte do livro que mais se destaca, por sua grande abrangência e relevância à prática cotidiana do psiquiatra, é a que engloba diversos capítulos que abordam as principais síndromes clínicas com as quais o entrevistador normalmente se depara.
