Objetivo: analisar os protocolos de enfermagem como um possível instrumento para a qualidade da prática do enfermeiro na Atenção Primária à Saúde.Método: estudo descritivo e qualitativo, realizado por meio de análise documental e entrevistas semiestruturadas, com 14 enfermeiros das Regiões Metropolitanas I e II do estado do Rio de Janeiro. Para o tratamento dos dados, foi utilizada a análise temática de conteúdo de Bardin.Resultados: a categoria analisada neste artigo foi o “lugar do cuidado”, a qual aborda a qualidade do cuidado nas ações dos enfermeiros e a importância desta para a profissão.Conclusão: a construção coletiva e dialogada são fatores motivadores para elaboração de protocolos locais. Além de ser uma estratégia de educação permanente, os enfermeiros reconheceram os protocolos como ferramentas para tomada de decisão, proporcionando-lhes apoio técnico e respaldo ético, o que contribui sobremaneira para a construção de conhecimento e o desenvolvimento de boas práticas em enfermagem.
RELATO DE EXPERIÊNCIA Desafios para gestão do cuidado no pré-natal durante a pandemia da COVID-19: um relato de experiênciaChallenges for prenatal care management during the COVID-19 pandemic: an experience report Desafíos para la gestión de la atención prenatal durante la pandemia de COVID-19: un informe de experiencia Resumo Objetivou-se descrever os desafios gerenciais para gestão do cuidado no pré-natal durante a pandemia da COVID-19. Trata-se de um relato de experiência sobre os desafios e estratégias gerenciais no cuidado prénatal durante a pandemia da COVID-19 em uma área na Zona Oeste, RJ. No contexto da APS, visando a adequada gestão do cuidado ao pré-natal, ações foram implantadas e estratégias desenvolvidas para manutenção dos acompanhamentos das gestantes, sendo encontradas barreiras que dificultavam a gestão compartilhada. Surgindo a necessidade de adequações nos fluxos assistenciais para atendimentos de casos de síndrome gripal, suspeitos de COVID-19, a qual gerou a emergente demanda de reorganização dos fluxos e processos de trabalho pelo alto contágio da doença. Devido à necessidade de manter distanciamento social, as práticas clínicas foram parcialmente atendidas com as devidas adaptações para a segurança das gestantes, e foi preservado seu atendimento, uma vez que a suspensão poderia culminar em perda de oportunidades terapêuticas à mulher, ao bebê. Portanto, as condutas praticadas e descritas na experiência demonstraram a aproximação das propostas da gestão da qualidade e da integralidade do cuidado, permitindo, mesmo com um acesso de forma adaptada, atingir as metas de garantia da manutenção e qualificação da assistência pré-natal.
Objective To identify and analyze the weaknesses of the nurse's practice in meeting spontaneous demands in primary care units in the city of Rio de Janeiro. Method Qualitative study carried out in Rio de Janeiro, in 2016, with 20 nurses recruited unintentionally. The focus group and simple observation were applied, and data were subjected to thematic content analysis. Results The spontaneous demand service causes tensions and work overload. Nurses and community health agents are primarily responsible for organizing access. Final considerations There was a lack of understanding of the practice of spontaneous demand as part of care management, in addition to a polysemy related to the term. While for some, welcoming practices means greater autonomy, incorporating a larger scope of actions related to care and expanding their clinical practice; for others, they represent a stage that precedes medical consultation and a disorganizing element of care.
