We report 12-year experience in the accelerated treatment (AT) of postpneumonectomy empyema (PPE). There were 38 patients (7 females, 31 males) in age 19-80 years. 34 patients underwent pneumonectomy due to non-small cell lung cancer (NSCLC), 2 for other malignancies, and 2 for lung abscess. 19 right and 19 left pneumonectomies were performed. PPE was caused by bronchopleural fistula in 16 cases (42.1%) and by pleural infection in 22 patients (57.9%). The interval between first symptoms of PPE and AT ranged 1-47 months. The technique described by Schneiter et al. is based on repeated debridement/lavage of the postpneumonectomy cavity every second day performed a total of three times. 35 patients (92.1%) were free from empyema definitively. 4 of them required additional thoracomyoplasty and another 2 of them thoracostomy due to PPE recurrence. 1 patient (2,6%) died during hospitalisation and 2 (5.2%) didn't complete treatment and remained drain carriers. AT alone without additional procedures healed 29 patients (76.3%). Follow up time for the NSCLC group was 8-148 months (median 67). Cancer recurrence or second malignancy rate was 8/36 (22%). Accelerated treatment of PPE is safe and effective. It provides cure for the vast majority of patients without thoracoplasty. Patients with cancer and PPE tend to live longer than similar patients without PPE.
Zenker's diverticulum (ZD), an acquired false pharyngoesophageal diverticulum, is a rare pathology. The prevalence of ZD among the general population is believed to be between 0.01% and 0.11%. Most patients are elderly men with symptoms of dysphagia. The most common treatments are open surgical diverticulectomy with or without cricopharyngeal myotomy and endoscopic myotomy. The authors report the case of a dental prosthesis lodged within an asymptomatic ZD in a 73-year-old man. It was safely removed endoscopically. So far, there have only been single reports of entrapment of an enteroscopy capsule lodged in a ZD in medical literature.
This study shows that chronic inflammation plays a role in impaired HRQL in patients with cirrhosis irrespective of minimal HE.
StreszczenieRozedma pęcherzykowa jelit występuje rzadko. Jej częstość szacuje się na 0,03-0,2% przypadków w badaniach sekcyjnych. Jest łagodną chorobą charakteryzującą się występowa-niem zazwyczaj mnogich torbieli, wypełnionych gazem, w ścianie przewodu pokarmowego. Zwykle pojawia się w przebiegu innych chorób, nie tylko przewodu pokarmowego (postać wtórna -85% przypadków). Rzadziej występuje postać pierwotna (15% przypadków). Etiologia schorzenia nie jest znana. Objawy są mało charakterystyczne. Autorzy przedstawiają pacjenta diagnozowanego z powodu przewlekłej, łagodnej biegunki, u którego rozpoznano pierwotną postać rozedmy pęcherzykowej jelit. Podjęto próbę antybiotykoterapii. Uzyskano ustąpienie objawów klinicznych i zmian w obrazie endoskopowym. AbstractPneumatosis cystoides intestinalis (PCI) is a rare disease. Its incidence varies from 0.03% to 0.2% in serial autopsy studies. It is defined as air-filled cysts within the wall of the gastrointestinal tract. Primary PCI is extremely rare (15%); in most cases PCI is due to an underlying disease or condition (secondary form -85%). The pathogenesis of PCI is unknown. Patients either remain asymptomatic or present with non-specific gastrointestinal symptoms. The authors present a case of a patient with mild, chronic diarrhoea. A diagnosis of primary pneumatosis was established. Treatment with antibiotics was initiated. Clinical improvement was achieved. A repeat colonoscopy revealed complete remission. Spotykane w piśmiennictwie inne określenia tego schorzenia to odma śródścienna jelit lub pneumatoza. Jest łagodną chorobą charakteryzującą się występowaniem zazwyczaj mnogich, przeświecających torbieli (ze względu na obecność w nich gazu) w ścianie przewodu pokarmowego. Mogą się one lokalizować praktycznie w każdym odcinku przewodu pokarmowego, jednak najczęściej zajmują jelito cienkie (42% przypadków) [2][3][4][5]. Etiologia tego schorzenia nie jest znana. Powstawanie pęcherzy próbuje się tłumaczyć dwiema teoriami: mechaniczną oraz -bardziej prawdopodobną -bakteryjną. Rozróżnia się dwie postaci pneumatozy: pierwotną, czyli idiopatyczną, oraz wtórną -występują-cą w przebiegu wielu chorób, zarówno przewodu pokarmowego, jak i innych narządów. W większości przypadków postać pierwotna choroby przebiega bezobjawowo. Wyjątkowo dochodzi do rozwoju objawów "ostrego brzucha". W postaci wtórnej przeważa obraz choroby podstawowej (zwykle o ciężkim przebiegu). Obecnie rozpoznanie pneumatozy ustala się na podstawie radiologicznych badań obrazowych (zdjęcie przeglądowe -RTG, tomografia komputerowa -TK) lub badań endoskopowych przewodu pokarmowego.Od postawienia właściwej diagnozy zależy dalsze postępowanie z chorym. W przypadku pierwotnej pneumatozy zazwyczaj wystarcza postępowanie zachowawcze. Oporne lub powikłane przypadki wymagają leczenia
