Background: Critically ill patients have intense muscle tissue mobilization, and attenuating protein catabolism may contribute to improved outcomes.Objective: To evaluate short-term muscle loss in critically ill patients. Methods:In this prospective observational study, we evaluated the thickness of the rectus femoris muscle by ultrasonography in young and older adults of both sexes admitted to a cardiological intensive care unit within 48 h of admission (baseline) and after 7 days. The results were compared and correlated with anthropometric, clinical, and biochemical parameters. The significance level for all statistical analyses was 0.05. Results:The final sample comprised 88 patients with a mean age of 66.2 ± 11.8 years. There was an average 13.5% reduction in rectus femoris muscle thickness over the study period (P < 0.001), regardless of the thigh circumference maintenance (P = 0.229). This reduction occurred even with improved clinical parameters (C-reactive protein, Simplified Acute Physiology Score) and was greater in patients receiving mechanical ventilation and sedation and in those who died. Regarding nutrition status, malnourished and eutrophic individuals showed greater muscle loss than overweight individuals. There was also an inverse correlation of muscle loss (percentage) with body mass index, arm circumference, and calf circumference (P < 0.05), demonstrating that the lower these anthropometric measurements, the higher the muscle loss obtained by ultrasound. Conclusion: Ultrasonography assessment detected muscle mass loss in the short-term more sensitively than the anthropometric method. However, it demands caution and further studies demonstrating this analysis.
Objetivo: este trabalho avaliou o estado nutricional e a capacidade funcional de pacientes com síndrome da imunodeficiência adquirida hospitalizados. Metodologia: Estudo transversal e descritivo realizado nas enfermarias de Infectologia, com pacientes HIV (Vírus da Imunodeficiência Humana), com idade igual ou superior a19 anos (n = 87). As variáveis analisadas foram: sexo, idade, desfecho, tempo de internamento, diagnóstico secundário, presença de comorbidade, peso, estatura, índice de massa corpórea (IMC), circunferência do braço, circunferência muscular do braço, área muscular do braço, dobra cutânea triciptal, força de preensão manual e espessura do músculo adutor do polegar. Resultados: No estudo, 64,4% dos pacientes eram do sexo masculino, com idade média de 39,24±10,45 anos. O risco nutricional foi observado em 64,4% dos casos. Houve prevalência de desnutrição, segundo o IMC, em 25,3% dos pacientes, mas essa prevalência alcançou 78,2%, pela dobra cutânea triciptal, e 62,2% pela circunferência muscular do braço. O tempo de internamento foi maior nos pacientes com risco nutricional (77,1%), que permaneceram internados por mais de 21 dias (p=0,041). A força de preensão manual e a espessura do músculo adutor do polegar foram menores nos pacientes com menor IMC. Conclusão: Houve alta prevalência de risco nutricional, desnutrição e baixo desempenho dos métodos funcionais nos pacientes hospitalizados com HIV. A espessura do músculo adutor do polegar e a força de preensão manual tiveram associação estatisticamente significativa como IMC, demonstrando ser bastante úteis na avaliação da capacidade funcional de pacientes com síndrome da imunodeficiência adquirida.DOI: 10.12957/demetra.2019.34792
Introdução: A desnutrição é frequente em pacientes cardiopatas hospitalizados e sua identificação precoce é crucial para instituir o tratamento nutricional adequado e prevenir seu agravo. A desnutrição é associada ao aumento da taxa de hospitalizações, maior prevalência de complicações, infecções e mortalidade. O índice de massa corporal é comumente utilizado para avaliação nutricional, porém, não é capaz de identificar a depleção de massa magra. Objetivos: Avaliar a prevalência de desnutrição de peso normal em pacientes cardiopatas internados em um hospital de referência cardiológica. Métodos: Estudo transversal e analítico, realizado com pacientes de ambos os sexos e idade maior ou igual a 20 anos. A avaliação do estado nutricional foi determinada pelo índice de massa corporal e bioimpedância elétrica. Classificou-se como desnutrição de peso normal os pacientes portadores de eutrofia pelo índice de massa corporal, associado a um baixo índice de massa muscular apendicular esquelética. Foi adotado um nível de significância de p<0,05. Resultados: A amostra foi constituída por 77 pacientes, com média de idade de 61,7 ± 13,8 anos, maior proporção do sexo feminino (55%) e idosos (62%). A prevalência de desnutrição de peso normal foi de 64%, apresentando associação significativa com a faixa etária (p=0,011) e tempo de escolaridade (p=0,021). Conclusão: A desnutrição de peso normal mostrou ser uma realidade entre pacientes cardiopatas, destacando-se a necessidade de adotar métodos de diagnóstico nutricional mais completos que auxiliem na avaliação da composição corporal e estado nutricional destes pacientes.