Relato de prática pedagógica sobre a temática “melhores práticas de enfermagem na APS”, realizado no 28° Fórum Catarinense das Escolas de Enfermagem e promovido pela Associação Brasileira de Enfermagem (ABEn) - Seção Santa Catarina. A Oficina “Atenção Primária em Saúde: a enfermagem em foco” buscou refletir sobre a qualidade das práticas do enfermeiro na Atenção Primária à Saúde (APS). Foi estruturada em três momentos/questões disparadoras: (1) O que o grupo compreende sobre melhores práticas em enfermagem na APS?; (2) O que existe documentado sobre melhores práticas em enfermagem na APS? (3) Como qualificar a prática clínica da enfermagem para que ela esteja filiada aos pressupostos das melhores práticas? A partir de metodologias ativas e reflexões, a Oficina contribuiu para a reflexão e, possivelmente, terá algum impacto na ação dos estudantes e profissionais envolvidos. O conhecimento técnico-científico; o domínio de instrumentos de cuidado, informação e gestão; o desenvolvimento de competências profissionais e a participação em movimentos de educação permanente são alguns elementos que oportunizam a trocas de saberes e experiências entre enfermeiros e constituem requisitos primordiais para o desenvolvimento de um cuidado de qualidade, com vistas as melhores práticas.
De acordo com a International Association for the Study of Pain, dor é uma sensação ou experiência emocional desagradável, associada com dano tecidual real ou potencial. É classificada em aguda ou crônica quando tem duração inferior ou superior a 30 dias. Segundo a portaria n°1083 de 2012, lançada pelo Ministério da Saúde, o Protocolo Clínico e de Diretrizes Terapêuticas da Dor Crônica, inexistem dados disponíveis no Brasil sobre a prevalência de dor crônica. No entanto, podemos perceber uma alta procura por atendimento relacionada a queixas osteoarticulares na Atenção Primária (APS). O caderno de Atenção Básica de Demanda Espontânea sugere que o atendimento aos usuários com dor crônica na APS seja realizado com o apoio dos profissionais do Núcleo de Apoio a Saúde da Família (NASF), pois o cuidado multiprofissional e interdisciplinar é potente para agregar recursos e contribuir para intervenções com maior resolutividade para o usuário, além de, capacitar as equipes de referência (MS, 2013). Descrever a experiência do grupo da Dor realizado em uma Unidade de Saúde. Relato de experiência. O grupo possui 70 usuários, de ambos os sexos, divididos em duas turmas quinzenais. São direcionados para o grupo por encaminhamento interno pelas equipes de saúde da Unidade. O critério para participação no grupo da Dor é: ter dor a mais de três meses, sem resposta terapêutica às intervenções da equipe de referência. Os usuários que atualmente participam do grupo tem entre 40 e 80 anos, sendo a maioria do sexo feminino. A atividade ocorre na Clínica da Família Felippe Cardoso, localizada no município do Rio de Janeiro. Iniciado em 2011, o grupo atualmente é coordenado pelo fisioterapeuta do NASF e pela enfermeira residente da Unidade, ocorre todas as segundas das 14h às 17h. Ao longo das três horas há a realização de exercícios, nas quais os usuários são estimulados a perceber suas estruturas corporais e a importância do autocuidado para a manutenção de suas funcionalidades. Em um segundo momento, há a formação de uma roda para dar as boas vindas aos novos integrantes, e aqueles que já fizeram parte dos encontros anteriores falam a respeito do estado de saúde antes e após a entrado no grupo. Posteriormente, há a discussão de temas de saúde escolhidos previamente pelos usuários. Dentre eles: estrutura corporal, chikungunya, alimentos com propriedades anti-inflamatórias, risco de queda, uso de compressas quentes e frias. Ao final do encontro realizamos exercícios com bozu, cama elástica ebola suíça para estimular o equilíbrio e a socialização através de atividades lúdicas. Sabe-se que mesmo com o crescimento da cobertura da APS no Brasil, as práticas em saúde estão focadas nas intervenções medicamentosas e nos encaminhamentos para os serviços especializados. Dessa forma, entendemos que APS é um modelo de atenção potente na desmedicalização do atendimento aos usuários com dor crônica. Por ter como princípios o cuidado longitudinal e integral à saúde, além de proporcionar um cuidado multiprofissional aos usuários o grupo atua como uma ferramenta excelente para aplicabilidade destes princípios e construção de espaços coletivos de autocuidado.
