The Family Health Strategy has been implemented in Brazil with the aim of reorganizing the health care system with an emphasis on promotion and prevention. Thus, encouragement for breastfeeding is one of the most important activities by family health care teams. The objective of this study was to assess knowledge and practices by family health care teams in relation to breastfeeding. In addition to a questionnaire on health professionals' characteristics, a specific knowledge test on breastfeeding was conducted with 41 physicians and nurses and another test on the same subject was given to 152 community health workers. Scores were above 80% on the advantages of breastfeeding in all the teams, but knowledge on practical issues concerning technique and management of the main problems encountered in breastfeeding was not uniform among the health professions. Nurses scored better than physicians, and community health workers had the lowest scores. Practical activities to promote breastfeeding were not developed uniformly among health care professionals. There is a need for systematic and ongoing training in breastfeeding management in the family health teams.
OBJETIVO:A iniciativa Unidade Básica Amiga da Amamentação representa um conjunto de atividades educativas dirigidas às unidades básicas de saúde. O objetivo do estudo foi avaliar a efetividade desta estratégia de promoção do aleitamento materno dirigida às equipes do Programa de Saúde da Família. MÉTODOS:Conduziu-se um estudo de intervenção controlado com 20 equipes do Programa de Saúde da Família, selecionadas aleatoriamente em Montes Claros (MG) em 2006. O grupo sob intervenção realizou programa de treinamento específi co de 24 horas para a promoção do aleitamento materno segundo a "Iniciativa Hospital Amigo da Criança". Enfatizou-se a assistência do profi ssional de saúde no suporte à amamentação e no manejo dos principais problemas da lactação. O grupo controle recebeu orientações habituais sobre aleitamento materno. As mães de todas as crianças menores de dois anos de idade assistidas pelas equipes foram entrevistadas antes (n=1.423) e 12 meses após a intervenção (n=1.491) e responderam questões sobre a prática da amamentação. Curvas de sobrevida do aleitamento materno foram construídas e comparadas para os dois momentos por meio do teste log rank. RESULTADOS:Houve aumento significativo no aleitamento materno exclusivo após atividades educativas voltadas às equipes de Saúde da Família. As curvas de sobrevidas para o aleitamento materno exclusivo no primeiro momento não mostraram diferença estatisticamente signifi cativa entre as mães assistidas por ambos os grupos (p=0,502). Após a intervenção, as curvas de sobrevida para o aleitamento materno exclusivo mostraram-se signifi cativamente diferentes (p=0,001). CONCLUSÕES:O treinamento das equipes de Saúde da Família da forma como propõe a Iniciativa Unidade Básica Amiga da Amamentação mostrou ser uma estratégia efetiva e de baixo custo para sensibilizar esses profi ssionais, uniformizando as informações e assegurando o apoio necessário para as mães com difi culdades para amamentarem seus fi lhos.
The training of Family Health Program teams as proposed by the Baby-Friendly Hospital Initiative proved to be an effective, low-cost strategy for raising awareness among health providers, providing consistent information, and assuring the required support to mothers with breastfeeding issues.
Introdução: No Brasil, o câncer figura como a segunda causa de morte na população. O diagnóstico precoce exerce significativa influência sobre os índices de mortalidade, visto que um dos fatores que dificultam o tratamento é o estágio avançado em que a doença é descoberta. É importante que os estudantes conheçam as ações de detecção precoce recomendadas pelo Instituto Nacional de Câncer (INCA), pois os médicos recém-formados são aqueles que estarão na linha de frente social, podendo aplicá-las na prática e, assim, minimizar o avanço dessa doença. Objetivo: Verificar o conhecimento que os estudantes de medicina têm sobre as estratégias de detecção precoce para os cânceres de mama, próstata, pele, pulmão, colo do útero e colorretal, com base nas recomendações do INCA. Métodos: Estudo transversal realizado por meio de questionário sobre as ações de detecção precoce adotadas pelos estudantes dos 5º e 6º anos de medicina da Universidade Estadual de Montes Claros (MG). Os dados foram analisados no EPI-INFO. Resultados: De um total de 90 estudantes que estavam matriculados nos 5° e 6°anos de medicina, 66 participaram do estudo. Os cânceres os quais os estudantes julgaram mais incidentes foram: próstata (96,9%), colo uterino (93,9%) e mama (93,4%). Conclusão: A maioria dos estudantes indicou medidas de detecção precoce para os cânceres de próstata e pulmão que não são recomendadas pelo INCA, para a população em geral. Os acadêmicos revelaram possuir um bom conhecimento sobre as estratégias de detecção precoce para os cânceres de mama, colo uterino e pele. A maioria dos estudantes não distinguiu sinais precoces dos cânceres citados nas manifestações tardias.
