Progressive posterior cortical dysfunction (PPCD) is an insidious syndrome characterized by prominent disorders of higher visual processing. It affects both dorsal (occipito-parietal) and ventral (occipito-temporal) pathways, disturbing visuospatial processing and visual recognition, respectively. We report a case of a 67-year-old woman presenting with progressive impairment of visual functions. Neurologic examination showed agraphia, alexia, hemispatial neglect (left side visual extinction), complete Balint’s syndrome and visual agnosia. Magnetic resonance imaging showed circumscribed atrophy involving the bilateral parieto-occipital regions, slightly more predominant to the right. Our aim was to describe a case of this syndrome, to present a video showing the main abnormalities, and to discuss this unusual presentation of dementia. We believe this article can contribute by improving the recognition of PPCD.
Conjugate eye deviation is characterized by a sustained shift in horizontal gaze, usually toward the affected brain hemisphere. When detected on neuroimaging, it is called the “eye sign”. It is classically associated with lesions involving the frontal eye fields, ipsilateral to the side of the deviation. Neglect may be conceptualized as a spatially addressed bias of the sensory events in explicit behaviors and in the absence of perceptual and motor deficits. Hemispatial neglect is a common disabling condition that occurs following acute unilateral brain damage, usually to the right side. We report a case of a patient presenting with the “eye sign” on tomography, following an acute subinsular stroke, in the absence of conjugated eyes deviation. Our hypothesis was that the sign may have been due to hemispatial neglect in this patient. The aim of this article was to discuss the mechanisms involved in the attention network and its neuroanatomic correlates.
Paciente, sexo feminino, sete anos de idade, proveniente de Campo Grande -MS. Queixa-se de aumento progressivo da região mandibular esquerda há quatro meses. Nega outros sinais, sintomas ou antecedentes traumáticos. Ao exame, observa-se abaulamento mandibular esquerdo, endurecido e indolor à palpação, sem sinais flogísticos locais. Não há evidência de lesões na mucosa oral ou infonodomegalias. Movimento mastigatório preservado. Trata-se de uma criança previamente hígida, com adequado desenvolvimento neuropsico-motor.Exame solicitado: TC de face. Biópsia: Tumor de células gigantes. O TCG ou osteoclastoma é uma neoplasia óssea benigna em 95% dos casos, porém localmente agressiva, sendo histologicamente caracterizada por células gigantes multinucleadas dispersas no tecido tumoral, cujo núcleo apresenta a mesma característica das células ovóides e fusiformes que formam o seu estroma. Representa de 5% a 8% de todos os tumores ósseos primários, cerca de 60% destas lesões ocorrem na extremidade articular de ossos longos, como tíbia proximal, fêmur distal, rádio distal e úmero proximal. A maioria dos pacientes possui entre 20 a 40 anos de idade, com predomínio do sexo feminino ( 2:1). A incidên-cia relatada em crianças abaixo de 10 anos é de 3% e em locais menos comuns, como a mandíbula, cerca de 7%; sua forma multifocal é rara, sendo observada em 0,5% dos casos.Clinicamente, a dor local, de intensidade variável, é o sintoma mais frequente, comumente associada a aumento volumétrico da região afetada, às vezes com evolução de poucas semanas.Os aspectos radiológicos clássicos o definem como uma lesão puramente lítica, insuflativa, excêntrica, que promove afilamento ou erosão cortical. Reação periosteal é descrita em menos de um terço dos casos.A cintilografia óssea pode ter um importante papel na caracterização da lesão, demonstrando ampla concentração do radioindicador na lesão com fotopenia central, "sinal da rosquinha".A ressonância magnética pode ser útil para avaliar sua extensão para partes moles.Entre seus diagnósticos diferenciais, devem ser incluídos o cisto ósseo aneurismático, histiocitoma fibroso benigno, granuloma reparador de células gigantes (pós-traumático), tumor marrom do hiperparatireoidismo, condrossarcoma com extensão articular, mieloma e metástases líticas.
